    Renovated clean facility, compassionate care

I'm very pleased with our experience - the renovated, very clean facility and friendly, knowledgeable staff gave me peace of mind. Skilled nursing and therapy (physical therapy and wound care) were excellent and helped my uncle meet goals and return home safely; activities and dining were welcoming. Move-in was smooth, administration stayed on top of Medicaid/SS and kept us informed, and the team handled end-of-life needs with compassion - I'd recommend this place.

    Summary of reviews

Country Club Rehabilitation Campus presents a mixed but distinct profile. Clinical rehabilitation and therapy services receive consistent praise: reviewers highlighted effective physical therapy, wound care, and skilled nursing that supported safe discharges home. Several families described smooth transitions from hospital to facility and valued the facility’s therapy teams for being engaging and goal-oriented. Long-tenured, friendly staff and administrative assistance with Medicaid and Social Security were also cited as important strengths that can reduce family burden during transitions and benefits navigation.

At the same time, reviewers describe operational and consistency issues that prospective families should evaluate. Admissions and insurance practices were a frequent source of frustration; some families encountered restrictive or poorly explained requirements around payer guarantees and Medicaid acceptance. Linked to operations, multiple comments pointed to understaffing that manifested as delays in routine assistance (for example, delayed help with meals or personal-care tasks) and to families stepping in to support basic needs. These issues contribute to perceptions of the facility prioritizing financial or payer considerations over uniformly timely resident care.

Safety and incident response are another area of divergence. While many reviewers praised nursing attention generally, others raised concerns about fall-prevention measures and the facility’s clinical response after falls, including communication and follow-up with families. Relatedly, communication and accessibility varied: some families found staff responsive and well informed, whereas others experienced difficulty reaching staff or obtaining clear explanations after adverse events.

The physical environment and services show variability. Several reviewers reported pleasant, renovated, and well-maintained areas with comfortable rooms and acceptable to good dining; a few noted recent renovations and praised cleanliness in updated units. Conversely, other reviewers described older sections, carpet and room maintenance issues, and inconsistent housekeeping. Activities programming was generally described as active and social (bingo, singing, etc.), contributing positively to resident experience for many.

Overall recommendation: Country Club Rehabilitation Campus offers strong rehabilitative care and administrative benefits for families needing help with benefits navigation, and many families find staff compassionate and competent. Prospective residents should, however, clarify admission and insurance policies up front, assess current staffing levels and personal-care responsiveness, and tour multiple areas of the building to judge maintenance consistency. Asking specific questions about fall-prevention protocols, post-incident clinical pathways, staffing ratios, and pet policies will help determine whether the facility’s operational practices align with an individual resident’s needs.

About Country Club Rehabilitation Campus , Delaware

Country Club Rehabilitation Campus, Delaware, sits at 478 S Sandusky St in Delaware, Ohio, and has offered care since 1974. The place runs under the watch of Janet Harris, Lyle Olson, and Dianna Ortman, and it's part of the long-standing Country Club Retirement Campus network, headquartered up in Ashtabula, Ohio. There are licensed nursing home beds and certified beds, with studio room layouts and independent, assisted, and skilled nursing services all available, so people can get the level of help that fits their needs as these might change over time. This campus brings a 5-star Centers for Medicare & Medicaid Services rating and an 8.6 out of 10 from reviewers, though state inspectors have also recorded deficiencies for things like infection control and resident abuse over the years, so that's important to keep in mind along with their approval from the Ohio Department of Aging for memory and assisted living programs. The building has a wide range of services-including in-home care, respite care, therapy programs, and memory care-with a large therapy gym open for physical, occupational, and speech therapy seven days a week. The brand new respiratory unit, watched by board-certified pulmonologist Dr. Shareef, has big private rooms with private showers and covers everything from routine breathing treatments to ventilator weaning and postural drainage.

You'll find the nurses and respiratory therapists there 24/7, and wound-certified nurses also, all aiming for quick, attentive care, while personal attention comes across through custom rehab plans and dining upgrades like a private party room and restaurant-style meals. The place is easy to get around with beautifully furnished halls, a two-story atrium with plants and a waterfall, a beauty and barber shop, a library, a worship center, and a living wall made of fresh flowers and greenery. There's a theater room, lounges, computer stations with Wi-Fi, and regular cultural, social, and spiritual activities-many organized by the committees and community groups that help run and shape the campus, just as you might see in a big community center. People can schedule tours, meet the staff and other residents, join in on events like dinners and golf outings, and see firsthand the spaces where they or their family members might live, whether in independent or assisted living apartments, or in the 22-bed skilled rehab unit. Housekeeping, laundry, nutrition services, an enrichment center, and safety programs all keep the place running. They use a mix of traditional healthcare like IVs, wound care, and peritoneal dialysis while also leaning on newer technology like Vero Diagnostics Lab, noninvasive Trilogy ventilators, high flow oxygen, and CPAP/BIPAP. Over the years, the campus and its programs-both inside and outside the main building-have helped many families find a level of personal care and social connection, though anyone considering a stay should review the inspection records and talk with staff to make sure it feels right for their needs.
