    Compassionate skilled rehab restored independence

I spent 35 days here after a spinal injury and couldn't be more grateful. The therapy team (Richelle, Sam, Joe, Ken and others) were skilled, compassionate, and got me walking again and safely home; nurses and aides were friendly and knew me by name. The facility felt clean, safe, and welcoming, rooms were pleasant, meals and activities were enjoyable. Overall a very strong, caring rehab experience-I would use them again.

    Summary of reviews

Overall impression Meadows Nursing & Rehabilitation Center is frequently described as a therapy-focused nursing facility with strong clinical teams and a largely positive, family-oriented culture. Many reviewers emphasize the facility’s strengths in rehabilitation, naming physical and occupational therapy staff who produce measurable functional improvements and successful transitions back to home. Nursing and aide staff are repeatedly characterized as compassionate, attentive, and personally engaged with residents; families commonly note staff who learn residents’ names and provide educational guidance for home care and equipment. Clinical care and therapy Therapy services are a clear institutional strength. Reviewers cite skilled PT/OT teams, individualized therapy plans, and staff who push appropriately to restore mobility after injury or surgery. Several accounts highlight coordinated discharge planning, equipment recommendations, and follow-through that supported safe returns home. Medical and nursing oversight are generally described as professional, and responsiveness to call bells is often praised. Dining and activities Residents and families describe an active auxiliary and regular programming — bingo, music nights, and occasional outings were noted as positive social features. The facility offers both dining-room and in-room meal options and a varied menu. At the same time, there are recurring comments about inconsistent meal temperature and occasional food-service reliability issues; some families requested clearer meal documentation for residents with specific dietary needs or preferences. Facilities, privacy, and infection control Facility cleanliness is frequently mentioned in positive terms (clean, well-kept, spotless), but there are also isolated but significant sanitation and infection-control concerns raised in other accounts, including at least one mention of a communicable-organism issue. Room accommodations are mixed: some reviewers describe pleasant, comfortable rooms, while others note small shared rooms with curtain dividers and limited privacy. These contrasts suggest variability across units or shifts. Additionally, some reviewers raised concerns about pressure-ulcer prevention and skin-integrity monitoring, indicating areas where clinical surveillance may be inconsistent. Management, administration, and finances Administrative interactions (admissions, insurance explanations, and care-plan coordination) are often described as seamless and helpful; several families appreciated clear communication about coverage and discharge plans. However, affordability and insurance-coverage limitations are a consistent operational constraint for some families. A few serious family-level concerns touched on informed-consent processes and documentation; those accounts point to the need to confirm consent and communication practices during admission and care transitions. Notable patterns and guidance for prospective families The dominant pattern is a facility with strong rehabilitation outcomes and a compassionate staff culture, tempered by intermittent operational shortcomings: variable staffing, occasional sanitation and infection-control concerns, limited privacy in shared rooms, and inconsistent meal-service quality. Prospective residents and families would benefit from an in-person visit that includes: tour of the specific unit and room assignment, questions about current staffing ratios, review of infection-control protocols and recent inspection records, clarification of meal-service procedures, and explicit discussion of informed-consent/documentation practices and financial/insurance expectations. These checks will help determine whether Meadows’ substantial clinical and therapy strengths align with an individual’s needs and risk tolerance.

A large 3,000-square-foot therapy gym gives plenty of room for physical, occupational, and speech therapy, and the rehab team helps with both inpatient and outpatient therapy, with programs like Recovery at The Meadows focused on getting people stronger and back on their feet. The staff includes nurses, therapists, physicians, and a medical director on-site, and specialists in medication management help keep everything safe, with extra medical services like optical, dental, audiology, and podiatry taken care of without leaving the campus.

There are family caregiver support services and a private family room for visits, and people can find homestyle meals in communal dining areas, with tables that adjust in height and meals suited to dietary preferences. Residents can join in daily activities, events, and outings, or visit the on-site chapel and beauty or barber shop, and there's always something happening, whether it's a craft fair or a golf tournament. The Meadows Nursing & Rehabilitation Center has a rating of 3.9 from 15 reviews, and skilled nursing services earned a 5-star rating, and people say it feels warm and welcoming, with attention to compassion and dignity. Being part of The Meadows Campus, the center connects with other living options like The Meadows Apartments and Hi-Meadows Apartments for independent living, and The Meadows Manor for personal care. There are virtual tours for anyone who can't visit right away. The center has a history of community engagement and wants to offer expert guidance, comprehensive resources, and a comfortable place for each resident. Meadows Nursing & Rehabilitation Center is listed as a rehabilitation center associated with Misericordia University and recognizes the need for age-friendly health systems, with a focus on quality and affordability for the community it serves.
