    Caring knowledgeable staff, effective rehab

I had a very good stay at Three Rivers - caring, knowledgeable nurses and therapists got my mom from bedridden to walking several steps, and the staff (nursing, CNAs, therapy, janitorial and admin) were friendly, responsive and attentive around the clock. The facility was spotless and odor-free with hotel-like amenities, great meals, clear communication, and a truly rehab-focused, family-centered approach I would recommend.

    Summary of reviews

Three Rivers Health & Rehabilitation Center is frequently described as a rehabilitation-oriented facility with a palpable emphasis on getting residents back to independent mobility. Reviewers consistently praise the therapy team for clinical knowledge, measurable progress, and proactive discharge planning, and many families describe timely follow-up and coordination with home health or outpatient services. Nursing and care-partner staff are commonly characterized as kind, attentive, and compassionate; several accounts highlight a family-like atmosphere and 24-hour availability of care.

Facility upkeep and amenities are regular strengths in the reviews. The building and rooms are often described as very clean and odor-free, with janitorial staff singled out for good work. Dining receives largely positive comments for meal quality and accommodating cooks, and some reviewers characterize the environment as having hotel-like amenities including single-room options. Activities and social programming are noted as engaging, and administrative staff are frequently credited with assistance around insurance navigation and appointment support.

Despite these positives, a pattern of operational concerns appears in the feedback. The most significant themes are inconsistent staff responsiveness (including difficulty reaching the facility by phone) and gaps in assistance with activities of daily living such as meal setup and toileting. These operational shortfalls are linked in some accounts to inadequate fall-risk management and to weight loss or reduced intake when assistance is not reliably provided. Families also describe variability in admission communication and administrative follow-through — for example, unexplained admission declines or delayed return calls — which suggests uneven case-management processes.

Performance variability across shifts is another recurring observation: while some visitors report prompt, communicative staff and strong outcomes, others describe delays or limited availability at particular times. Noise levels in rooms or common areas were occasionally noted as disruptive. Taken together, the reviews point to a facility with clear strengths in rehabilitation, cleanliness, and interpersonal warmth, but with operational inconsistencies that merit attention.

For prospective residents and families: verify specific operational details during your visit. Ask about fall-prevention protocols, scheduled assistance for activities of daily living (including meal assistance and toileting plans), staffing levels by shift, and how the facility manages phone and family communication. Also request to observe a therapy session and to tour resident rooms at different times of day to assess noise and staffing responsiveness. Those focused on recovery-oriented care and a clean, rehabilitation-centric environment may find Three Rivers aligns well with their goals, provided the facility’s operational practices meet the family’s expectations for consistent assistance and communication.
