    Exceptional rehab and attentive nursing

My mom received outstanding rehab here - the physical therapy was exceptional and the nursing staff were attentive and prompt. Staff were friendly, easy to communicate with, and did an amazing job especially during COVID; activities and little touches like a home-cooked Christmas dinner were wonderful. The facility is very clean, bright and well-run - not glamorous, but a great fit for our family and rural area; we're very grateful for the care.

    Summary of reviews

Reviews of Transitions Healthcare Autumn Grove are highly mixed, with distinct clusters of positive and negative experience. Many families praise the facilityโ€™s rehabilitation services, noting strong physical-therapy programs and good outcomes for short-term rehab residents. Activity programming and special events (including holiday meals) are frequently cited as strengths, and some reviewers describe attentive nursing aides, friendly administrative staff, and clean, bright common areas. The facilityโ€™s VA approval and instances of clear, timely family communication are additional positive elements that families referenced.

At the same time, a substantial set of concerns recurs across reviews. Sanitation and odor-control issues in resident rooms and shared spaces are described repeatedly, along with related incontinence-care delays. Reviewers also raise ongoing staffing problems โ€” both chronic shortages and reliance on agency staff โ€” which appear to contribute to inconsistent day-to-day care and interruptions in routines. Several accounts point to gaps in medication administration and recordkeeping; at least one review described a medication-related clinical escalation requiring hospitalization. Concerns about responsiveness โ€” for example, delays in attending to toileting and bedside needs โ€” are prominent and tie back to staffing and supervision issues.

Infrastructure and amenities receive mixed comments. Positive mentions note clean, bright common areas and occasional special meals, but other reviewers describe broken equipment (wheelchairs, TVs), worn furnishings, and a need for repainting and upgrades. Food quality is uneven in reviewersโ€™ experiences, ranging from enjoyable holiday/home-cooked meals to complaints about overcooked or unpalatable daily fare. Activity availability is often a strength, especially during the week and for rehab patients, but some families cited limited weekend programming and inconsistent opportunities for outings.

Communication and management present a clear pattern of variability. Several families describe helpful, communicative staff and an approachable administrator, while others report slow or nonresponsive phone communication, missed updates, and lapses in scheduling medical appointments. Security controls and personal-property management are additional areas of concern: missing items and allegations of theft (including narcotics diversion) were raised and contribute to distrust among some families. Finally, visitation and unit-access policies are described as inconsistent by reviewers, with at least one account of restricted visiting that family members found problematic.

Overall, the facility appears to provide capable rehabilitation and, in many cases, caring hands-on nursing and activities support. However, persistent operational weaknesses โ€” sanitation and odor control, inconsistent staffing and supervision, medication-management gaps, property maintenance shortfalls, and uneven communication โ€” create a split picture of quality. Prospective residents and families should tour the unit(s) of interest, ask for current staffing levels and turnover data, request recent state inspection and medication-management audit results, and verify oversight measures (security, cameras, or pharmacy controls) before making a placement decision.

    About Transitions Healthcare Autumn Grove

Transitions Healthcare Autumn Grove sits at 555 South Main Street in Harrisville, PA, and serves older adults with all sorts of needs, so you'll find people staying for short-term rehab, others there long-term, some using assisted living, and others needing help with memory and dementia care, so they've set themselves up to offer just about every type of care you might think folks might need as their health changes. The place has skilled nursing 24 hours a day, offers help with daily tasks, and provides rehab services like physical, occupational, and speech therapy; they work with people who are recovering from strokes, those needing pulmonary care with round-the-clock monitoring, and even have peritoneal dialysis that can be done right in the room if need be, plus wound care and programs for people managing heart failure or memory loss, which can be a comfort for families. Their "Transitions Rehab to Home" program focuses on helping people move from rehab back to living on their own or in a less intensive care setting, and they've updated things lately to make the rooms and common areas more comfortable and modern, so common spaces feel brighter and residents can visit with each other or family. The rooms, whether private or semi-private, are designed for comfort and there's always staff around, including nurses and therapists, making sure needs don't go unmet. They try to keep folks active and well by offering meals managed by a nutrition team, and the professional staff tailors care to each person, so whether it's managing medicines, helping someone walk again, or dealing with the tough parts of dementia, there's someone around who knows how to handle it. Transitions Healthcare Autumn Grove is part of a bigger network under Transitions Healthcare, LLC, and they try to coordinate well with hospitals and community groups, connecting residents to resources for mental health and recovery, plus they even provide support with post-incarceration reentry and participate in health programs funded by the state and federal government, using a health library and online tools to help guide residents and families. People can take part in community events, get help with advocacy, and use interactive tools like a symptom checker if needed, which is handy for families who want to keep track of things. All told, Transitions Healthcare Autumn Grove offers a range of care services in a setting that tries to be as homelike, supportive, and flexible as possible, meeting people where they are with what they need, whether that's short-term rehabilitation, long-term support, or specialized care for memory, heart, stroke, or complex medical conditions.
