    Rehab restored mobility; compassionate staff

I did rehab here after surgery and was very pleased. Therapy and wound care were excellent and nurses and aides were compassionate and proactive (Shirley and Ryan stood out) - they helped restore my mobility and get me home. The building was clean, staff supportive, activities plentiful, and social work/management were helpful; overall a very positive experience.

    Summary of reviews

The collected feedback presents a mixed portrait of Greenbrier Health Center: there are clear, consistent strengths in therapy and activity programming alongside recurring operational concerns that families should assess closely. Many accounts praise the therapy teams (physical and occupational), noting good rehabilitation outcomes and timely discharges home. The site offers an active schedule of social and recreational activities, including off-site trips via a facility shuttle, a cafรฉ/snack area, salon services, and counseling/social-work support. Several reviewers described small-unit or private-room arrangements and clean, welcoming common areas that contribute to a residential atmosphere in parts of the campus.

At the same time, multiple patterns of concern emerge. Staffing consistency and responsiveness are frequent issues: reviewers describe understaffing on some shifts, long waits for assistance with basic needs, and variability in aide attentiveness and training. Those operational gaps are linked in several accounts to delayed transfers, slow toileting and repositioning assistance, and inconsistent oversight during high-risk activities. Families also reported variability between units and shifts in how promptly clinicians and aides respond, which contributes to an uneven care experience.

Dining and nutrition are an area of repeated dissatisfaction. Many comments point to poor meal quality, improper temperature control, repetitive menu choices, and inadequate preparation of texture-modified diets. There are also specific concerns about meal suitability for residents with dietary restrictions. Reviewers described instances where dining service practices did not meet expectations for consistency or nutrition.

Facility and housekeeping feedback is inconsistent. Some reviewers described the building and shared spaces as clean and well-maintained, while others noted sanitation and pest-control concerns in resident rooms, linen shortages, overflowing waste carts, and odor issues in certain areas. These mixed observations suggest variability in housekeeping performance across units or over time rather than a single uniform standard.

Management, communication, and clinical oversight are additional focal points. Several families reported poor communication from administration or social-work staff, unclear coordination of care, and frustration with discharge planning or billing practices. Clinical concerns include gaps in medication administration, pain and end-of-life management delays in some cases, and accounts of pressure-injury prevention and wound-care issues; a small number of reports describe serious clinical incidents and concerns following a resident's death. There are also remarks about property-security gaps and incomplete documentation that families found troubling.

In summary, Greenbrier offers strong rehabilitative therapy and an active activity program with pleasant shared spaces and some engaged staff. However, the facility appears to have inconsistent performance in core operational areas: staffing reliability, basic-care responsiveness, meal quality and nutrition management, cleanliness in resident rooms, and administrative communication. Prospective residents and families should tour the specific unit they would occupy, ask for unit-level staffing ratios and recent inspection results, review dietary accommodations and sample menus, inquire about infection-control and pest-management protocols, and clarify communication procedures and escalation pathways for clinical concerns before making placement decisions.

    About Greenbrier Health Center

Greenbrier Health Center is a nursing home that offers care for seniors who need skilled nursing services, long-term care, or support after surgery, and it also provides short-term rehabilitation, outpatient rehab, and subacute medical care. The center has 162 certified beds and usually about 124 residents each day, and the staff includes nurses who provide about 3.19 hours of care per resident each day, which is lower than the state average of 3.7 hours, and the nurse turnover rate is 64.6%, which is higher than the state average of 51.0%. The place is managed by Health Care Facility Management, LLC since 2004, and it's part of CommuniCare, a company that specializes in post-acute care, and it has no public information about how it manages its employees. Greenbrier Health Center has specialized care like in-facility adult day services and provides care for residents with Alzheimer's or dementia, and families can help plan a resident's care, while residents get access to services such as therapy, counseling, and regular doctor check-ups, and there are activities like gardening, community outings, and an annual picnic.

The facility has a modern look, shared common areas, a hair salon, and dining services where residents can eat together, and it offers respite care too. Residents have access to transportation services, physical therapy, occupational therapy, and speech therapy, as well as equipment for patient use. Greenbrier Health Center accepts Medicaid as a payment option. It has a history of 39 deficiencies noted in inspection reports, including four deficiencies for infection control that don't meet federal standards, and there have been complaints about not providing safe pain management or planning resident discharges based on each resident's needs and wishes. Inspection records show issues with assessing residents and planning their care, especially related to managing pain. The environment tries to be supportive, focusing on both physical and emotional well-being, but it's important to note the regulatory concerns and nurse staffing challenges.
