    Clean, welcoming home; excellent care

I placed my mother here and have been very pleased. The facility is clean, welcoming and feels like a home away from home; staff are consistently friendly, caring and communicative. Meals are tasty and dietician-supervised, therapy and rehab are excellent, meds and updates are prompt, and there are plenty of activities (bingo, Bible study, intergenerational and family events). Overall very good with just a little room for improvement - I'd recommend it.

    Summary of reviews

Titusville Nursing and Rehabilitation elicits a mix of strong praise and substantive operational concerns. Many families and residents describe clinical strengths: a core of compassionate nursing staff, timely condition updates, and a therapy department that produces measurable progress โ€” including weekend therapy availability. Frontโ€‘line staff are frequently described as polite, helpful, and able to create a welcoming, homeโ€‘like environment. Common areas and many rooms are characterized as clean and calming, and social programming โ€” religious services, bingo, intergenerational activities, and family events โ€” is a consistent positive for resident engagement.

Clinical care shows important strengths alongside variability. Physical and occupational therapy receive repeated commendation for effectiveness and individualized progress. Nursing care is often described as attentive, with prompt responses and additional care when needs arise. However, reviewers also describe unevenness in staff attentiveness and supervision that manifests as delayed responses during certain shifts, inconsistent bathing and personalโ€‘care schedules, and occasional gaps in medication administration and clinical followโ€‘up. These operational inconsistencies suggest staffing stability and shift coverage are important areas for verification by prospective families.

Dining and activities present a mixed picture. Several comments praise homemade food, dietitian supervision, and satisfactory meals with organized social dining and special events. At the same time there are recurring notes of inconsistent food quality and menu variability across stays. Activity programming is generally robust โ€” with regular spiritual offerings, group entertainment, and community events โ€” but availability can vary by unit or shift, leading some families to perceive limited recreation for particular residents.

Facility infrastructure and management followโ€‘through are additional areas of concern. Multiple reviewers indicate the building would benefit from updating and remodeling, and mention specific maintenance issues such as heating problems and limited maintenance staffing that delay repairs. There are also serious governance and safety themes raised: allegations of staff theft, concerns about incident investigation and administrative followโ€‘up, and sanitation or incontinenceโ€‘care concerns that point to lapses in oversight. These matters, while not universal across accounts, are significant and warrant direct discussion with facility leadership and review of recent inspection or investigation records.

Overall, Titusville appears to offer meaningful rehabilitative resources, caring staff, and an active social program for many residents, but prospective residents and families should balance these strengths against reports of inconsistent staffing, variable personalโ€‘care practices, infrastructure needs, and governance concerns. Recommended next steps for families: schedule a tour across different days/shifts, meet therapy and nursing managers, review staffing ratios and incidentโ€‘management policies, and ask about recent corrective actions for maintenance and safety issues to form a comprehensive view of current operations.

    About Titusville Nursing and Rehabilitation

Titusville Healthcare and Rehabilitation Center sits in Titusville, Pennsylvania, surrounded by green spaces that help keep things calm and peaceful, and the place does have a cozy, home-like atmosphere with updated facilities and modern amenities, including studio and semi-private or companion room layouts so folks can feel comfortable and valued. The center offers both short-term rehabilitation and long-term skilled nursing services, with 24-hour care always available every day of the week from a team of compassionate professionals who focus on comfort, dignity, and well-being for everyone living there, and the staff works closely with residents and their families to set up very personalized care plans for each person's medical and emotional needs, which can mean memory care, skilled nursing, memory support, palliative care, restorative care, respite care, and therapy services such as physical, occupational, and speech therapy whenever that's what's needed. The nursing home received a 5-star rating from the Centers for Medicare & Medicaid Services, and it serves as a healthcare cornerstone for Titusville and the surrounding community, operating as a non-profit and being affiliated with Valley West Health LLC since November 2024, offering emergency, inpatient, surgical, radiology, rehabilitation, laboratory, outpatient services, and wellness programs, along with outdoor areas and community programs to help physical and mental well-being, and they've developed protocols to provide safe care, especially for respiratory needs. The center keeps a nurse staffing level of about 3.56 nurse hours per resident per day, with an average of 56 residents in a building certified for 77 beds, and staff try to give support in a way that's tailored to every individual, with an interdisciplinary team who works together on decisions for treatment, but there have been inspection reports with deficiencies, the most recent one from July 2024 listing five areas, including one related to infection control and others tied to treatment following care plans or resident preferences, with a history of nine total deficiencies, yet they continue to place importance on improving care and following orders set out in those plans. Amenities and features in the center include serene outdoor spaces, community rooms, modern wellness facilities, and programs aimed at keeping folks engaged and comfortable, and the entire place prides itself on being a supportive and diverse community where everyone can feel included, respected, and able to thrive, focusing every day on doing their best for the people they care for.
