    Friendly staff good food comfortable

I'm generally pleased with Jelly Coop - the staff are welcoming and attentive, the food is good, and it's a comfortable, friendly place with a couple small areas that could be improved.

    Summary of reviews

The reviews indicate a polarized view of Arcadia Valley Skilled Nursing & Rehabilitation. A small number of comments praise aspects such as a welcoming staff, satisfactory meal quality, a positive overall rating in at least one review, and the availability of at least one social activity identified as "Jelly Coop." However, the predominant themes are operational and clinical concerns that prospective residents and families should evaluate closely prior to placement.

Care quality and clinical responsiveness are recurring areas of concern. Several reviewers described delays in attending to residents' needs and raised issues about hydration and timely medical attention; there are also references to a serious event that prompted concerns following a resident's death. Medication-administration processes were specifically questioned in terms of safety controls. These items cluster into a broader pattern suggesting inconsistent responsiveness and potential gaps in clinical workflows, particularly during periods of increased demand or limited staffing.

Staffing and conduct issues are another prominent pattern. Multiple comments describe understaffing that appears to affect the timeliness and dignity of direct care, alongside concerns about the tone or conduct of some staff members. Reviewers also flagged privacy-related practices (for example, care delivered with doors open) and inconsistencies in maintaining resident dignity. At the same time, some families found individual staff members to be welcoming and pleasant during interactions.

Facility condition and environment are frequently mentioned. The building and rooms are described as aging, with maintenance shortcomings such as exposed piping and areas needing renovation. Reviewers also noted odor and general sanitation concerns in specific areas as well as inadequate mitigation of winter hazards in the parking lot. These maintenance and environmental issues contribute to an impression that parts of the physical plant may require investment and closer operational oversight.

Management, communication, and transparency are additional patterns to note. Several reviewers reported difficulty obtaining return phone calls, accessing medical records, or receiving timely administrative responses. The facility’s social-media presence was described as active, but families reported that their attempts to engage or leave feedback were not always acknowledged and that some comments were removed or users were blocked; this raises questions about responsiveness and openness to family feedback.

Dining and activities receive some positive remarks: meal quality was described favorably by some, and at least one named social activity was mentioned. These items suggest there are elements of daily life at the facility that residents may find appealing, even as operational and clinical issues are worked through.

Recommendation: Families considering this facility should schedule an in-person tour and ask specific, documented questions about staffing levels (including night shift ratios), clinical response protocols, medication-administration safeguards, incontinence-care procedures, privacy policies, maintenance and renovation plans, winter parking and sidewalk management, and the facility’s process for handling family complaints and medical- records requests. Request recent inspection reports and evidence of corrective actions for any cited deficiencies. Observing shift change, mealtime, and an activity session can help gauge day-to-day operations and whether the positive aspects noted by some families are consistent with overall care standards.

About Arcadia Valley Skilled Nursing & Rehabiliation

Arcadia Valley Skilled Nursing & Rehabilitation sits at 25675 Main Street in Coolville, Ohio, in the village, and offers skilled nursing and rehabilitation for people who need help with daily living, whether for a short stay to recover or for long-term care. The facility has 50 certified beds, is licensed as both a skilled nursing facility and a physical medicine and rehabilitation clinic, and is known for providing a safe, warm environment with staff who speak English and sometimes other languages. Residents usually number about 45 each day. There's a 24-hour call system for safety, and residents can use an arts room, library, game room, and enjoy walking paths, a garden, fitness room, spa, and wellness room, with scheduled daily activities, movie nights, music programs, and events run by fellow residents. Nursing care lasts 12 to 16 hours a day, with about 2.99 nurse hours per resident each day, which is a little lower than the state average of 3.7 hours, and the nurse turnover rate sits just under the state average at 50%. The center offers both nursing home and assisted living support, handles rehabilitation needs, and tries to give families advice on caregiving and long-term care insurance. Medicare and Medicaid are accepted, and it's part of a Continuing Care Retirement Community, connected to Continuing Healthcare Solutions Inc, which owns and manages the place along with several listed owners. The latest report from May 20, 2024, shows 14 deficiencies, including several related to infection control, and a very serious Quality of Life and Care deficiency for not following basic life support rules like CPR before emergency help arrived, with a total of 54 deficiencies in all. A complaint on September 18, 2024, led to a $13,627 fine and an extra deficiency. Staff aim to run a friendly and caring place, but the inspection history and staffing numbers give families some facts to think about, especially for those focused on medical support and overall care standards. The community is currently not taking new patients. Directory information here gets updated at least every month so people can check for the latest status.
