    Caring staff, excellent personalized rehab

I'm very pleased with my experience - the staff are attentive, caring and genuinely friendly, therapists provide excellent, personalized rehab, and administration was extremely helpful with transfers, insurance and meds. The community feels clean and safe, offers welcoming activities and family events, and is convenient for visitors, so I'd recommend it.

    Summary of reviews

The reviews present a mixed picture of Hall of Fame Rehabilitation & Nursing Center: many families praise the direct care team and therapy services, while others describe operational weaknesses that affect daily life and confidence in care. Strengths most consistently cited include attentive nursing staff, robust rehabilitation programming, and a social environment that supports family involvement. Several reviewers highlighted 24-hour nursing availability, individualized therapy plans that aided recovery, and administrative assistance with insurance and care transitions.

Care and staff: Clinical strengths center on nursing teams and therapists who are described as caring, skilled, and helpful; therapy is commonly credited with meaningful functional gains. However, staffing instability is a recurrent operational issue. Reviewers describe delays in responding to resident requests, variability in aide availability, and uneven attention to bedside needs. There are also concerns about the tone of some staff interactions and inconsistent clinical oversight in parts of the building. Notably, experiences appear to vary by unit or floor; some units receive consistently positive comments while others are characterized by lower responsiveness.

Dining and rehabilitation: Therapy services and rehabilitation regimens receive strong positive remarks for personalization and effectiveness. Dining feedback is mixed: some families appreciate the food and dietary staff, while others report inconsistent food temperature, preparation problems, and variable meal quality. Meal-service continuity and timing appear to be unreliable at times, contributing to family frustration.

Activities and social environment: The facility offers a range of activities, including family-inclusive events, and reviewers often note a welcoming atmosphere in communal areas. Many residents formed friendships with staff and peers, and activities are viewed as a meaningful part of daily life for some occupants.

Facilities and maintenance: The building is described as dated in places, with recurring infrastructure concerns such as lighting problems, intermittent water outages, cosmetic repairs needed, and uneven housekeeping. Sanitation and linen-change practices are described as inconsistent, producing sanitation concerns in some rooms or common areas. Security and access-control weaknesses are also cited, including allegations of unauthorized room access and, in a few accounts, theft; these are operational risk areas that prospective families should evaluate directly.

Management and systems: Several reviews note active efforts by new management and some administrators to improve systems and communication, and specific admissions staff were praised for facilitating transitions. At the same time, weaknesses in front-desk coverage, weekend phone access, and general communication responsiveness are frequent themes. Transportation coordination, social-work support, and after-hours managerial presence are additional areas where families reported inconsistent performance. Reviewers also raised concerns about workplace cultureโ€”perceptions of favoritism and morale problemsโ€”which can contribute to turnover and inconsistency in service.

Overall patterns and guidance: The facility offers real clinical and rehabilitative strengths, particularly where nursing and therapy teams are stable and engaged. Conversely, operational problemsโ€”staffing variability, maintenance and housekeeping inconsistencies, communication and reception gaps, and security concernsโ€”are recurring and can materially affect resident experience. Prospective residents and families should arrange a focused visit to the specific unit being considered, ask about staffing ratios and recent quality-improvement actions, inspect room cleanliness and maintenance, observe meal service, and request details on after-hours phone coverage and security policies before making a placement decision.

About Hall of Fame Rehabilitation & Nursing Center

Hall of Fame Rehabilitation & Nursing Center sits at 2714 13th St NW in the Westbrook neighborhood of Canton, Ohio, and it's a Skilled Nursing Facility that's pretty well known for both its nursing care and rehabilitation services, so you'll see a mix of long-term and short-term residents there, and with 69 certified beds, it tends to have about 36 people staying on any given day, and right now they're not taking new patients, but they do answer questions about future openings. The staff often speaks English, but some know other languages, which comes in handy, and you'll find round-the-clock nursing care, as well as licensed therapists who handle speech, physical, and occupational therapy, and resources for things like tracheostomy care, stroke recovery, cardiac or orthopedic rehab, hospice, and specialized post-acute care, all in a fairly spacious place with private rehab suites, rooms, and a secure courtyard, plus some simple comforts like complimentary cable TV, WiFi, housekeeping, laundry, an enclosed patio, and a beauty salon, which residents seem to appreciate. The facility's got a relationship with Divine Healthcare Management, and they take Medicare, Medicaid, and other insurances, working with folks who need help after surgery or illness, all while offering 24/7 direct admissions from local hospitals for those needing care quickly.

Staff is pretty attentive and tries to treat people like family, greeting folks by name and coming up with treatment plans unique to each person, regardless of how long they stay, and they go through regular training to stay up to date with new skills. Nurses are on hand about 3.28 hours per resident per day, but it's worth knowing the nurse turnover rate is on the higher end, sitting just above 52 percent. Hall of Fame has reported deficiencies on past inspections-32, in fact-including citations for infection prevention and some related to safe respiratory care, though none have caused actual harm, but they did have potential for more than minimal harm, so it's something for families to think about. The team works together across nursing, therapy, social services, activities, and dietary groups to make sure the care matches what each patient needs while aiming for a comfortable and friendly environment, with organized activities, outings, and special events to encourage social connections, all provided with some warmth and compassion you'd hope for in a care setting. Long story short, the place offers skilled nursing and modern rehab services in a setting that balances medical support with enough day-to-day comforts and personal touches to help people recover or manage long-term needs, though like many facilities, it's had a few bumps along the way with compliance and staffing you may want to ask about if you're considering care here when openings become available.
