    Compassionate staff saved my mom

I'm extremely grateful for the kind, knowledgeable staff - nurses and aides were attentive to medical needs, gave clear explanations, arranged transportation, and even provided effective PT and engaging activities. The facility is clean, residents seem happy, staff remembered my late wife and literally saved my mom's life; I highly recommend it, with only minor quibbles.

    Summary of reviews

Reviewer feedback for Ridgeview Healthcare & Rehabilitation Center is strongly polarized. A substantial portion of comments highlight positive direct-care interactions: many families and residents praised nurses, aides, and therapy staff for kindness, skill, and attentiveness. Several accounts specifically commend physical/occupational therapy outcomes, staff who provided clear explanations, transportation assistance to appointments, and occasions of high-quality meals and room-delivered dining. These reports suggest the facility can and does deliver effective rehabilitative services and individualized attention for some residents.

Contrasting these positive experiences are recurring operational concerns that affect perceived overall quality of care. Reviewers describe inconsistent clinical oversight and care coordination, including delays in medication administration, missed or poorly executed clinical orders, and gaps in wound and skin management. Several comments describe delayed responses to basic personal-care needs and long waits for assistance. These patterns point to systemic issues in clinical monitoring and responsiveness rather than isolated interpersonal problems.

Staffing and workforce stability emerge as a central theme. Multiple reviewers cite staffing shortages, frequent use of agency personnel, long shifts, and high turnover; these factors are linked in the reviews to inconsistent staff training, variable communication, and uneven continuity of care. While many individual staff members receive accolades for compassion and competence, the overall staffing pattern appears to produce variability in resident experience from shift to shift.

Facility-level concerns include sanitation and incontinence-care issues in some units, inconsistent housekeeping practices, and reports of security and property-control weaknesses. Families also raised serious process concerns around discharge coordination, complaint handling, and clinical-incident response; a limited number of reviewers referenced a resident death and subsequent coroner involvement, which was accompanied by complaints about communication and follow-up. There are also allegations of property loss and financial-account concerns that prospective families should note and, if applicable, pursue through appropriate channels.

Dining and activities produce mixed impressions: the activities program and certain meals receive positive comments, but reviewers also describe variability in meal quality and delivery consistency. Therapeutic services are often cited as a strength, with several families crediting therapy staff for measurable improvements.

Management and communication are recurring focal areas for improvement. Reviewers report difficulty reaching management, unhelpful or indifferent front-desk interactions, and perceived shortcomings in complaint resolution. Positive reports of managers or clinicians providing clear explanations indicate that responsive leadership exists in some instances, but consistent responsiveness appears to be uneven.

For prospective residents and families: an on-site visit is recommended with targeted questions about staffing ratios by shift, wound-care and medication-administration protocols, incident and discharge procedures, property-security measures, and cleaning schedules. Ask to meet therapy staff and observe an activity session and a meal service. Reviewing recent inspection reports and asking management for documentation of staffing levels and incident-response policies will help clarify whether the facility’s strengths align with your needs and whether the operational concerns reported by families have been addressed.

About Ridgeview Healthcare & Rehabilitation Center

Ridgeview Healthcare & Rehabilitation Center sits at 200 Pennsylvania Avenue in Shenandoah, PA, and runs a skilled nursing facility that takes care of about 90 residents each day and has 111 certified beds available, offering both short-term and long-term care, as well as respite stays and comprehensive clinical services for those coming out of the hospital or needing specialized therapies like physical, occupational, or speech therapy, and memory care for people with Alzheimer's. Residents can get adaptive recovery services, palliative and terminal care, IV therapy, wound care including Wound Vac's, and things like orthopedic and pulmonary rehabilitation, all from a team including registered nurses, licensed practical nurses, certified nursing assistants, physical and occupational therapists, wound care nurse and physician, dietician, clinical social worker, and psychology and psychiatry consultants, all working together on individualized care plans. The place handles assistance with medication, ongoing health checks, and manages chronic health problems, but it has had some issues, too, like being a Special Focus Facility due to a history of quality problems, and has a total of 104 documented deficiencies in inspection reports, with specific problems related to infection control, failure to follow treatment according to orders and resident wishes, and administration not always meeting licensing standards, and it had a complaint in March 2025 that led to 7 new deficiencies, including one for infection control, which adds up to 4 infection-related deficiencies overall. The nurse turnover rate is high at 59.4%, above the state average, but total nurse staffing comes out to about 3.85 hours per resident per day. Rooms are private or semi-private with modern furniture and appliances, and residents can use the dining room, therapy suite, spa, chapel, beauty and barbershop, game pool, and various activity spaces, plus daily housekeeping and laundry services. The schedule includes a mix of social activities, games, wellness programs, religious services twice weekly led by local clergy, and group events to keep a sense of community and friendships going. The system uses caregiver tools like the My Account Dashboard for messages, care coordination, visitor access, and emergency info, and connects residents and families to a range of support resources for behavioral health, opioid info, suicide prevention, disaster mental health, and community-based clinical trials. Ridgeview also supports people with mental and behavioral health needs, provides access to home health, supports both HMO and PPO insurance contracts, accepts Medicare, Medicaid, Medicaid Pending, and Self-Pay, and stays active in programs offered through ACA and HRSA-supported health centers, plus state and federal funding. There's help for everyday needs, rehabilitation after illness or surgery, and extra support for those with long-term medical or memory needs, and the facility tries to create a warm, healing place where independence is encouraged, but families should be aware of the documented quality issues and look at inspection reports before deciding.
