    Caring staff restored my mobility

I stayed here for rehab and was very pleased. The staff were kind, professional, and genuinely caring - nurses, med aides and the therapy team (Ethel, Taylor, Anna, Lauren) got me walking again with less need for a walker. The facility is beautiful, spotless, secure, and lively with great activities; admin and security were friendly and helpful. My family felt informed and comforted - I would definitely recommend it.

    Summary of reviews

Reviews of Emerald Nursing and Rehab Mercy present a mixed and often polarized picture, with a clear pattern of strengths concentrated in the therapy and some nursing teams, and recurring operational weaknesses affecting day‑to‑day care. The facility's rehabilitation services receive frequent praise for helping residents regain function and return home; several accounts single out individual therapists and therapy teams as particularly effective contributors to recovery. Many families also note welcoming reception staff and an active activities program that supports social engagement.

At the same time, a substantial number of accounts describe persistent care‑delivery problems. Prominent concerns relate to medication management and documentation, with delays, missed administrations, and incomplete med records linked to emergency transfers. Call‑response times and family communication are often described as unreliable; reviewers report long waits for attention, limited routine updates, and difficulty reaching staff by phone. Wound care, bathing schedules, and incontinence‑care processes are also areas where reviewers describe inconsistency, creating infection‑control and skin‑care worries in some cases.

Dining and nutrition emerged as another variable domain. While some reviewers praise meal presentation and adequate nutrition, others describe interrupted meal service, cold or limited meals, and occasional shortages that affect residents' food intake. Facility‑level housekeeping and environmental maintenance likewise show divergence: several families report clean, odor‑controlled common areas and recent renovations, while others report laundry and sanitation concerns, pest‑control issues in parts of the building, and aging infrastructure.

Staffing and management practices are central to many complaints. Reviewers describe high turnover, reliance on agency staff, and uneven staff training or accountability; these factors are tied to reports of inconsistent care, supply shortages, and broken or missing equipment. Communication and administrative transparency are additional pain points—families report confusing or delayed discharge planning, unexpected out‑of‑pocket financial requests, and at least one allegation of theft and other financial disputes. There are also repeated references to challenges during infection‑control events (including COVID isolation), such as restricted visitation, limited updates, and delays in care coordination.

Despite these issues, the facility receives notable positive feedback from families whose loved ones experienced effective nursing and therapy, respectful interpersonal care, and successful returns home. Patterns in the reviews suggest that resident experience may depend heavily on unit assignment, shift, and which staff members are on duty. Prospective residents and families should consider visiting multiple units, asking detailed questions about staffing ratios, medication policies, wound and incontinence care protocols, infection‑control procedures, discharge planning, and financial/billing practices. Where possible, seek specific references for therapy staff and nursing teams, and confirm what improvements management has implemented in response to past concerns.

About Emerald Nursing and Rehab Mercy

Emerald Nursing and Rehab Mercy sits at 7410 Mercy Rd in Omaha and offers care for people who need skilled nursing or help recovering after a stay in the hospital, and you'll notice right away they focus on making things a little easier for those who need constant medical support, with 174 certified beds and an average of 101 residents most days, and though the place is owned by River City O with a handful of other indirect owners like Jw River City Llc and Yc River City Llc, and managed by Emerald Healthcare Llc along with Evolve Therapy Services and some others, the presence of longtime staff-some with several decades of experience-means you'll see familiar faces if you visit often enough, folks like Amy Fish, David Fleischmann, and a few more who manage day-to-day, and even though nurse turnover remains high at 79.1%, quite a bit higher than the state's average, and the nurse hours per resident per day come in a bit below average at 3.44, there's a clear commitment to round-the-clock care with a physician in the building all week and on-call for weekends, and the care teams, including physical, occupational, and speech therapists, get together with families to set up individual care plans, plus social services and dietary help all get involved, and the folks here offer different levels of nursing and therapy and even home-grown therapy with the same department leading the programs daily, while amenities like big private rooms and patio areas for fresh air, plus personalized dining with gourmet menus, help to support comfort and recovery, but you do have to know, their last inspection in March 2025 turned up some deficiencies, with 50 total, including three related to infection control, one of which pointed out failures in infection prevention that could've caused more than minimal harm, and there were also problems storing, preparing, and serving food correctly; at the moment, they're not taking new residents, and the services, amenities, and providers can change day to day, but the staff speaks English, they offer services tied to skilled nursing and rehabilitation, and their information gets updated in the online directory about once a month, so while there's no perfect facility, and certainly this one has had its share of inspection findings, it stays focused on serving the greater Mercy Region for those who need ongoing care or support for getting back on their feet.
