    Caring competent staff excellent meals

I'm very pleased with my stay at Tremont - the caring, competent staff and on-site doctor helped me recover after surgery, got me to appointments, and communicated clearly throughout. The facility is clean, meals are good, activities and social opportunities kept me engaged, and admissions/administration were helpful and personable, so I'd happily recommend them.

    Summary of reviews

Reviews of Tremont Health & Rehabilitation Center present a mixed but patternable picture. Many accounts praise individual staff members, the admissions process, and the activities program; others describe significant operational gaps related to clinical monitoring, timeliness of care, and communication. This creates a split impression in which the facility’s strengths are delivered unevenly across residents and shifts.

Care quality shows both strengths and vulnerabilities. Several narratives highlight compassionate, personalized nursing and positive rehabilitation outcomes after surgery, with staff who take extra steps to support residents and families. At the same time, reviewers raised substantive concerns about clinical monitoring and follow-up—examples include limited access to fluids, delays in assistance, inconsistent delivery of meals and medications, and questions about medication management. Leadership has acknowledged staffing constraints in some cases, and reviewers noted limited therapy resources and mobility support for residents who require active rehabilitation.

Staff and management receive frequent praise for individual professionalism and helpfulness: admissions staff, administrative personnel, front-desk workers, dietary and maintenance teams were cited for being welcoming and responsive, and the facility is noted for assisting families with Medicare/Medicaid matters. However, there are recurring comments about uneven communication with families and occasional lapses in follow-up after incidents. Some reviewers described unprofessional conduct by specific staff members and raised concerns about outreach and privacy practices; these items suggest variability in staff training and supervisory oversight rather than a uniformly consistent culture.

Dining and activities are clear positive areas for many residents. Food quality was described as better-than-average by several reviewers, and the activities program—bingo, karaoke, and social events—was frequently cited as engaging and beneficial for resident socialization. That said, intermittent reports of missed meals and meal-delivery problems point to operational reliability issues that prospective families should query directly.

The physical environment is generally described as clean and well-maintained in common areas, with pleasant decor. The building itself is older and has not seen extensive renovations, which may influence room accommodations and amenities. Operationally, staffing shortages were explicitly referenced by facility leadership in at least one report and appear to underlie several of the operational weaknesses reviewers described.

Notable patterns: praise tends to concentrate around individual caregivers, admissions and activity staff, and successful rehab outcomes, while concerns cluster around inconsistent clinical oversight, timeliness of assistance, communication and incident follow-up, and an aging facility. For prospective residents and families, recommended inquiry areas include staffing ratios and shift coverage, hydration and incontinence-care protocols, medication administration and oversight procedures, therapy availability, and the facility’s policies on family communication and privacy. A tour and direct conversations with clinical leadership can help assess whether the facility’s praised strengths are reliably available for a given resident.

About Tremont Health & Rehabilitation Center

Tremont Health & Rehabilitation Center sits in Tremont, PA and opened in 2018, serving Schuylkill, Lebanon, and Dauphin Counties for over 35 years. The community has 180 certified beds, but by June 2025 only 25 are currently certified, and it usually has about 154 residents each day. This for-profit Skilled Nursing Facility is owned by Saber Governance, LLC, with Stephen Maganzini in management since 2021, and links to Saber Healthcare Group and Shg Rewd LLC. Inspections have found a total of 13 deficiencies, with recent complaint reports in September 2024 noting three more deficiencies, including ones for not always providing care according to doctor's orders and resident preferences, and not completing resident assessments on time, with a severity level marked as D. The nurse turnover rate is 54.7%, which is higher than the state average, and the nurse staffing hours per resident per day are below average at 3.19.

The center offers skilled nursing care, rehabilitation, long-term care, assisted living, and memory care programs. It provides services for people needing help with bathing, dressing, transferring, medication management, non-ambulatory care, and diabetic and incontinence care. The facility accepts both Medicare and Medicaid and sits inside a Continuing Care Retirement Community. There's round-the-clock call systems and supervision with 12 to 16 hours of direct nursing care available, and certified staff like Registered and Licensed Practical Nurses, Certified Nursing Assistants, and Therapists are on hand. Residents can receive post-acute care, post-surgical recovery assistance, physical, occupational, and speech therapy, and specialized support for stroke or cardiac recovery, IV therapy, and wound care. The interdisciplinary team works with doctors and families to make individualized plans.

Amenities include private or shared rooms with bathrooms, cable TV, kitchenettes, phones, air conditioning, and Wi-Fi, and there are game rooms, an arts room, a library, business facilities, activity and fitness spaces, wellness and spa areas, a garden, walking paths, and scheduled community activities, events, and outings like Club, Wii Night or Alzheimer's Fun Day. Meals are served in a restaurant-style dining room by a chef with allergy-sensitive, diabetic, and all-day dining options. Housekeeping, laundry, and move-in help are available. The caregiving staff speaks English, and the team helps residents with everyday needs and mobility, including walkers or wheelchairs or being bedridden. The Memory Care Neighborhood aims to help ease anxiety for residents with dementia using familiar activities. Social services, family support, and transportation add to the overall support.

The facility is close to a doctor's office, pharmacy, churches, dining, recreational trails, and parks, and it's in a small town with a population around 2,400, a high median income, and a low poverty rate. The environment is meant to feel warm and friendly while covering many levels of care from short-term rehab to more advanced or long-term needs. Potential residents and families are encouraged to tour the center, meet the staff, see the services firsthand, and get to know the routines and equipment used in care and recovery. Meals, activities, and different room options try to create a comfortable daily life, but as with any facility, it's important to look at inspection reports and ask questions so you know what to expect.
