    Clean caring staff successful rehab

I stayed here for rehab and was very pleased - the facility is clean, admissions and sign-in were easy, meals were nutritious, and therapy helped me go home. The staff across administration, nursing, aides, housekeeping and PT/OT are caring and attentive (Stephanie truly puts her heart into care); activities, gospel singing and prayer were meaningful, and I would recommend this place to others.

    Summary of reviews

Care quality at this facility appears highly mixed. Several reviewers describe positive clinical outcomes — notably successful physical and occupational therapy programs that supported residents returning home — and praise individual caregivers for compassionate attention. At the same time, multiple accounts raise substantive operational concerns: delayed or inconsistent responses to urgent clinical needs, repeated falls with resultant injuries, and apparent variability in basic personal-care tasks such as timely assistance with toileting and bathing.

Staff descriptions are polarized. Many families highlight staff members who are attentive, kind, and effective; activity staff and certain administrators receive specific praise. Conversely, reviewers describe other employees as inattentive or brusque, and some note that personal-device use during shifts has interfered with care. There are also comments indicating uneven staff training, particularly in working with residents who have learning disabilities or cognitive impairment, which suggests variable competency across shifts and roles.

Dining and therapy receive generally favorable remarks: meals are described as nutritious, and the rehabilitation services are credited with positive outcomes. However, there are recurring comments about off-hour snack and beverage availability, indicating inconsistent night-time meal service. The activities program is a clear strength for many residents, with organized events, faith-based offerings, and a structured sign-in process noted as beneficial to engagement and socialization.

Facility conditions are similarly mixed. Some reviewers report a clean, well-maintained environment and effective housekeeping, while others describe sanitation concerns and noticeable odor issues in particular rooms or common areas. Maintenance problems such as inconsistent hot-water availability were also mentioned, implying variation in environmental services and upkeep.

Management and organizational practices are an area of concern for reviewers. There are statements about inconsistent communication with families, questions about honesty from staff or activities personnel, and at least one allegation of documentation irregularities, which raises issues about incident transparency and recordkeeping. These management-related themes compound clinical concerns because they affect families’ ability to obtain clear information after adverse events.

Overall pattern: the facility appears to combine genuine strengths (dedicated caregivers, effective therapy, and a robust activities program) with recurring operational weaknesses that affect safety, basic personal care, and transparency. Prospective residents and families should weigh the positive rehabilitation and engagement offerings against reports of inconsistent staffing responsiveness, fall-prevention gaps, sanitation and maintenance variability, and communication/documentation concerns. When evaluating this facility, consider direct observation of care routines, specific questions about emergency protocols, fall-prevention measures, staff training, night-time coverage, and procedures for family notification and incident documentation.
