    Warm professional care; highly recommended

I'm very pleased with my loved one's care at Crown Heights Center - the staff are warm, professional and responsive, and Ms. Sirlina and the social-work team went above and beyond to handle paperwork and keep us informed. The facility is clean, the rehab and nursing services were effective, meals and activities were enjoyable, and communication was prompt. So far a positive, reassuring experience - I'd recommend asking for Sirlina.

    Summary of reviews

The reviews of Crown Heights Center for Nursing and Rehabilitation present a mixed but instructive picture. Many families and residents praised the facility for compassionate direct care, a strong rehabilitation program, and a social work team that communicates effectively and supports discharge planning. At the same time, a significant set of operational concerns appears repeatedly: inconsistent staffing, communication breakdowns with families, and several clinical- and safety-related failures that warrant careful consideration by prospective residents and their advocates.

Care quality and clinical operations vary across accounts. Positive experiences highlight effective physical, occupational, and speech therapy that helped residents regain function and return home. However, other accounts identify delays in medication delivery, documentation mistakes, and delays in clinical testing and escalation that in some cases led to hospital transfer. Reviewers also raised concerns about inconsistent delivery of personal-care tasks (for example, bathing and toileting) and call-response times; these items point to workload and staffing-scheduling issues rather than a uniform clinical standard.

Staffing and conduct are central themes. Numerous reviews single out individual staff and social workers (by name) for attentive, respectful, and family-oriented care; these staff are frequently cited as the reason families felt secure. Conversely, there are recurring descriptions of staff impatience, abrupt communication, and unprofessional interactions, along with claims of inadequate supervision. These accounts indicate variability in staff training, morale, or oversight: when staffing and supervision are strong, families report very positive experiences; when they are weak, care and communication suffer.

Dining, activities, and the daily environment receive mostly positive comments. Several reviewers noted enjoyable meals, active recreation and music programs, and a generally welcoming, hotel-like ambiance in common areas. Housekeeping and concierge services are praised by many, although this positive view is not uniform across all units and rooms.

Facility condition and infection-control are areas of divergence. Some reviews describe clean, well-kept rooms and communal spaces, while others raise sanitation concerns and pest-control issues in specific rooms or wings. There are also reports indicating uneven infection-control or cohorting practices. These mixed signals suggest variability by unit or by time; prospective families should verify the current status during an in-person visit.

Management, accountability, and logistics show a split pattern. Several families commend specific managers and social workers for clear communication, timely callbacks, and advocacy. At the same time, there are frequent complaints about unanswered phones, voicemail systems that are not maintained, poor follow-through on family concerns, and coordination failures for off-site services such as dialysis or transportation. Additionally, some reviews raise serious claims regarding belongings management and financial handling; the summary term "allegations" is used here because these items, when present, involve potential misconduct that would require investigation.

Notable patterns and advice: the facility demonstrably provides strong rehabilitative care and has staff members who deliver compassionate, family-centered service. However, variability in staffing, supervision, medication processes, room-level sanitation, and communication systems creates an uneven experience. Prospective residents and families should (1) observe staffing levels and cleanliness in the actual unit where the resident will be placed, (2) ask about medication-safety protocols and incident escalation procedures, (3) confirm policies for belongings security and visitor access, (4) meet the social work point person and clarify communication expectations, and (5) inquire about transportation and off-site care coordination. These steps can help identify whether Crown Heights’ strengths are present and whether the operational gaps noted in the reviews have been addressed.

    About Crown Heights Center

Crown Heights Center for Nursing and Rehabilitation sits in Brooklyn, New York, and offers both short-term rehab and long-term care for adults needing different levels of help, and the building has 295 beds available, though only 10 beds are certified for Medicare and Medicaid as of June 2025, which is important to know if insurance is a concern, and you'll find furnished rooms there with things like private bathrooms, kitchenettes, cable TV, telephones, air conditioning, and Wi-Fi, all set up so residents can stay comfortable, watch their own shows, and keep in touch with family or the outside world, and for folks who need extra help, there's a dedicated nursing staff who handle transfers, medication management, bathing, dressing, and other everyday needs, and even though you get 12 to 16 hours a day of skilled nursing staff, there's always someone around for supervision because a 24-hour call system means someone can answer if a resident needs something at any hour, and so folks with Medicare and Medicaid can come there, and they run a wide range of healthcare services, which includes skilled nursing, orthopedic rehab, and even onsite dialysis, and in terms of specialties, people can use services like the Hearts in Motion program, Salud Latina, and special care for geriatric psychiatry and cardiac rehab, and for those needing methadone treatment, the facility handles that, too, and you'll see community-sponsored activities, arts and crafts rooms, a game room, library, walking paths, outdoor space, a garden, a wellness room, and a spa, so there's enough going on for folks who want to keep busy or just get outside for some air, and the emergency alert system means staff gets notified quickly if something goes wrong, and you'll also find that they've tried to blend a cozy, boutique-style feel with new technology, and they've got world-class doctors, nurses, and therapists, and even some unique programs like the 365 Advantage initiative, and throughout all this, the goal stays patient-centered, so the main focus really seems to be finding support and comfort for seniors and disabled adults who need both nursing care and a community atmosphere, all set at 810 St Marks Ave in Brooklyn.
