    Clean, caring, professional rehab facility

I was impressed by the clean, well-kept facility and spacious, sunny rooms. The staff were consistently kind, professional and attentive - nurses, CNAs, social workers and recreational staff - and PT/OT was excellent and progress-driven. Food and activities were good, families felt welcome, and overall I recommend this caring, well-run place for rehab and short stays.

    Summary of reviews

Island Nursing & Rehab Center elicits a wide range of experiences. Many reviewers praise the facility for strong rehabilitative services, effective therapists, and staff who support measurable progress and discharge goals. Positive comments frequently describe knowledgeable PT/OT teams, attentive CNAs and nurses on certain units, helpful social-work support, bright and spacious rooms, family-friendly visiting spaces, and an overall welcoming environment following recent facility improvements.

Care and staffing present a mixed picture. Several accounts describe attentive, compassionate care that enabled functional recovery, while an equal number describe inconsistent staffing levels that produced delays in assistance, missed showers or extended periods in bed, and slow responses to call bells. These staffing inconsistencies appear to affect personal care, timely medication administration, and the facility’s ability to respond reliably to alarms and urgent needs. A subset of reviews raised concerns about wound care, IV management, delayed antibiotics, and other clinical-practice gaps that, in some cases, led to hospital transfers and family distress.

Dining and activities show variability. Many residents enjoyed nutritious meals and occasionally highlighted especially good dishes, and the facility offers on-site programming such as bingo, concerts, and game nights. At the same time, reviewers noted inconsistent meal quality and temperature control, limited off-site outings, and variable resident engagement in activities — suggesting the recreation program may be active but unevenly delivered.

Facility condition and operations are similarly mixed. Numerous comments praise a clean, bright environment, roomy accommodations, accessible parking, and tidy common spaces. Contrasting reports cite sanitation concerns in particular rooms or units, laundry and personal-item management problems, and occasional lapses in housekeeping standards. Operational concerns also include phone and communication unresponsiveness and difficulties reaching leadership or clinical staff by phone.

Management and communication emerged as recurring themes. Positive interactions were described at admissions and with some front-desk and nursing managers; however, several families cited inconsistent leadership responsiveness, a brusque communication tone from particular staff, and gaps in interdepartmental coordination. There are also serious allegations in some accounts concerning medication security and missing items; these raise questions about inventory controls and safe medication handling.

Overall, Island Nursing & Rehab Center demonstrates notable strengths in rehabilitation, therapeutic skill, and certain aspects of hospitality and environment. At the same time, recurring operational issues — especially inconsistent staffing, variability in personal-care responsiveness, clinical communication gaps, and uneven housekeeping and dining standards — warrant attention. Prospective residents and families should weigh the facility’s therapy capabilities and environment against potential variability in day-to-day nursing coverage and operational reliability. When considering admission, recommended questions include current staffing ratios and turnover, protocols for medication administration and wound care, alarm-response processes, laundry and personal-item procedures, and the scope and frequency of recreational outings.
