    Supportive professional staff enabled recovery

I spent two months in rehab at Ivy@Greatfalls and was very pleased with the supportive, professional staff. Kim in admissions, the nurses, CNAs, therapists and social worker were attentive, communicative and hands-on - therapy helped a smooth recovery and my family got frequent updates. The facility feels clean, home-like and family-oriented with engaging activities; I would use Ivy again and recommend it.

    Summary of reviews

Reviews of The Ivy at Great Falls describe a polarized set of experiences, with distinct strengths in short-term skilled nursing and therapy alongside recurring operational concerns in some long-term care areas. Many families praised the rehabilitation and therapy teams for effective recovery work, noting clean and spacious rehab rooms, attentive physical therapists, and positive therapy outcomes. Multiple comments highlight compassionate nursing staff, approachable admissions and administrative personnel, and proactive social-work assistance with discharge planning and Medicare — all factors that support smoother short-term stays and transitions.

At the same time, several reviews raise concerns about persistent staffing pressures and their downstream effects on care. Reported patterns include slow responses to nurse-call systems, delays in medication administration and procedures such as catheter care, and inconsistent personal-care routines (including bathing and grooming schedules). Reviewers describe staff as generally kind and hardworking but stretched thin; that combination appears to produce reliable care at times and gaps in responsiveness or clinical timeliness at others.

Facility and amenity feedback is mixed. The therapy and activities programs receive consistent praise for engaging residents and involving families; many reviewers highlighted social programming, life-skill initiatives, and a welcoming atmosphere in parts of the building. Conversely, other comments describe uneven facility upkeep — dark or sparsely equipped rooms, removed televisions, missing personal photos or items, and intermittent supply shortages. These differences suggest variability between units and periods of occupancy or maintenance rather than uniform conditions across the campus.

Management and communication show similar variability. Several families reported clear, frequent updates, hands-on administration, and helpful admissions staff who facilitated admission and coordination. Others experienced poor phone responsiveness, scheduling issues for staff coverage, and concerns about personal-item security. A recurring operational theme is inconsistency: while some shifts and departments function smoothly, others exhibit gaps in training, scheduling, and safety oversight that can affect resident experience and family confidence.

For prospective residents and families, the pattern suggests The Ivy at Great Falls can provide high-quality short-term rehabilitation and has many caring employees, but it may also experience episodic performance issues tied to staffing and unit-level management. Recommended due diligence includes touring the specific unit a resident would occupy, asking about current staffing ratios and nurse-call response protocols, inquiring about personal-item policies and inventory procedures, clarifying hygiene and bathing schedules, and discussing therapy goals and expected timelines. These targeted questions can help align expectations with the variable experiences described in reviews.

About The Ivy at Great Falls

The Ivy at Great Falls is a large nursing home in Great Falls, Montana, that has 278 beds and offers both short-term rehabilitation and long-term nursing care for seniors, and although it's not a continuing care retirement community, it does have memory care services for people with Alzheimer's and dementia, and the building is carefully monitored with activities designed to keep residents engaged while staff provide individualized wellness care and help with daily needs. The facility focuses on creating a safe and caring environment, brings in qualified healthcare professionals, and provides tailored rehabilitation if someone needs help recovering after surgery or an injury, with flexible respite care available by the day, week, or month. Although The Ivy at Great Falls has these services and says it aims for high standards, the facility has faced serious challenges over the years, having been cited by federal agencies multiple times for deficiencies in healthcare and fire safety, and after inspections in October 2023, January 2024, and March 2024, inspectors found 20, 3, and then 12 different deficiencies, respectively. Because of ongoing quality issues, the Ivy at Great Falls got put on the State-Focus Facility program list, which is meant to help places with chronic problems improve, but after failing to meet those requirements-like not achieving "graduation" status in the last inspection-it lost its certification for both Medicare and Medicaid, and the federal government fined the home $17,872 after the October 2023 inspection. With a provisional state license to help residents move out safely and programs like temporary management and transitional support in place, the facility is right now working on moving residents elsewhere, and while the Ivy participates in the Money Follows the Person program to support Medicaid residents who want to return to more community-based living, right now it can't take new residents through the main federal health programs. The Ivy at Great Falls doesn't provide a lot of public details about its services or features beyond what's listed, but it's known as a for-profit, corporate-owned facility that says it wants to offer personalized and supportive care for seniors facing various health needs.
