    Safe compassionate care, fast recovery

    I stayed here and felt safe, cared for, and genuinely valued - compassionate, attentive staff and a personalized medical/rehab plan (including in-house dialysis and excellent therapists) helped me recover faster than expected. The campus is serene, clean and homey with good food, activities and responsive management; I'd happily recommend it.

    Spring Creek Nursing & Rehabilitation Center, found at 401 N Broadway St in Green Springs, Ohio, offers a wide range of care options and services for adults who need different types of support, and the campus there includes both Spring Creek Assisted Living, which gives people more independence with some personal help, and the main nursing and rehabilitation center itself, and to be honest when you walk the grounds you'll notice it's a peaceful spot with quiet scenery and even a mineral spring nearby which they say adds to the spot's therapeutic atmosphere, which some folks find quite soothing. The place has 120 certified beds and takes people who need skilled nursing, memory care, hospice care, ventilator or tracheostomy support, short-term rehab between hospital and home, home health, and even adult day services, so it fits a lot of different needs, and they do take most insurance plans to help families with the cost, and they have their own in-house dialysis unit, which is pretty rare in nursing homes. There's a team on staff led by Executive Director Deanna Turner, LSW, LNHA, and one of the registered nurses there is Warren Gilbolingo, and they're joined by other nurses and therapists who provide nursing care around the clock, help with rehab services like physical and occupational therapy, assist with daily living tasks, offer outpatient therapy, and even take care of meal planning with a nutrition specialist and keep things tidy with cleaning services. Management has been handled by Michael Brick and Joshua Koenig since December 2020, and ownership is through SC Op Holdings LLC along with several indirect owners, including family trusts.

    Residents who are more independent have their own living spaces, sometimes with a kitchenette, and get support if they need it but can still keep their routines, and some days will have organized group activities, spiritual programs, and chances to socialize, plus there's even salon services for personal grooming if folks want a haircut or shave.
