    Attentive, knowledgeable staff; excellent care

I'm very pleased with the attentive, professional, and friendly staff-knowledgeable, long-tenured leaders (like Amber and Dawn) who provided personalized, family-like care. My loved one did exceptionally well with rehab and meds, the building is clean, the food is good, and staff welcomed our family and kept us fully informed. Overall a wonderful, nurturing facility that eased my worries; I would gladly recommend and return.

    Summary of reviews

Overall impression: Feedback about Country Meadows Nursing & Rehabilitation Center is strongly polarized. A substantial body of comments highlights compassionate, stable staff and positive clinical outcomes—especially in rehabilitation—while another set of comments raises operational and safety concerns that families should evaluate closely. The facility elicits both high praise for individualized, family-like care and serious questions about consistency in basic processes.

Care and clinical quality: Many families praised direct-care staff as attentive, nurturing, and consistent, noting long-tenured personnel and positive therapy outcomes for procedures such as joint replacements. At the same time, reports indicate variability in clinical processes: extended nurse response times, inconsistent speech-therapy effectiveness, and gaps in medication handling and hygiene controls. These contrasts suggest that while clinical strengths exist, execution and reliability of basic clinical tasks vary across shifts or units.

Staffing and staff conduct: Reviewers repeatedly mention dedicated, experienced caregivers and specific staff members who provide strong personal attention. However, there are recurring concerns about understaffing and coverage that appear to affect response times and supervision. Separate, serious claims include privacy-protocol lapses and an allegation involving involuntary restriction; those items represent high-severity issues that warrant direct inquiry with management and regulators when being considered.

Dining and dietary services: Evaluations of food are mixed. Several families described good meals and responsive dietary staff, whereas others described inconsistent food quality, meals served at unfavorable temperatures, and insufficient accommodation of special diets. Prospective families should confirm current menu practices, meal-service timing, and the facility’s process for implementing dietary restrictions.

Activities and engagement: Positive comments portray a welcoming, home-like social environment with staff who foster resident interaction. Conversely, some reviewers characterized programming as limited in variety (for example, a reliance on simple group activities). The pattern suggests activity offerings may be adequate for some residents but lack breadth for others; ask for a current activity calendar and how programming is individualized.

Facilities and maintenance: Many reviewers describe a clean, bright facility with active housekeeping on the units. Yet, there are isolated but significant maintenance and environmental concerns—odor issues in some common areas, difficulties with call-light reachability, and equipment problems affecting assistive devices. These items indicate uneven facility maintenance and environmental controls.

Management and communication: Leadership received praise from several families for effective, personable management and strong departmental leaders. At the same time, other comments point to inconsistent communication, privacy-protocol lapses, and unresolved ethical or safety concerns. This mix indicates variability in administrative follow-through and complaint resolution processes.

Notable patterns and considerations: The most consistent strengths are long-tenured caregivers, strong interpersonal rapport with residents, and positive rehabilitation outcomes for many. The most important areas for further inquiry are staffing consistency, medication- and hygiene-control procedures, response times, dietary implementation, activity variety, and any formal documentation related to serious ethical or rights-related allegations. Prospective residents and families should tour the facility, request current staffing ratios, review incident/complaint resolution procedures, and observe mealtimes and activity sessions to assess whether the facility’s strengths align with their expectations and care needs.

About Country Meadows Nursing & Rehabilitation Center

Country Meadows Nursing & Rehabilitation Center sits at 3301 W Park Row Blvd in Corsicana, TX, and holds 96 certified beds, though most days about 43 residents live there, so folks usually have some space, and if you need assistance with daily life, they do have nurses and help available, though nurse staffing runs about 3.36 hours per resident per day and nurse turnover is on the high side at 50%, which means there might be new faces fairly often. Since June 2019, Regency Integrated Health Services LLC has managed the place, and the facility runs as a for-profit limited liability company, with Capital Funding Group, Inc. as the direct owner and John Dwyer as an indirect owner, and the staff mainly speak English, but there might be others who speak different languages.

The center focuses on skilled nursing and rehabilitation services, with levels for assisted living, memory care for people with dementia or Alzheimer's, and skilled nursing home care, and if someone needs help with bathing, dressing, or getting around, personal care assistants help with that, and they also offer medical support like wound care, medication support, podiatry, and transportation services. There's physical, occupational, and speech therapy available, along with therapy for those recovering from surgery or joint replacements, and for those needing long-term skilled nursing, the center covers conditions like cardiac care, diabetes, IV and tube feeding, pulmonary care, and respiratory therapy, all under a system shaped by federal standards meant to protect from infections.

Amenities include a fitness center, salon/barbershop, therapy gym with a kitchen for practice, washers and dryers, private and semi-private rooms, cable TV, WiFi, an activities room, guest parking, and options for meals in a dining room, plus social activities, art, crafts, and health or wellness programs, and memory care residents have special activities and safety features built for them. Housekeeping and safety features like handicap access and a sprinkler system are in place, and the center pays attention to supporting both residents and their families, including hospice care when comfort is the focus near the end of life.

Inspection reports show the center's had some trouble, with deficiencies noted related to infection control, food service practices, doctor's orders at admissions, and sometimes in helping residents who can't manage daily life alone, and the records show two infection-related deficiencies, so ongoing attention is needed in those areas, but the staff are said to be compassionate, with care plans adjusted to each person, and management's got experience in community service over years. Though they've received some recognition and accreditations for care, Country Meadows doesn't currently accept new patients, and their office hours aren't public, but residents wanting to recover strength, receive skilled nursing, or get help with daily living needs do find a range of support, and if someone wants simple comforts like cable television, internet, or just a place to sit with others, those are available too, and you can look up their profile online if needed.
