    Compassionate, attentive, clean, secure care

I'm very pleased with the caring, attentive staff and organized management - the director of nursing and team truly go above and beyond. The facility is clean and pleasantly scented, secure, easy for drop-off/pick-up, and offers strong rehab and hospice services with clear communication; overall a very good, compassionate place I trust for care.

    Summary of reviews

Reviewers describe a facility with a mixed profile: there are repeated notes of compassionate, courteous caregivers and functioning clinical services (notably rehabilitation and hospice), but there are also consistent operational weaknesses that affect care continuity and family confidence.

Care quality appears variable. Positive accounts highlight attentive nurses, cooperative therapy teams, and successful rehabilitative discharges. Contrasting accounts raise concerns about clinical responsiveness, including delays in addressing infections and pressure-related skin conditions, and instances where therapy or clinical services were interrupted or inconsistent. Several descriptions point to difficulties locating nursing or aide staff on shifts and to reliance on agency personnel, which reviewers perceive as contributing to uneven care continuity.

Staff and management impressions are mixed. Some families praise a well-organized management team and an engaged director of nursing who prioritize compassionate care and good internal teamwork. Other reviews describe limited administrative visibility, difficulties reaching leadership, and indications of staff morale or workplace-culture problems. There are also notes that an ownership change prompted visible updates and improvements in some areas, suggesting recent management efforts to address concerns.

Dining, activities, and daily living supports show inconsistent performance. Reported issues include menu items not matching posted menus, omissions from meal service, and limited accessibility of beverages. Activity programming and communal engagement are described as sparse by several reviewers; an older, quieter environment with restricted movement or curfew-like expectations may not meet the needs of more active seniors.

Physical environment and safety observations are mixed as well. The facility is described as secure and conveniently accessible for families, with some reviewers noting pleasant indoor fragrance and improved maintenance after ownership change. At the same time, the building’s appearance is often characterized as dated, and there are sanitation inconsistencies in resident rooms and shared areas. More serious concerns include allegations of theft or property-security lapses, which underscore the need for clear property-handling policies and family communication.

For prospective residents and families: the facility offers demonstrable strengths in rehabilitative care, hospice support, and staff members who are experienced and caring. However, the variability in staffing, clinical responsiveness, sanitation consistency, and activity programming suggests an on-site assessment and focused questioning are important. Recommended topics for inquiry include current staffing stability and agency use, discharge and therapy-continuity procedures, recent survey results and what changes followed, room sanitation practices, property-security measures, and opportunities for social and activity engagement. These targeted questions can help determine whether the facility’s current performance aligns with a specific resident’s care and lifestyle needs.

About LaVerna Manor Health & Rehabilitation

LaVerna Manor Health & Rehabilitation in Savannah, Missouri, gives seniors different levels of support all in one place, starting with independent living and going all the way up to full nursing care, and folks can move through these as their needs change, so some stay in small board and care homes in a neighborhood-like setting and get daily help, while others need more nursing support or might use the bigger skilled nursing area where there's everything from wound care to full rehabilitation services. There's a strong focus on memory care, with secure environments and 24-hour supervision, so seniors with Alzheimer's or dementia get special attention, memory-friendly activities, and safe spaces, and families can count on staff to share updates and answer questions about care. The rooms come furnished with private bathrooms, cable TV, air conditioning, kitchenettes, and Wi-Fi, while the building stays clean and comfortable, smelling nice and welcoming, and decorated for holidays or special events by staff who try to brighten things up. Meals are served restaurant-style in a dining room, and the kitchen staff can handle special diets, whether for allergies or diabetes, which is important for a lot of folks. There's help for daily needs like bathing, dressing, medication, or getting around, and a 24-hour call system means help is always close by. Activities fill up the days, from bingo and arts and crafts to music, church services, walks on outdoor paths, movie nights, and outings on the community bus, plus there's a therapy dog named Miss Ruby who visits for companionship. Residents can enjoy time in the library, fitness room, or outside in gardens or common areas, and the place has its own movie theater along with resident-run events. There's laundry and housekeeping, and move-in help if needed. A clinical team works to find out what each person needs and makes a plan, and they adjust care along the way, always aiming to help people get stronger, do more for themselves, or just feel better day-to-day. Medicare and Medicaid cover care here, and the facility has space for 66 residents at a time. Staff speak plainly and work to keep people treated with dignity, showing respect for families and residents. The facility runs between 8 am and 8 pm daily for visitors, and there's always support around the clock for those in memory care or with other high needs.
