    Compassionate staff, clean facility, excellent

I'm very pleased with the genuinely caring, responsive staff-Jess and Kelsey especially-who treated my mother like family and kept the facility clean and welcoming. The preserved Victorian charm, bright private rooms, strong therapy/rehab team, fun activities (bingo, salon, church) and excellent communication made our experience comfortable and reassuring.

    Summary of reviews

The reviews reflect a facility with clear strengths alongside notable operational inconsistencies. Families frequently praise the clinical and therapy teams for compassionate, attentive care and effective rehabilitation when available. Reviewers described knowledgeable physical, occupational, and speech therapy staff and several positive discharge-coordination experiences. Social-work support and a family-like culture are recurring positives; many accounts highlight individual caregivers and unit teams who go above and beyond for residents.

At the same time, a substantial number of reviews identify operational weaknesses that may affect daily living. Cleanliness and linen-management practices appear inconsistent across the facility: concerns include soiled linens, missing pillowcases, and general sanitation issues in some areas. Relatedly, reviewers described gaps in bathing schedules and pressure-injury prevention, suggesting inconsistent implementation of basic personal-care routines. Chronic staffing shortages were repeatedly linked to delayed responses, residents not being brought out of bed as intended, limited outdoor time, and delays in medication administration.

Dining and amenities show mixed feedback. Some families reported positive engagement through on-site activities (church services, bingo, yoga, crafts), a salon, and events such as a farmer’s market and craft fairs. Others noted the food as unappetizing or limited in variety. The building’s historic, Victorian-style sections and bright rooms were described positively by many, but other comments point to aging infrastructure and maintenance issues (low water flow in showers, nonfunctional TVs, and other equipment problems).

Communication and management oversight are areas of divergence in the reviews. Numerous accounts compliment front-line staff and specific administrators or nurses for transparency and responsiveness; however, other reports raise concerns about staff conduct, poor tone in family communication, and inconsistent handling of complaints. A very serious claim regarding theft and financial misconduct appears in the record; this is an allegation that warrants independent verification through facility management and local regulators. Staffing, cleanliness, medication coordination, and property-management issues together suggest variability in performance across shifts or units rather than uniform facility-wide practice.

For prospective residents and families: consider touring at different times of day and on different shifts, ask for current staffing ratios and recent inspection or complaint histories, review laundering and linen-change protocols, inquire about pressure-injury prevention and bathing schedules, confirm medication-administration procedures and error-tracking, and request documentation of financial controls for residents’ accounts. Overall, Foltsbrook Center shows strong individual caregiving and an active activities program, but families should verify that cleanliness, staffing levels, medication coordination, and facility maintenance meet their expectations before making a placement decision.
