    Caring staff, improved facility, recommended

I'm impressed by the caring, family-like staff - Miss Adria, Nurse Emily, Don and the social worker went above and beyond. The facility is clean, meds are on time, and therapy and nursing are attentive and professional. New management has improved rooms, food and staff teamwork; my father was treated like family and was happier here. I recommend this place as it continues to build on real progress.

    Summary of reviews

Reviewer feedback presents a polarized view of Buckeye Terrace Rehabilitation and Nursing Center, with distinct clusters of positive and negative experiences. Positive accounts highlight strong rehabilitative therapy, supportive social-work services, and individual staff members who provide attentive, person-centered care. Several comments emphasize improvements following leadership changes and identify specific employees who contributed to better outcomes. These favorable impressions commonly reference effective therapy, coordinated short-term rehab, and moments of compassionate engagement that families found meaningful.

Conversely, a substantial set of reviews describes enduring operational weaknesses. Clinical care quality appears inconsistent: nursing and aide performance varies across shifts, and reviewers describe delays in attending to personal-care needs and wound-care concerns. Several commenters raised issues with medication administration and staff clinical knowledge, as well as unreliable handovers and poor cross-shift communication that contribute to inconsistent care continuity.

Dining and nutrition are recurring concerns. Reviews describe interruptions to regular meal service, instances of dietary management errors (including serving foods contrary to allergy needs), and general dissatisfaction with food quality and meal delivery processes. Conversely, some recent comments indicate incremental improvements in food and service under new staffing arrangements.

Facility, environmental, and activity-related issues are also prominent. Multiple accounts mention sanitation and pest-control concerns, odor issues in some areas, and delayed housekeeping or linen changes. Maintenance and safety deficits — including lighting, peeling finishes, and isolated structural incidents — were cited and have raised questions about environmental safety. Activity programming and access to outdoor areas are described as limited at times, with reports of early closures or restricted resident outings.

Management and culture show mixed signals. Several reviewers praised recent administrative responsiveness and named staff who provided exemplary support. At the same time, persistent criticisms point to weaknesses in leadership oversight, staff training, and workplace culture. Serious allegations include theft and other staff misconduct, concerns about inappropriate relationships among staff, and pressure to manipulate online reviews; these claims suggest areas requiring regulatory or investigatory follow-up. Safety and security concerns — including behavioral incidents and inconsistent supervision — were also noted.

Notable patterns: the facility appears to be in transition, with some reviewers observing tangible improvements under new management while others describe longstanding operational failures. The most consistent strengths are therapy and social-work support; the most consistent weaknesses are staffing consistency, sanitation and maintenance, meal-service reliability, and communication/oversight practices.

For prospective residents and families: arrange an unannounced visit if possible, ask for recent state inspection results, inquire about current staffing levels and staff turnover, request documentation of pest-control and housekeeping schedules, review medication-administration and wound-care protocols, and meet the social-worker and therapy teams. Confirm dietary accommodations and daily personal-care routines (including bathing schedules and linen change frequency) before admission to help evaluate whether current operations match your expectations.

About Buckeye Terrace Rehabilitation and Nursing Center

Buckeye Terrace Rehabilitation and Nursing Center sits in Westerville, Ohio, and is a for-profit center with 70 certified beds, managed by Northwood Healthcare Group LLC since September 2017 and has Ephram Lahasky as an involved owner, with business led by Ms. Marlen Radbill as President, Mr. Lawrence Deering as CEO, and Mr. Joseph Conti as President; and you'll see they provide skilled nursing and rehabilitation services every day by appointment only, caring for both short-term rehabilitation patients and long-term residents. They offer services like physical, occupational, and speech therapy, as well as memory care, wound care, pulmonary, and respiratory therapies, aiming to develop personalized care plans for each resident, with help from a Director of Nursing leading the care team, and support from qualified healthcare staff, though nurse turnover is higher than average at 68.8% compared to the state's 51%, and the staff nurse hours per resident each day, at 3.48, sits a bit below the state average of 3.7. Buckeye Terrace offers social activities every day, therapy programs, respite care for caregivers needing a break, and social services to help families through changes, and they focus on making everyone comfortable with a home-like environment designed for both accessibility and comfort, plus medication management to look after health needs and support through transitional times. While they promise dedicated attention and a nurturing atmosphere, inspections have noted some deficiencies, with 95 total citations listed in inspection reports, including cases relating to resident rights, dignity, and self-determination (though listed as causing no actual harm but with potential for more than minimal harm and affecting few people), some gaps in following resident care orders or preferences, and two infection-related issues that did not create actual harm, but could have, noting federal standards were not fully met. The facility is licensed and accredited, and they're known for providing a wide range of therapy and long or short-term skilled nursing, though they've had inspection issues you should keep in mind as you decide what's right for you or your loved one.
