    Caring staff and active community

I'm very pleased with the administration and responsive, caring staff - nurses and therapists (Vera outstanding) provide excellent care. The activity program keeps residents social and busy (bingo, Rummy 500, church services, monthly restaurant outings and musical entertainment), there's a snack shop, my sister made friends and plans to stay in touch, staff have been accommodating even at late hours, and upgrades are planned; I would happily recommend this community.

    Summary of reviews

Overview: Reviews present a highly mixed picture of WeCare at Mt. Lebanon. Many families describe positive experiences tied to individual caregivers, therapy staff, and the facility's activities program, while other reviews raise recurring operational concerns that affect daily care and family communication. The contrast suggests variability in performance across shifts, teams, and time periods.

Care quality and clinical concerns: Families praised specific nursing staff and therapists for attentive, effective care and reported meaningful clinical improvements for some residents. At the same time, a number of reviews identify systemic issues: inconsistent delivery of personal care (including missed showers and delayed toileting assistance), medication-administration and documentation gaps, and delays in clinical response during certain shifts. Sanitation and linen management were repeatedly described as uneven, creating ongoing sanitation concerns and periodic odor concerns in some areas. There are also reports indicating limited availability of basic equipment (for example, wheelchairs), which can delay resident care and mobility.

Staffing and management: A major theme is staffing instability. Reviewers describe short staffing, use of temporary and out-of-state agency workers, and perceived low pay contributing to turnover. This staffing variability appears linked to the other operational gaps noted above (delays in response, inconsistent personal care, missed medications). Complaints about communication breakdowns with administration, slow or absent callbacks, inconsistent leadership accountability, and uneven enforcement of policies are recurrent. A subset of reviews includes serious allegations of criminal-level staff conduct; these allegations are distinct from general operational concerns and warrant direct verification with regulators or facility leadership.

Dining, activities, and facility amenities: Activity offerings are a consistent positive: organized games, crafts, music, religious services, and monthly outings are frequently mentioned and described as beneficial to resident engagement. On-site therapy services and a snack shop/game room add to the social environment. However, meal quality and meal-service continuity are described as inconsistent, with some families perceiving insufficient portions or limited menu variety. Facility-level amenities such as private rooms and proximity to the nursing station are noted as advantages, but common-area cleanliness and equipment availability show variability across reviews.

Safety, security, and communication patterns: Concerns about property security (missing personal items, alleged theft) and lapses in privacy/medical-information handling (HIPAA concerns) recur across reviews. Family communication and phone responsiveness are often cited as weak points; several families reported difficulty reaching nursing staff or receiving timely updates. Visitation and access policies during infection-control events were described as restrictive by some families and inconsistently applied by others. A few reviews report that leadership or staff responded well when specific incidents were escalated, while other accounts describe delayed reporting to authorities.

Notable patterns and advice for families: The facility appears capable of providing very good care under certain staff and leadership conditions, but performance is inconsistent. When evaluating this facility, prospective residents and families should verify current staffing ratios, ask about agency staffing use and turnover, review infection-control and sanitation protocols, inquire about medication-administration safeguards and incident-reporting procedures, and confirm equipment availability (mobility aids, shower chairs). Requesting references from current families, inspecting linens and common areas during a visit, and asking for recent survey or inspection records can help clarify whether the concerns raised in some reviews reflect lingering systemic problems or issues that have been addressed under new management.

    About WeCare at Mt. Lebanon

WeCare at Mt. Lebanon sits as a large care center with 350 certified beds and serves an average of 84 residents each day, and over the past year, it's changed hands in ownership and currently links up with Wecare Centers and also goes by WeCare at Mt. Lebanon Rehabilitation and Nrsng Ctr. The facility is a for-profit corporation run by Wecare Hcc LLC, with Gedaliah Wielgus, Jarod Stragand, and Carolyn Hall having started management in August 2024. It's got a Skilled Nursing Facility and offers short-term rehabilitation programs for people who need rehab between hospital and home, with services like physical, occupational, and speech therapy, dialysis care, pain management, and personal care home options. WeCare has a memory care unit for any stage of Alzheimer's, assisted living, senior living, and adult day center services, and it also has outpatient rehab and a transitional rehabilitation unit for around-the-clock care after hospital stays. There's a gym for residents to keep active, a beautician on-site for grooming and social visits, and a store for picking up basic things, while the staff run programs and activities designed to help keep both mind and body active. However, the facility has a high nurse turnover rate at 91.4%-much more than the state average-and staffing sits lower with 3.17 nurse hours per resident each day, which is below the state's average. There's a history of 77 total deficiencies in inspection reports, including problems with infection control (6 of those), pest control failures, timing of meals, alternatives to meals and snacks, and dietary accommodations, especially regarding allergies and personal preferences for residents. Despite efforts from dedicated teams and a family-like setting, these care and compliance challenges have been noted, so families may want to weigh the environment, staffing, and care when considering WeCare at Mt. Lebanon.
