    Attentive rehab improved uncle's health

I'm very pleased with this clean, well-organized facility and its caring, friendly staff. Ms. Johnson's communication and the helpful supervisor made coordination easy. My uncle gained weight, looks groomed and happier thanks to attentive rehab and collaborative care. We'll continue using their services and would recommend them.

    Summary of reviews

The reviews describe a facility with a clear contrast between the quality of hands-on care and the condition of the physical plant and operations. Clinical care and rehabilitation receive multiple positive remarks: nursing staff and CNAs are frequently characterized as caring, attentive, and effective in tasks such as pressure-area prevention and rehabilitative recovery. Several accounts describe measurable clinical improvement in residents (for example, weight gain and improved hygiene) and successful short-term rehabilitation stays. Specific staff members and supervisors are singled out for strong communication and advocacy on families' behalf.

At the same time, recurring operational concerns are prominent. The building is described as aging and in need of repairs โ€” reviewers cite leaks, interior damage, and general deterioration โ€” and housekeeping and sanitation practices are uneven, producing odor concerns and other cleanliness issues in some areas. Several comments point to delays in incontinence care and to housekeeping being overstretched. While some housekeeping employees are described as friendly and conscientious, maintenance and sanitation appear inconsistent across the facility.

Staffing and management themes are mixed. Many reviewers praise individual caregivers and supervisory staff for dedication, empathy, and helpful communication. Conversely, others report understaffing, high CNA workloads, and instances where staff did not respond promptly or where communication systems (voicemail, phone responsiveness, intra-staff handoffs) failed families. These operational stresses are associated with variable staff conduct, occasional complaints about tone or responsiveness, and indications of turnover or internal personnel tensions.

Dining and resident life are similarly variable. Food quality is described as inconsistent and at times unsatisfactory, though organized rehabilitation and recreational opportunities are present and some visitors observed engaged residents in communal areas. The facility does have an outdoor courtyard or grounds that families find appealing, but the available outdoor space may be limited by the site's condition and surrounding land use.

Management issues raised include mishandling of personal belongings, miscommunication around donations, and discrepancies between promotional images and the facility's current condition. There are also references to restrictive visitation policies during infection-control periods; reviewers differed in how those policies were implemented and communicated. A small number of accounts raise serious concerns following adverse resident outcomesโ€”these are individual, severe recollections that families will likely want to investigate further through inspection reports and regulatory records.

Overall pattern: prospective families will find strong, committed caregiving in many parts of the operation, particularly among specific nurses, CNAs, and rehabilitation staff, but should be aware of ongoing facility-maintenance and operational gaps that can impact cleanliness, comfort, and consistency of service. Recommended due diligence before placement includes an in-person tour of resident rooms and common areas, direct questions about staffing ratios and shift coverage, review of recent inspection reports, inquiry into property-handling policies, and clarification of current infection-control and visitation practices.

    About Far Rockaway Center

Far Rockaway Center sits over on Virginia Street in Far Rockaway, New York, and it's a skilled nursing facility that's got a hundred certified beds. The center is open all day and all night, with medical-surgical units, an intensive care unit, emergency services, rehab, maternal and child health, and behavioral health services, covering plenty of needs as folks come and go. There's a big staff - they seem to know everybody by name - and they aim to help people feel at home, even when home's out of reach for a while. The staff includes licensed nurses, practical and vocational nurses, and clinical specialists, all working together to give nursing care, post-acute care, and therapy, with on-site services like RehabStrongโ„ข using up-to-date technology for physical and occupational rehab. Far Rockaway Center belongs to Centers Health Care, one of the large names in post-acute care around the Northeast, and the building's had some recent improvements to keep things up to date, making room for both short-term stays and long-term folks needing more help day-to-day.

Most folks there get individualized plans, since there's therapy that's tailored to each person, and they offer special programs for stroke, cardiac issues, pain management, ventilator care, TBI/neurobehavioral needs, dementia, wound care, hospice, HIV/AIDS, dialysis, and more, including even a program for COVID-19 care. The staff focuses on helping people get strong enough to go home or back to active life when possible. Families and friends are able to visit, with some rules and COVID-19 precautions, and the place offers rapid and PCR testing for safety. There are outside spaces where residents can relax, enjoy some fresh air, and gather for a bit of sun, which helps with well-being and brings a bit of calm to the day. The facility's nutrition and wellness programs look after what folks eat and how they feel, and there's a free lunch for residents. Social activities happen to support family, friendship, and a closer-knit environment, and a Resident Council helps keep residents' voices heard about what goes on.

The center keeps up with modern technology, too - there's an advanced electronic charting system to help the team communicate clearly about patient care, tracking things like re-hospitalization, pressure sores, medication use, and infection rates, all aiming to improve quality. They report those numbers as required and hold certifications for both short-stay and long-stay quality care. Far Rockaway Center's certified for Medicare and Medicaid participation, with a lower-than-average staff star rating of 2, a health inspection star rating of 2, and an overall CMS rating of 2 out of 5, so that's something to keep in mind, but the resident care itself earns an average 3-star mark. The place has not received federal fines or penalties in the last three years, which can help bring a little peace of mind to families making choices. With comfortable rooms, many services, and a broad reach of care options, Far Rockaway Center works alongside families to help people heal, regain strength, and stay connected to their community as best as possible.
