    Compassionate staff and excellent care

I had a very positive stay - compassionate, friendly staff consistently went above and beyond, nurses and therapists were professional and effective, and the activities director kept me engaged. The facility was clean, meals were good, management and social work were responsive, and I left feeling cared for and secure.

    Summary of reviews

Reviews for Maple Ridge Rehabilitation and Healthcare Center present a mixed picture: many families and former residents praise the clinical rehabilitation services, compassionate caregivers, and an engaging activities program, while others raise operational concerns that affect day-to-day living. Positive comments highlight strong therapy teams (physical and occupational), attentive nursing leadership in some units, a welcoming admissions and social-work presence, and staff who go beyond expectations to make residents comfortable. Several reviewers credited the facility with successful rehab outcomes and transitions back home, and noted a helpful business office and efficient payroll solution that supports staff (Next Day Pay).

Care and staffing: Clinical strengths are most apparent in the therapy department and among staff who are described as compassionate and patient-centered. However, recurring operational issues center on inconsistent staffing levels and a dependence on agency nursing personnel, which reviewers linked to variability in day-to-day care. Commonly cited operational consequences include delays in responding to call lights and personal-care needs, gaps in clinical oversight and continuity of care, and scheduling coordination problems. There are also concerns about staff conduct and communication tone in certain units; some reviews included more serious allegations of discriminatory behavior and harassment, and other accounts described use of unprofessional language. A small number of reviews referenced regulatory complaints or investigations, which suggests families may wish to review the facility’s current compliance status directly.

Dining and activities: The activities program receives frequent positive mention for engagement, holiday events, and efforts by activity staff to include residents. Dining impressions are mixed: some families praised special meals and enjoyable menu items, while others described inconsistent meal quality and dissatisfaction with everyday dining options. Prospective residents might want to observe a mealtime and review menus during a tour.

Facilities and infection control: Many reviewers described the facility as clean and home-like, with helpful reception and welcoming common areas. At the same time, multiple accounts raised sanitation and odor concerns in particular units or floors, and some identified aging equipment or furnishings that may need replacement (for example, older beds and curtains needing attention). A few reviews raised issues about infection-control practices and room cohorting during illness events; these suggest asking about current infection-prevention protocols would be prudent.

Management and communication: Positive management features include an on-site social worker, personalized intake processes, and staff who facilitate family involvement. Operational weaknesses cited by reviewers include scheduling problems, gaps in phone responsiveness, and inconsistent family communication. Several reviews singled out helpful individuals in leadership or therapy; others described situations where families felt their concerns were not acknowledged promptly.

Overall impression and guidance: Maple Ridge appears to offer strong rehabilitative care and an active activities program supported by many compassionate caregivers. At the same time, patterns of inconsistent staffing, variable dining and sanitation experiences, and communication shortfalls introduce variability in the resident experience. Families touring the facility should observe staffing during peak care times, ask about current staffing ratios and use of agency staff, sample a meal, inquire about recent regulatory findings and infection-control policies, and speak directly with the therapy team and nursing leadership to assess continuity of care and communication practices.

About Maple Ridge Rehabilitation and Healthcare Center

Maple Ridge Rehabilitation and Healthcare Center sits in Kingston, PA, and has 92 certified beds with an average of about 86 residents each day, and folks get at least 3.45 hours of nursing care daily, with registered nurses on site around the clock, which means someone's always watching out for residents, and on top of that, they have skilled nursing staff who give at least 0.55 hours of RN care per resident a day and 2.45 hours of certified nursing aide care each day as required by law, though in practice, residents usually get around 3.65 hours of nursing a day. This place is run by Ralley LLC since May 2023 and is affiliated with Century Healthcare, while a nurse turnover rate of 48.4% has been noted lately, and inspectors have found 23 total deficiencies, including some related to resident assessment, care planning, safety, and supervision, with ratings at severity D and E, and the public inspection reports document these details for families who want to see them. Care options here cover both short-term rehabilitation and long-term stays, with physical, occupational, and speech therapy, so someone can come back from the hospital for rehab before going home or just settle in for more ongoing care if that's needed, especially for those who are frail and dependent on regular nursing support. Maple Ridge accepts both Medicare and Medicaid, and offers a variety of healthcare and rehabilitation services, and staff write personalized care plans for each resident, focusing on resident goals and medical needs, and there's meal service, cable, Wi-Fi, laundry, housekeeping, linen services, a dining room, media and activities room, and some pleasant outdoor space, which helps keep life comfortable and gives the place a bit of a community feeling, though they're always working to make improvements, especially after management changes or inspection findings. While Maple Ridge aims to provide compassionate, family-like care with amenities for resident comfort, families will want to look at inspection reports and ask questions about deficiencies and staff turnover before making decisions about care.
