    Compassionate, clean care with dignity

I'm very pleased with Pebble Creek - the nurses, CNAs and therapy team were compassionate, skilled and responsive, and admissions (especially Lourdes) made our transfer smooth. The facility is clean, well-maintained and welcoming; rooms are comfortable with daily cleaning, good meals and engaging activities. Staff kept our family informed and supported us through recovery and hospice with dignity. I felt confident leaving my loved one in their care and would recommend them.

    Summary of reviews

Reviews for Pebble Creek Nursing Center are strongly divided, producing a mix of high praise for frontline staff and therapy services alongside persistent operational concerns. Many families describe compassionate, attentive caregivers, effective rehabilitation programs, and welcoming, well‑kept facilities. At the same time, a consistent set of criticisms centers on staffing patterns, communication failures, and leadership responsiveness.

Care quality shows dual themes. Positive feedback highlights skilled CNAs, nurses, and therapists who support recovery, provide 24‑hour observation, and create individualized care plans; several reviewers named specific staff members and liaisons who facilitated smooth hospital discharges and ongoing family contact. Conversely, other accounts point to inconsistent responsiveness to call requests, variable bathing and personal‑care schedules, and instances where clinical incidents were not clearly explained to families. These patterns suggest that resident experience may be highly dependent on staffing levels and which direct‑care staff are on shift.

Staff and workplace culture are a prominent dividing line. Numerous notes of gratitude emphasize friendly, professional, and proactive employees, effective admissions coordination, and staff who go above and beyond. However, there are also comments indicating low morale, concerns about leadership and HR responsiveness, and problematic conduct or communication tone by some personnel and contractors. The contrast implies that individual staff members and teams are strengths of the facility, while systemic workforce and management issues may undermine consistency.

Dining, activities, and facilities generally receive favorable comments: reviewers mention varied meal options, accommodating kitchen staff, lively recreation programs (including culturally relevant activities), and clean, attractive common areas and grounds. That said, there are isolated sanitation and localized odor concerns—including specific comments about a therapy space—and some mentions of inconsistent meal quality. Prospective families may want to evaluate dining menus and observe activity sessions during a tour.

Management and operations are the areas most frequently identified as needing improvement. Concerns include chronic staffing shortages, inconsistent weekend leadership coverage, gaps in incident notification and follow‑up with families, and lapses in handling patient information and identification. There are also notes about unclear ancillary fees and occasional contractor management problems. These operational traits point to risks in consistency of care and communication rather than a uniform absence of competent staff.

Overall, Pebble Creek appears to offer strong individual caregivers, effective therapy programs, and a welcoming environment for many residents. At the same time, variability in staffing, leadership responsiveness, communication around clinical incidents, and localized cleanliness issues are recurring themes. Families considering the facility should schedule a tour that includes observation of care routines, questions about staffing ratios and weekend coverage, clarification of additional service charges, and review of incident‑notification and privacy policies to assess how the facility addresses the operational concerns identified in reviews.

    About Pebble Creek Nursing Center

Pebble Creek Nursing Center sits over on Scott Simpson Drive in El Paso, and you'll find a range of care options under one roof, which means they take in folks who need both short-term rehab and longer stays, and they've got respite care for families needing a break, plus they help folks coming home from the hospital, too, with programs meant to help each person regain strength at their own pace. The staff here help with daily things like bathing, getting dressed, and using the bathroom, and they're around day and night, since they offer 24/7 skilled nursing care, and they're used to helping people who have medical or memory problems, including those with Alzheimer's or dementia, since the facility tries to keep wandering and confusion down by having special supports in place for memory care. People who live at Pebble Creek get their laundry washed, their rooms cleaned, and their meals prepared for them, with the kitchen serving up regional favorites alongside other nutritious options, and the team there carefully manages medication, too, so nothing gets missed. The place has wound care, ventilator care, in-house dialysis, diabetes help, and therapy services like physical, occupational, and speech therapy, plus, if someone's coming back from surgery or illness like a stroke or heart trouble, the staff create an individual plan to support recovery, making sure no one's needs get overlooked. Veterans services are offered here, along with hospice care, and Pebble Creek's part of the Creative Solutions in Healthcare network, which means the care team sticks to consistent standards for each resident, making adjustments as health needs change. The building itself feels more like a home than a hospital, and everyone tries to treat residents with respect and dignity, and the atmosphere gets credit for being friendly, since a lot of regulars say the staff act kind and helpful, so the place doesn't feel cold or distant. Beyond medical needs, Pebble Creek tries to engage people with activities, spiritual services, beauty and barber offerings, and things like optometry, podiatry, and dentistry visits on-site, while transportation gets provided for appointments or outings as needed. Family members can check in on loved ones' wellness and progress using an online portal, and there's assistance for concerns or feedback, since the center makes it a point to keep communication open, even offering a care line and ombudsman services. People with religious interests can take part in services, and social events keep minds and bodies active, so no one feels too alone, even on quiet days. This center admits residents who come from home settings or hospitals, assesses them with the help of a skilled nurse team, and then puts together a care plan that will fit their situation, and whether it's skilled therapy, restorative programs, memory support, or just friendly daily help that someone needs, this place aims to meet them where they're at and see them through, so residents keep some independence, eat well, and feel cared for each day.
