    Clean compassionate staff helped mom

I'm very pleased with Altercare of Mentor. The facility is spotless with a pleasant, wide-open atmosphere, and the nurses, aides and hospice team are compassionate, attentive, and keep families well informed - they go above and beyond, resolve issues quickly, and helped my mom make real progress. I'd recommend this place and would use it again.

    Summary of reviews

The reviews indicate a polarized experience at Concord Ridge Health & Rehabilitation. Many families praised the clinical and compassionate side of care: nursing staff, aides, and hospice partners received repeated commendation for attentive bedside care, end-of-life support, and therapeutic progress during rehabilitation stays. Several reviewers highlighted clean rooms, active maintenance, and a facility undergoing positive renovation efforts; management and certain administrators were also credited for responsiveness and for resolving issues when they arose.

At the same time, a persistent pattern of operational weaknesses emerges. A number of accounts describe slow or inconsistent responses to call alerts and requests for assistance, which reviewers interpreted as gaps in staffing coverage or prioritization. There are recurring concerns about staff professionalism in specific interactions—cell-phone use while on duty, curt communication, and inappropriate language were mentioned—leading to variable resident experiences. Relatedly, personal-care practices were described as inconsistent, including delays in attending to toileting or dressing needs; these observations point to uneven care routines rather than isolated clinical errors.

Dining and activity-related feedback is mixed. Meal quality was characterized variably as adequate to poor, suggesting inconsistent menu execution or appeal rather than a systematic absence of food service. Few reviews addressed programming or recreational activities in detail; when present, comments were neutral to positive, with more emphasis placed on therapeutic and nursing services than on an organized activities program.

Facility and management themes are similarly mixed. The building is described as older but with "good bones," and maintenance and housekeeping earned praise for keeping rooms orderly and largely odor-free. Renovation activity was noted and sometimes described as disruptive; reviewers also described comfort issues such as bedding and furnishings that might benefit from replacement. Management responses drew both positive and negative remarks: some families experienced timely intervention and clear communication from leadership, while others perceived gaps in accountability and follow-through when concerns were raised.

Notable patterns for prospective residents and families: expect variability. With the same facility, experiences range from highly positive—compassionate, skilled care and clean surroundings—to concerning operational issues around responsiveness, personal-care consistency, and property management. If considering this facility, prioritize direct conversations about staffing levels on the intended unit, protocols for call response and incontinence care, procedures for safeguarding resident belongings, and how ongoing construction will be managed during a resident’s stay. Visiting during different shifts and speaking with both nursing leadership and current families can help clarify how the facility’s strengths and weaknesses will affect an individual’s needs.
