    Clean bright facility, phenomenal staff

I felt very comfortable placing my mother here - the facility is clean, bright, updated and the large private rooms have lovely country views. The staff were phenomenal: attentive nurses, outstanding PT/OT, wonderful salon and rehab teams, and a responsive head nurse (Cindy) who kept us informed. There are plenty of activities (patio concerts, rec room, daily mass, movie nights), friendly residents, and a welcoming, respectful atmosphere that helped my mom recover and go home at her best. I highly recommend this caring, well-run place - worth the cost.

    Summary of reviews

The reviews present a mixed but distinct pattern: Lutheran Care Center at Concord Village is frequently praised for its rehabilitation services, therapy staff, and many aspects of daily life, while simultaneously drawing concern for several operational inconsistencies. Positive comments emphasize a strong PT/OT program, caring and engaged nursing and therapy teams, abundant social programming (movie nights, bingo, concerts), on-site salon services, bright private rooms with exterior views, and clean, welcoming common areas. Families commonly describe a pleasant entry experience, accessible parking, and the availability of religious services and leisure amenities that support resident quality of life.

Despite these strengths, recurring operational issues are evident. Staffing levels and coverage are described as inconsistent, and reviewers link that inconsistency to delays in care response, limited therapy availability at times, and uneven attention to routine needs. Dining and dietary management emerge as a frequent concern: reviewers describe meal preparation that can be prepackaged, high in sodium/carbohydrate profile, and at times not aligned with individualized dietary restrictions. Relatedly, there are reports pointing to call-bell accessibility problems and other responsiveness gaps that suggest variability in day-to-day care processes.

Clinical-safety and facility-maintenance themes also appear in the feedback. Several reviewers raised issues consistent with gaps in clinical escalation, allergy/medication oversight, and fall-risk supervision; a small number described serious clinical outcomes that underscore the importance of clear escalation protocols. Maintenance and safety compliance concerns (for example, localized maintenance problems or safety items needing attention) are described alongside otherwise clean and updated areas, indicating inconsistency across units or shifts. Reviewers also note variability in staff professionalism and accountability, including night-shift coverage and noise-management, and some families describe a decline in performance following administrative turnover.

For prospective residents and families: the facility offers clear strengths in rehabilitation, therapy, social engagement, and an overall welcoming environment. At the same time, it would be prudent to verify current staffing ratios, obtain details on dietary-accommodation procedures, review fall-prevention and clinical-escalation protocols (including allergy and medication checks), and ask about recent maintenance and safety inspections. Observing a mealtime and speaking with therapy leadership and the unit nurse manager can help confirm whether the operational concerns raised in reviews have been addressed. Given the facilityโ€™s fee structure, clarify cost expectations and what services are included before making placement decisions.

About Lutheran Care Center at Concord Village

Lutheran Care Center At Concord Village sits at 965 Dutchess Tpke, Poughkeepsie, New York, and offers a range of care services for seniors age 55 and up. The staff includes physicians, nurse practitioners, registered nurses, licensed practical nurses, certified nurse assistants, social workers, registered dietitians, and specialists for physical, occupational, and speech therapy, so residents can get different kinds of health and personal care. The center focuses on offering seniors compassionate support with respect, dignity, and as much comfort as possible, all in an environment that feels more like a modern retirement residence than an old-style nursing home. People who come here find 120 long-term care beds and 40 short-term beds, with rooms that look out on gardens and sunlit spaces, and an easygoing mid-sized place that lets caregivers focus on each person's needs. The facility stays connected to a continuing-care community, so there are independent living apartments, assisted living, nursing care, and even adult day care options. Seniors who want more independence can live in their own apartments, and those who need extra support can join assisted living communities with services that help with daily tasks if needed.

You'll find a chapel with stained glass windows, South Gibbs Garden, a recreation center with a library, and an atrium full of plants that brings a warm feel to the place. Meals, usually one or two prepared each day, focus on good nutrition, with staff making sure residents get enough vitamins and minerals in what they eat. Regular housekeeping and laundry keep things tidy, and there's weekly onsite barber and beauty care, transportation services, and special event rooms that families can use for gatherings. Seniors who need special help for memory problems, like Alzheimer's or dementia, can get memory care designed to reduce confusion and keep them safe, with support from skilled caregivers who use therapies matched to their needs. There's a dedicated rehab center and home transition gym to help people recover after illness or surgery, and a recreational therapy center for physical and mental activity throughout the week. People who want spiritual support can find it through the Lutheran tradition of care that guides the community's approach, focusing on body, mind, and spirit, with options for spiritual services when requested.

The environment feels calm, peaceful, and friendly, helped by a staff known for being helpful and kind to residents and visitors. There's a strong mix of social, physical, and mental activities, with programs and outings planned to keep people active and engaged, and those who want quiet can sit and visit in one of the lounges or the special seating areas. Lutheran Care Center At Concord Village's care options reach from home care with trained aides for those wanting help while staying in their own homes, to nursing home services, long-term care, short-term rehab, respite care, adult foster family care, and hospice services when needed. The facility has earned general recognition in the community, showing a quality measures rating of 4 stars, with staffing and health inspections both rated at 3 stars, and an overall CMS Five Star rating of 3 stars. People can schedule tours to meet staff, see the amenities, and get a sense of the day-to-day atmosphere, Monday through Saturday during regular business hours. The center responds best to English-speaking residents, and everyone is welcomed into a community-focused setting where support and respect guide every step of care.
