    Compassionate, clean, excellent rehab facility

I had a very positive experience at Ayden - the staff are genuinely caring and friendly, from nurses like Katie and Jackie Wilkinson to the rehab team. The facility is clean, bright, well-staffed and rehab-focused; Tim and Logan gave a wonderful tour. My family felt welcomed and confident in their care, and I'd recommend this compassionate, professional place for short- or long-term stays.

    Summary of reviews

The reviews reflect a mixed experience at this facility. Many families and short‑term rehab residents praise the caregiving team, therapy services, and the activities program, and several reviewers described a clean, bright environment with responsive maintenance and an efficient clinical team. At the same time, a subset of accounts raises substantive operational concerns that prospective residents and families should consider when evaluating placement.

Care and staff: Numerous reviewers emphasized compassionate, personable staff and effective rehabilitation/therapy programs; individual staff members and teams were singled out for particularly positive interactions. However, other accounts describe inconsistent responsiveness, unprofessional conduct at times, and variability in staff communication with families. These contrasts suggest that day‑to‑day care experience may depend on staffing assignments, shift, or individual caregivers.

Safety, infection control, and sanitation: Multiple reviews describe issues that point to weaknesses in infection‑prevention practices and environmental housekeeping. Specific concerns included hand‑hygiene lapses, sharps-disposal and protective‑garment practices, and inconsistent cleaning of resident bathrooms and common areas. Several reviewers also reported repeat falls or transfer‑related safety problems, indicating potential gaps in fall‑prevention protocols and monitoring. Regulatory complaints and ombudsman involvement were mentioned in some accounts; families should review the facility's current compliance and quality reports and ask for infection‑control and safety policies during a tour.

Dining, activities, and facilities: Feedback about meals is mixed — some residents found the food appetizing, while others described limited choices or inconsistent quality. The activities department received positive comments for engagement and programming. Physical facilities were frequently described as bright, clean, and well maintained, though some reviewers noted tight room layouts and policy restrictions that limited freedoms for some residents.

Management and communication: Experiences with administration varied. Positive notes referenced recent leadership changes and a more welcoming atmosphere under new management; negative notes focused on front‑desk and HR communication issues. There were also accounts indicating concerns about property security and response to serious incidents. Prospective families should ask about staffing ratios, property‑security measures, incident‑response procedures, visitor and resident‑privilege policies, and how the facility communicates with families following clinical events.

Recommendation: The facility demonstrates clear strengths in caregiving, rehabilitation services, and aspects of campus upkeep, but the pattern of comments about infection prevention, sanitation consistency, fall safety, and administrative responsiveness warrants careful inquiry. A recommended approach for families is to schedule an on‑site tour, request recent inspection or quality reports, meet therapy and nursing leaders, and ask specific questions about infection‑control protocols, housekeeping schedules, fall‑prevention plans, property‑security safeguards, and how the facility handles and communicates about incidents.

    About Ayden Healthcare of Piqua

Ayden Healthcare of Piqua sits at 275 Kienle Drive in Piqua, Ohio, and people know it as a place focused on general healthcare services for seniors, whether someone needs assisted living, memory care for Alzheimer's or dementia, or skilled nursing and rehabilitation. The building has 99 certified beds, but they usually care for about 81 residents every day. They provide a range of services including therapy for stroke recovery, physical and occupational therapy, care after orthopedic surgery, wound care, palliative and hospice care, and physician services, and those with complex medical needs have help right there. You'll find modern accommodations with quiet spaces and some nice outdoor landscaping so folks can enjoy being outside or gather together inside in the common rooms. The staff aims to give support with laundry, housekeeping, and up to two prepared meals a day, so residents don't have to worry about those chores, and the nurses spend about 3.17 hours with each resident daily, which is less than the state average, as some folks have pointed out.

Management comes from Ayden Healthcare, with Adam Cusner handling things since January 2025 along with Jeffrey Degyansky, Eliezer Goldish, Solomon Kazarnovsky, and Abba Stein, all joining in May 2024. Now, the facility has had 40 deficiencies in reports, including four related to infection control, and people have raised concerns about timely reporting of suspected abuse, neglect, or theft, issues that were officially noted in 2025, and there's also the matter of their last standard inspection being more than two years ago, which is longer than the regular 9 to 15 month interval. Even so, there's a real effort from the team to provide warm, compassionate care, with programs that encourage independence and dignity, open social activities for residents, and a feeling of community as a focus. The place gives options for both active independent seniors and those who need higher levels of nursing or rehab. Residents can choose comfortable living spaces, from residential homes to apartment-style rooms, with medical care available as needed and extra support for memory care. The healthcare professionals at Ayden Healthcare of Piqua try to make things feel a little more like family, always aiming to enhance wellness, encourage socializing, and help each person feel at home in their own way.
