    Pleased with care and management

I placed my mom here and I'm very pleased. Staff are friendly and attentive, activities (bingo and other events) are outstanding, and the facility is spotless with clean clothes provided daily. New management has made great changes, residents seem happy and well cared for, and the food and large dining room are nice - it gives our family real peace of mind.

    Summary of reviews

Reviews for this facility are mixed, with a clear split between accounts of strong person-centered care and descriptions of persistent operational problems. Many families highlight compassionate nurses and attentive direct-care staff who provide daily personal care, clean clothing, and a sense of security for residents. At the same time, other reviewers describe systemic issues that affect the consistency of that care.

Care and staffing present a contrast: positive comments emphasize caring nursing interactions and attentive caregiving, while negative comments point to inconsistent staffing levels, delayed responses to call lights, and breakdowns in communication with families. Several accounts describe falls, weight loss, and clinical decline; these items suggest gaps in monitoring, nutrition oversight, and timely family notification rather than isolated interpersonal problems. Prospective families should verify current staffing ratios and incident-reporting practices during a visit.

Food and activities are also described variably. Some reviewers praise the dining room environment and certain meals, as well as an active schedule (bingo and other events) that contributes to resident engagement. Others report unappealing meals, inconsistent service, and limited individual participation in activities. The activity program may be effective in parts of the facility but constrained by staffing or resident mobility in others.

The physical plant and administration raise a number of operational concerns. Several reviews reference small, crowded rooms with shared bathrooms, deferred maintenance, and sanitation or infection-control lapses in particular areas. There are multiple mentions of missing personal items and financial/billing issues, indicating weaknesses in property security and administrative processes. A subset of reviewers attribute improvements to recent management changes, noting better responsiveness and a more positive environment since those changes.

Notable patterns to weigh: the facility appears capable of delivering compassionate, competent care in some units or under certain staff, but reliability is uneven across shifts and areas. Issues cluster around staffing consistency, communication with families, property security, nutrition monitoring, and facility maintenance. For families considering this facility, recommended due diligence includes an in-person tour of the specific unit, review of staffing schedules, written policies for personal-property and incident reporting, clarification of billing procedures, and discussions about nutrition and fall-prevention plans. Asking to speak with current residents and family members and verifying documented improvements under newer management can help assess whether recent changes have addressed the operational weaknesses described.

    About Continental Springs

Continental Springs, officially registered as CONTINENTAL SPRINGS LLC, sits at 3200 G St and works as a for-profit, individually owned nursing home that provides Skilled Nursing Facility and Nursing Facility (SNF/NF) care for people needing both medical help and daily living assistance, so people come here when they need more complex care, recover from being in the hospital, or need help after an illness or injury, and while there are both short-term and long-term care options, many residents stay a while, with an average daily count of about 50 people-though the place has 77 certified beds, and it offers all sorts of rooms including both private and semi-private ones, as well as Assisted Living Apartments and Studio Suites. The facility helps residents stay active and healthy across different areas-intellectual, social, physical, emotional, and spiritual-by offering activities, special dining options, laundry, personal care with things like dressing and getting around, and manages medications as well as any special diets, so families can also take comfort knowing staff are available all day and all night. They have a professional interdisciplinary team that pulls together nurses, social services, doctors, patients, and families to guide care, and they've set up support for daily living as well as programs to encourage independence within a safe, individualized setting. The facility also offers Rehabilitation Services, Respite Care for temporary stays, and even transportation for scheduled local trips, so people can get where they need to go. On top of all this, the social services department helps plan for home care after discharge and connects families with resources, and residents can count on meals, housekeeping, and laundry services.

The facility has adjusted nurse staffing hours per resident per day of 4.71 and adjusted RN staffing hours of about 0.73, but on weekends this drops to 0.32 RN hours per resident per day, and the official total nurse staffing hours reported is just over 4.39. Recent ratings reflect mixed performance: while the health inspection rating comes in at a solid 4 out of 5 stars, staffing gets 3 stars, and quality measures come in low with 1 star, which means there's some room for improvement, and the weighted health survey score is 24.67. The center has had 1 fine totaling $15,000 and 3 substantiated complaints, all of which suggest careful attention's needed before choosing it. Average user reviews give it a 2.4 out of 5 rating based on 8 reviews.

Medicaid, Medicare, and other qualifying managed care payments are accepted, and alongside all the usual care-like medication management, help bathing and dressing, personal laundry, meals, assistance with mobility, and vital sign checks-residents can take part in activities designed to promote a sense of community. Continental Springs works to create a comfortable, supportive environment that focuses on patient-centered care and keeps all kinds of people moving forward, whether they're recovering or settling in for the long-term.
