    Compassionate staff and excellent care

I had a very positive experience - compassionate, family-like staff (CNAs, nurses and therapy team) went above and beyond and kept my loved one smiling. Clean, homey facility with private rooms, excellent rehab and engaging daily activities; grounds and management were lovely and responsive. I'd recommend this place for short- or long-term care.

    Summary of reviews

Reviews of Hickory Falls Health and Rehab describe a facility with a mixture of clear strengths and notable operational inconsistencies. Many families and residents praise the caregiving teams — particularly CNAs, nurses, and the therapy staff — for compassionate, attentive bedside care and effective rehabilitation outcomes. The on-site therapy services (physical, occupational and speech) are frequently cited as a strong point; several reviewers described measurable recovery progress and preparation for discharge. The physical plant, including private rooms with private bathrooms and well-kept grounds, is generally viewed positively and contributes to a home-like environment for residents.

At the same time, a recurring theme is inconsistency in how the facility operates day-to-day. Communication with families about clinical status and care plans is frequently described as unreliable, and reviewers note variability between shifts and units. Staffing levels and staff continuity appear uneven; understaffing or shift-to-shift turnover is linked in several accounts to delays in assistance, missed therapy sessions, and inconsistent implementation of care plans. Some families described serious clinical concerns involving wound care, medication administration, and delayed transfers to higher levels of care, which point to gaps in medication-management practices and clinical coordination that warrant careful review by prospective families.

Dining and ancillary services are another area of mixed feedback. The facility offers multiple meal choices and some reviewers praised meal variety, but others described inconsistent food quality and limited diabetic-appropriate options. Laundry and personal-belongings handling emerged as a consistent operational issue in several accounts, with clothes mix-ups and lost items cited. Activity programming and social engagement receive both positive and negative comments: some residents experienced robust, family-like community events and celebrations, while others reported promised activities that did not occur.

Management and administration receive polarized evaluations. Some reviewers highlighted effective problem resolution and approachable administrators and social workers; others described poor responsiveness to billing questions, room-assignment changes without notice, and lapses in infection-control practices (including instances when staff tested positive for infectious illness). Safety monitoring and incident-response practices are a notable concern for families who experienced delays or inconsistent responses during clinical emergencies. In summary, Hickory Falls offers meaningful clinical and rehabilitative strengths and a well-maintained environment, but prospective residents and families should closely evaluate unit-level staffing patterns, communication processes, medication-management protocols, and assurances around therapy delivery and personal-belongings handling when making placement decisions.

About Hickory Falls Health and Rehab

Hickory Falls Health and Rehab sits over in Granite Falls, North Carolina, right on Sunset Street, and offers both short-term rehabilitation and long-term skilled nursing care with 120 certified beds, where you'll find private and semi-private rooms, and there are three therapy gyms where physical, occupational, and speech therapy take place every day to help people recovery from illness or injury or just to keep up daily skills, and you'll see folks working together with an interdisciplinary team that includes nurses, therapy specialists, and nutrition experts to make personalized care plans that change as needed, which is especially important for wound care, memory care, hospice, and when someone needs help with daily living. The facility has an ice cream parlor and even a little chicken coop outside, so you might visit with the birds after a meal or during an afternoon activity session, and there are always planned daily events like bingo, crafts, and outings when the schedule allows, since they try to keep minds and bodies busy, which helps people feel connected. Visitors can come at any hour, since the facility allows for 24-hour visitation, and family can bring food in too, though the nursing staff does want a quick conversation first just to make sure all dietary needs and safety rules get followed, and residents can also request special menus or rely on an "Always Available Menu" if nothing on the main meal rotation at breakfast, lunch, or dinner works for them, with all meals served in the main dining room and nutrition staff on hand to handle dietary restrictions. The rooms come with a phone, TV, and Wi-Fi included, and laundry's taken care of at no cost, although families can do wash themselves if preferred, and an on-site beautician provides haircuts and grooming at an extra charge. The place has strict rules: no tobacco use anywhere inside or out, and no alcohol unless prescribed by a doctor, and pets can visit with paperwork and a leash if approved beforehand. Residents are encouraged to pack light-with just a few comfortable outfits, pajamas, supportive shoes, and their own favorite toiletries if they'd rather not use the supplied brands. The staff focuses on caring for each person's health, well-being, and comfort, though the government has found problems here in recent inspections, with fines issued and some safety issues marked as serious but affecting only a few people. Hickory Falls Health and Rehab is part of the SanStone community and belongs to the North Carolina Health Care Facilities Association, under the direction of administrator Brandee Fulbright, and while it's had about 29 public reviews and runs with an average of 114 residents per day, it supports families with resources and guidance for long-term care, offering familiar comforts with social activities, nutritious dining, therapy options, and skilled nursing services around the clock.
