    Compassionate care and peaceful passing

I'm very pleased with this facility - the caring, friendly staff (nurses like Amy and Doug; Monica and Lynette went above and beyond) treated my loved one like family, provided a private room and a peaceful passing. Great meals and nightly snacks, excellent therapy and engaging activities really helped with rehab, and the new management and leadership feel proactive and knowledgeable. I'd highly recommend it.

    Summary of reviews

Reviews of Oakhill Manor Skilled Nursing & Rehabilitation present a mixed and at times polarized picture. Several reviews describe clear operational weaknesses that affected clinical safety and daily living, while others note meaningful improvements under recent leadership and praise specific departments such as therapy. Prospective residents and families should weigh both the recurring operational concerns and the areas of strength when assessing the facility.

Care quality: A recurrent theme concerns medication administration and clinical escalation. Families cited instances of medications being given without clear orders or late administrations, as well as delays in arranging transfers to higher levels of care. These issues indicate gaps in medication-administration controls and in the facilityโ€™s clinical escalation processes. Conversely, the rehabilitation/therapy program received consistent positive remarks for effectiveness and staff expertise, and some reviewers reported that new nursing leadership has improved clinical oversight.

Staffing and staff conduct: Reviews describe two distinct staffing narratives. Many reviewers expressed concerns about chronic understaffing, high turnover, and slow responses to call lights and resident requests, which they linked to delays in hydration, toileting assistance, and comfort care. At the same time, other reviewers characterized direct-care staff as caring, diligent, and proactive, and several accounts highlighted committed nurses and aides who check residents regularly. Multiple reviews credit recent administrative changes with improved staffing levels and morale, though some families remain skeptical about whether those improvements are sustained.

Dining and activities: Dining received mixed feedback. Some families complained about inconsistent meal service, inadequate meal options for dietary needs, and earlier poor food quality. Other reviews noted that food quality has been improving. The activities program was frequently described as engaging, and some staff in activities were praised for responsiveness and creativity. Meal consistency and matching food to documented dietary requirements are areas noted for continued attention.

Facilities, maintenance, and security: Reviewers identified maintenance-related service interruptions (notably hot-water availability) and sanitation or odor concerns in certain areas. There are also serious operational concerns about security and control of resident belongings โ€” including accounts of unsecured storage and missing items โ€” which suggest weaknesses in property- and access-control processes. Families should confirm the facilityโ€™s current practices around asset security, maintenance responsiveness, and environmental cleanliness.

Management and communication: Several reviews referenced recent leadership turnover, including a new administrator and director of nursing; these changes were linked to reported staffing improvements and more consistent care by some families. However, other reviewers described past poor management, inconsistent family communication, and inadequate hospice coordination. There are also notes of insurance and admission-process disputes, including at least one alleged discriminatory denial; such matters may warrant further inquiry by families and their insurers.

Overall pattern and guidance: The pattern across reviews is one of significant variability โ€” strong therapy services and committed direct-care staff coexist with operational shortcomings in medication management, staffing consistency, maintenance, and security. Recent administrative changes appear to have produced tangible improvements in several accounts, but skepticism remains among some families. Prospective residents and families should arrange an on-site visit, ask for current staffing and medication-administration policies, review recent inspection or licensing reports, and request specifics on security measures for resident belongings and on how the facility manages clinical escalation and hospice coordination before making a placement decision.

About Oakhill Manor Skilled Nursing & Rehabilitation

Oakhill Manor Skilled Nursing & Rehabilitation has served seniors for 22 years, offering a mix of short-term rehab and long-term nursing care for people who need daily help or recovery support, and the place sticks to simple, honest care, not fancy promises or big claims, just a real effort to help residents feel safe and respected. The building allows 99 residents in its 99 units, with rooms that offer private bathrooms, cable TV, kitchenettes, telephones, air conditioning, and Wi-Fi, and every room is built with safety and accessibility in mind. The facility accepts both Medicare and Medicaid, and is not a Continuing Care Retirement Community, but it does focus on skilled nursing, memory care, hospice, respite, and rehabilitation services with specialized staff and registered nurses offering 12-16 hour nursing shifts and 24-hour basic supervision, which helps people with both simple and complex needs, including those related to Alzheimer's and dementia, managing wound care, stroke recovery, diabetic care, cardiac care, and even psychiatry, podiatry, dental, and hearing care.

Therapy services include occupational, physical, and speech-language therapies, which helps residents keep up their strength and daily living skills, and staff handle medication management, personal care like bathing and dressing, incontinence care, and transfers for those who need help getting in and out of bed or chairs. Residents enjoy two professionally prepared meals a day with room for special diets like allergies and diabetes, and seating in a communal dining hall makes mealtime more social, while a dining room and restaurant-style options try to make it feel homier, and there's always time for snacks, coffee, and conversation. Oakhill Manor has regular activities to keep people busy and connected, like bingo, arts and crafts, daily exercise, devotional opportunities, off-site events, and outings for those who want to get out, plus on-site movie theater nights, walks in the gardens or along outdoor paths, trips to a business room or fitness center, and visits with friends in the living room, library, or activity areas. There's even a hair salon and spa for a little extra comfort, and the game room's good for cards or just relaxing.

For those in need of close monitoring, such as people with any stage of Alzheimer's or dementia, the place runs a separate memory care program led by trained nurses and has dedicated care units with heightened security and a 24-hour emergency call system for everyone's safety. The staff works with a resident council, and while there isn't a mention of a family council, family support services and guest amenities make it a bit easier when loved ones visit. Housekeeping, laundry, and move-in help are part of daily routines, making life less stressful for residents.

Inspections have noted a handful of deficiencies in the past, but nothing severe, and the most recent inspection gave a solid B grade, which puts Oakhill Manor in a decent position for care quality and safety. The building's not new, but management seems to focus on steady upkeep to keep the facility comfortable for seniors, and the main focus stays on personalized care, safety, and maintaining a good quality of life without trying to impress anyone-just helping each resident live as well as possible, regardless of health or mobility changes.
