    Outstanding compassionate care, clean rooms

I'm very pleased with the care my mother received at Concord Village. The nurses, aides, therapists and social worker were compassionate, professional and responsive; therapy was outstanding, rooms were clean, the food was good, communication was clear, and I would recommend them.

    Summary of reviews

Concord Village presents a polarized profile in these reviews: the facilityโ€™s therapy departments and many direct-care aides receive consistent praise, while clinical and operational areas show repeated weaknesses. Physical and occupational therapy are frequently described as strong contributors to functional improvement, and multiple families highlighted compassionate, attentive nursing aides and supportive rehabilitation staff. The facilityโ€™s new, modern design, private rooms, pleasant grounds, and a welcoming front desk and social services presence are also noted as strengths, and pet visits and proximity to a hospital are convenient features for some families.

However, a substantial theme across narratives is variable nursing and clinical follow-through. Several families described inconsistent nursing care quality, including long delays responding to call lights, delayed or missed medications, and uneven wound- and infection-management practices. These concerns are associated with clinical consequences in some accounts, and reviewers specifically called out problems with timely antibiotic administration, wound-vac care, and monitoring of lines and oxygen delivery. Safety-management issues such as falls and transfer-related problems were also described as patterns to watch.

Operationally, staffing and communication are recurring weaknesses. Many families reported chronic understaffing, staff distraction (for example staff on phones), and long wait times for assistance. Communication gaps surfaced at multiple levels: inconsistent documentation, family-notification failures, errors or omissions in electronic records (including changes in primary physician not reflected in charts), and variable social-work coordination. Discharge planning and aftercare processes were frequently described as unreliable, with some families encountering billing or appeal difficulties and limited assistance with transitions to home or hospice.

Facility maintenance and dining received mixed feedback. Several reviewers praised a clean, modern facility and appealing food, while others raised sanitation and pest-control concerns in specific rooms or corridors and described inconsistent housekeeping and laundry service. Activity participation and daily-living assistance also varied: some residents engaged in activities and rehabilitation, while others were said to receive minimal therapy or to be left without routine bathing and hygiene support.

Management and leadership impressions were inconsistent: some families found social-work staff and managers responsive and helpful, whereas others experienced difficulty reaching administrators, exchanges perceived as unapproachable, or concerns about staff accountability. Staffing turnover and variability in individual caregiver competency were commonly mentioned as contributing factors to inconsistent experiences.

Overall, Concord Village appears to deliver strong rehabilitation services and benefits from a modern, well-appointed environment, but prospective residents and families should be alert to patterns of inconsistent nursing care, operational shortfalls related to staffing and communication, and variable housekeeping and clinical follow-through. When evaluating placement, consider asking targeted questions about staffing ratios, call-button response benchmarks, medication administration procedures, wound- and infection-management protocols, discharge and hospice coordination, and recent pest-control and housekeeping audits. A pre-admission visit to observe nursing shifts, therapy sessions, and housekeeping routines can help gauge whether the facilityโ€™s strengths align with a prospective residentโ€™s needs.

About Concord Village Skilled Nursing & Rehabilitation

Concord Village Skilled Nursing & Rehabilitation sits in a peaceful spot with 72 private rooms, each coming with a TV, Wi-Fi, and an ensuite bath, and you'll find the facility helps people who need recovery after illness, injury, or surgery, along with long-term skilled nursing and hospice care for those with chronic conditions or facing more serious health needs, and, this place takes Medicaid/Medi-Cal, Medicare, and private insurance, so many folks can get the help they need without as much worry. Meals come every day, served in a restaurant-style dining room, but there's also a cafe snack area and a dietitian team designs special diets for those who need them, and people can enjoy things like art therapy, educational classes, music and dance programs, movies, exercise, and various social activities, so there's always something to do besides sitting in your room. Licensed skilled nurses watch over residents around the clock, making sure everyone gets help with medication, wound care, diabetes and cardiac care, IV therapy, injections, respiratory support, pain management, help with bathing, dressing, and all those daily needs, plus Concord Village covers recovery from surgery or stroke, post-hospital care, and sub acute care specialties.

Doctors come on site for medical needs, and the place offers rehabilitation therapy, home infusion, podiatry services, lab tests, mobile radiology, cardiology, wound care, and even mental health professionals, audiology, and dentistry visits when needed, all adding up to a wide list of services delivered without having to leave the facility, and there's support for special medical diets and enteral nutrition too. Community spaces include a hair salon, library, chapel, outdoor courtyards, a lounge, and both comfortable and safe places for healing and connection, where friends and family can visit or just enjoy the fresh air. Nurse staffing is somewhat higher than average in the state, though nurse turnover runs high as well-64.6% compared to the state's 51%-and average daily residents number about 58 out of 72 certified beds, so it never feels overly crowded, but you may see new staff here and there.

As for inspections, Concord Village has had 16 total deficiencies, including two infection-related ones and a specific citation (F0695) about safe respiratory care, though no actual harm occurred but there was a chance for more than minimal harm, so it pays to keep an eye on their track record along with their high BBB rating at A+. The place is managed by Vrc Management, Inc., with five owners each holding a 20% stake, and it's operated as an LLC with Michael J. Francus as business manager, so there's a clear record of who's responsible. Staff focuses on making treatment plans tailored to each person, working to restore health and support residents' independence, with both short-term and long-term options, plus the option to reserve care ahead of time for scheduled surgery and recovery. While the facility's rooms are all private and designed to feel secure and welcoming, there's always the sense of trying to balance peace, dignity, and personal comfort for every resident, so folks looking for skilled nursing or rehab in a place with a wide list of services, on-site medical care, and private rooms may find Concord Village worth considering, and it remains a steady fixture in the area for those who need both medical help and a little extra attention each day.
