    Excellent rehab care; food inconsistent

I stayed here for short-term rehab and the nursing, OT/PT and support staff were outstanding - caring, professional, kept my family updated, and helped me get back home safely. The facility is clean, well-organized and social with pleasant common areas, though food quality was inconsistent and I experienced occasional slow responses from staff (including one urgent alert). Overall I'd recommend it for rehab and attentive care, but be aware of staffing/meal issues.

    About Elm Wood Center

Elm Wood Center, part of Haven Health Centers, sits in a calm suburban area, offering skilled nursing care and rehabilitation services, and you'll find it associated with the provider "Elm Wood Center at Claremont." The facility offers both short-stay rehabilitation and long-term care, so some folks come to recover after the hospital while others live there for care around the clock. Nurses and attending physicians, including an on-site Medical Director, oversee health needs day and night, helping with everything from medication and wound care to colostomy and IV therapy. The staff speak mostly English, though some can assist in other languages, and they've got coordinators who help set up individual care plans, manage discharges, and keep dietary needs and nutrition on track. Services cover palliative and hospice care, pain management, respite stays, and dementia support, and the place works under VA contracts and accepts Medicaid HCBS for home and community-based services.

Therapists offer speech, occupational, and physical therapy every day, whether you're staying long-term or coming just for rehab after a hospital stay. Occupational and physical therapy even happen at home through telemedicine if you need that, and there's a special effort to make sure pediatric therapy is safe and gentle. Elm Wood Center runs coordinated accountable care programs, brings in contract rehabilitation, and handles staffing and physician services through its network and subsidiaries. There's a focus on health monitoring like blood pressure and weight checks, and they provide home care and home health care if folks need recovery or assistance outside the building.

The building itself contains private and semi-private rooms, each with individual climate controls, private bathrooms in some cases, and either a balcony, porch, or patio to sit outside. Residents' comfort is addressed with daily housekeeping, complimentary laundry, personal care, and security measures like alarm systems to help folks with cognitive challenges. Meals are served in a spacious dining room restaurant-style, with an always-available menu for special requests, and there's a country kitchen and private dining spaces for family gatherings. Café service, common lounges, and sunrooms provide spots to relax, along with gardens and courtyards for enjoying the outdoors. Elm Wood Center keeps up with recreation and wellness programs, offering activities that span cultural, educational, religious, and social interests, as well as an activity room, a spa, beauty salon, and barber services.

Other support includes dental, vision, psychiatric, and podiatry care; wound and pain management; x-ray services; and even pharmacy delivery, mail, and newspaper services. There's wireless internet, phone service, and coordinated transportation for doctor's visits or outings. The community maintains a smoke-free environment with a 24-hour emergency alert and response system. According to 40 customer ratings, Elm Wood Center has a patient satisfaction rating of 3.7 out of 5, which means it gets decent but not perfect reviews. Elm Wood Center allows scheduled tours so you can get a firsthand look at its rooms, shared spaces, and the overall atmosphere before making a decision.
