    Excellent care clean rooms staff

I'm very pleased with the very good care my mom received - a private, spotless room, tasty meals, and warm, attentive staff who go above and beyond. The rehab team (Becca and Marcie stood out) helped my dad regain strength, communication was clear and discharge smooth, and I would recommend this welcoming, well-run facility.

    Summary of reviews

This facility elicits a mixed but broadly constructive picture: many families emphasize effective rehabilitation services, respectful bedside care, and an overall clean, welcoming environment, while other accounts describe operational inconsistencies that can affect day-to-day care. The dominant positive theme is a rehabilitation focus — physical, occupational, and speech therapy teams receive repeated praise for helping residents regain mobility and return home. Clinical leadership and members of the therapy staff are frequently cited as knowledgeable and responsive, and several families credited the interdisciplinary team with measurable improvement in their loved ones' function.

Nursing and caregiving staff are often described as compassionate and attentive, and hospice and comfort care received favorable mention when used. At the same time, reviewers describe variability in staff responsiveness and engagement; concerns include delays in routine care tasks, lapses in incontinence-related care processes, and coordination problems that have led to missed clinical appointments for some residents. There are also reports that indicate staffing shortages may contribute to inconsistent delivery of care across shifts.

Dining and activities are generally strengths for many residents: the facility offers group programming, creative activities, and a pleasant dining room environment, and some families praised meal quality and special guest dinners. However, meal satisfaction is not uniform: other accounts describe subpar meals or inconsistent food quality. Prospective residents should evaluate menu variety and special-diet accommodations during a visit.

Facility condition and housekeeping are frequently described as positive, with many reviewers noting an immaculate, well-decorated interior and comfortable private rooms. That said, the pattern of comments shows variability — some reviews identify sanitation and laundry process issues in particular units or shifts. This suggests that cleanliness performance may be uneven rather than uniformly poor or excellent.

Management and communication receive generally positive marks for transparent discharge planning and family updates, and several families felt reassured by administrative responsiveness. Contrasting comments point to instances of disengaged or brusque management and coordination shortfalls; these appear to be intermittent but important to family confidence. Safety-related issues such as falls and transfer practices were raised enough to recommend asking about fall-prevention protocols, staff training on transfers, and supervision levels during care transitions.

In summary, Mountain Top Rehabilitation and Healthcare Center shows clear strengths in rehabilitation programming, compassionate frontline staff, and an overall pleasant environment, which have produced strong recovery outcomes for many residents. At the same time, notable operational concerns — inconsistent staffing, incontinence-care and hygiene-process gaps, variability in cleanliness and dining, and occasional breakdowns in appointment coordination — merit direct inquiry by prospective residents and their families. Recommended steps before placement include observing multiple shifts if possible, asking about staffing ratios and fall-prevention practices, reviewing incontinence and bathing protocols, sampling meals, and confirming how clinical appointments and discharge planning will be coordinated.

Mountain Top Rehabilitation and Healthcare Center is located at 185 S Mountain Blvd, Mountain Top, Pennsylvania, 18707.

About Mountain Top Rehabilitation and Healthcare Center

Mountain Top Rehabilitation and Healthcare Center sits in a spot with 106 certified beds and usually about 98 residents each day, and the folks there get about 3.58 nurse hours per resident per day, though nurse turnover is 48% which is a bit high, but they do have licensed practical nurses and certified nursing assistants around the clock. The center is owned by Century II Pennsylvania Holdco LLC, and is managed by Ralley LLC since May 2023, with ties to Century Healthcare and other indirect owners like Kulanu OC Trust, Shai Berdugo, and Efraim Klein, and being a for-profit corporation, they seem to put an emphasis on keeping the place running efficiently.

Mountain Top offers skilled nursing and a good range of rehabilitation services, so residents see physical, occupational, speech, and respiratory therapies offered right there in the building, and there's support for diagnosis-related conditions like diabetes, wound care, joint replacements, trauma injuries, pneumonia, and general rehab after hospital stays. For people who need help with daily living, staff help with things like bathing, dressing, grooming, eating, and medication monitoring, and there's care for seniors who need diabetic, non-ambulatory, incontinence, or palliative and hospice care. Dementia and Alzheimer's care is also available, along with a specific personal care program in a 34-bed unit offering private and shared rooms.

The building offers semiprivate rooms with things like cable TV and telephones, and some have views of the community, with common areas for dining and recreation, outdoor courtyards, a barbershop and beauty salon, plus a dining room focused on nutritious meals planned and made by chefs. Community activities happen on-site, including devotional activities and social events, plus outings, and there's transportation and parking for those who need it. The staff takes care to develop personalized care plans for everyone, aiming for a family-like atmosphere with warmth and dignity.

Mountain Top supports respite stays for those who need short-term care, and the focus stays on helping residents recover and, if possible, return home. Their multidisciplinary team includes various healthcare professionals who work together on patient-centered care. They offer behavioral management and discharge planning as well. Renovations have taken place recently to make things more comfortable for residents, and the facility tries to maintain a welcoming and compassionate environment with a mix of modern amenities and supportive care.

The latest state inspection from April 18, 2025, found 16 deficiencies, with some related to not sending assessment data to the state on time and not always making complete care plans within 7 days after assessment, so that's something to be aware of when looking at the overall service they provide. Still, the center keeps a focus on giving seniors a safe, home-like, and respectful environment where care is tailored to each person's goals and medical needs.
