I placed my mom here and I'm very pleased overall - the facility feels homey, is always clean and secure, and the staff genuinely care. Meals are varied and timely, activities are small and intimate so residents get real interaction and near one-on-one attention, and administrator Kaylene is responsive and an advocate (little touches like Donut Fridays show the personal care). The move-in was smooth and staff were very accommodating; I would recommend this family-style community.

Summary of reviews
The reviews for Pacific Living Centers of Medford at Brookhurst describe a facility with clear strengths in personal, family-oriented caregiving and a small-household atmosphere. Many families praise individual caregivers for being compassionate, attentive, and familiar with residents’ preferences; several comments highlight an administrator (Kaylene) whose hands-on advocacy and visible leadership have produced noticeable improvements in daily life and family confidence. The facility is repeatedly described as secure, homey, and, in many areas, clean and well-decorated, with timely meal service and special food events that residents enjoy.
Care quality appears mixed. Positive accounts emphasize near one-on-one attention and staff who treat residents like family; these accounts note attentive admissions support and staff who help with transitions. However, there are repeated operational concerns that affect care consistency: reviewers describe unpredictable staffing levels and scheduling reliability that sometimes leave staff overwhelmed. These staffing inconsistencies are associated with delays in routine monitoring, inconsistent assistance with dressing and feeding, and concerns about responsiveness to personal-care needs. Reviewers also raised sanitation concerns in particular areas and noted variability in housekeeping and odor control.
Dining and activities show strengths and limitations. The dining program is frequently described as varied with enough options and timely service; some families appreciated weekly social food events. Activity offerings exist (an activity board with prescribed sessions) and the small resident population can foster meaningful interactions, but several reviewers felt programming was limited in scope or frequency and that resident engagement could be greater.
Facility and property observations are generally positive: reviewers mention a secure, well-maintained interior and spacious rooms, with the small size enabling more individualized attention. At the same time, there are reports of inadequate personal-effects controls (clothing mixed or lost) and occasional odor or sanitation issues in specific common or bathroom areas, indicating uneven execution of housekeeping processes.
Management and operational patterns show both improvement and friction. Some families describe a positive, responsive administrator and clear improvements in care under current leadership. Other comments indicate communication breakdowns, scheduling miscommunications, and management practices perceived as creating staff stress. There are also concerns about clinical oversight and medication-administration controls; one allegation referenced insulin administration without documented delegation, which raises questions about clinical delegation and oversight procedures. Concerns about incident-reporting, mandated reporting procedures, and family communication following critical events were also raised.
Notable patterns for prospective residents and families to consider: the facility’s small size and engaged staff can deliver a warm, family-like environment and individualized attention, particularly when staffing and leadership are aligned. Conversely, inconsistent staffing, variability in housekeeping and personal-effects management, and lapses in routine monitoring or clinical oversight have been described and could affect reliability of daily care. Families touring the facility should ask specific questions about current staffing ratios and schedules, medication-delegation and clinical oversight policies, laundry and belongings tracking, sanitation protocols for common and bathroom areas, and the frequency and diversity of activities to assess whether the site’s operational practices match their expectations.

About Pacific Living Centers of Medford at Brookhurst
Pacific Living Centers of Medford at Brookhurst sits on Brookhurst Street in Medford and serves as a residential care facility with a focus on smaller, cozy living. The home has 14 memory care beds, designed for seniors living with Alzheimer's and other forms of dementia, and the rooms and common spaces feel more like a house than an institution, decorated to be comfortable and familiar, and offering short hallways and open kitchens, which helps residents move about easily and even take part in cooking. The community provides several levels of care, including independent living, assisted living, memory care, nursing home, long-term care, and even hospice, so people can get more help as their needs change, and there's also home care and adult day services if someone doesn't want or need to live there full time. The facility offers many kinds of rooms, with kitchens or kitchenettes and washers and dryers, and the building has safety and handicap features along with a sprinkler system for fire protection, secure entry, and key code visitor access to keep residents safe, while the grounds are clean, spacious, and suitable for walking or enjoying the outdoors. Staff at Brookhurst aim to build a caring, family-like environment, and the small number of residents in the memory care part helps them offer close attention and comfort, with people saying it's more like a family home than a big facility. There are health services such as nursing, wound care, podiatry, occupational therapy, medication support, and personal care assistants help with tasks like dressing and grooming. The meals can be tailored for special diets and everyone can dine together, and there are activities like arts and crafts, social gatherings, educational programs, and health and wellness classes. Residents have access to a fitness center, garden, game room, salon/barbershop, cable TV, internet, parking for guests, and even transportation if they need it, plus someone keeps the place clean, does the laundry, and takes care of the rooms. Financial guidance and various payment plans are available, including options for long-term care insurance, and people interested in moving in or just learning more can schedule a tour or an assessment to see the place and meet the staff and residents. There are photos to look at if you want to get a feel for the community before visiting.
