    Beautiful rehab facility, caring staff

I had a rehab stay and visits here and I'm very pleased - the building is beautiful, newly remodeled memory care, spotless and well-maintained with a lovely courtyard and inviting dining and activity rooms. The nurses, aides, therapy team and admin are caring, responsive and professional, and staff make family visits and activities feel warm and welcoming. Overall I'm happy with the care and would recommend this facility for loved ones.

    Summary of reviews

Overall impression: Reviews convey a strongly positive view of Fairfield Nursing and Rehabilitation Center, with repeated emphasis on cleanliness, facility upkeep, and a pleasant physical environment. Families and visitors describe the building and grounds as well maintained, with an attractive dining room and an inviting courtyard that support residents' quality of life. The facility's newer and remodeled areas, including a memory-care unit and private rooms, are highlighted as contributing to a comfortable, homelike atmosphere.

Care and staff: Staff are consistently characterized as compassionate and engaged. Nursing and aide teams receive specific praise for attentiveness and kindness, with night-shift nursing described as particularly supportive. Rehabilitation and physical-therapy services are noted as effective for short-term stays, and reviewers point to responsive maintenance and a helpful front office as part of a coordinated care experience. There is evidence that individual staff members and interactions can positively influence family confidence in care.

Dining and activities: Dining is presented as a strength—reviewers cite a pleasant dining room and positive comments about the food. The activity program is highlighted for its variety and resident engagement: an active craft room, organized entertainment, water-based activities, and informal features (such as lobby birds) contribute to a lively social environment. Overall, programming appears oriented toward improving residents' day-to-day quality of life.

Facilities and amenities: The facility's aesthetics and cleanliness are recurring themes. Renovated spaces, private rooms, and outdoor amenities like the courtyard are described as well cared for and inviting. Attention to maintenance and decoration supports the impression of a professional, comfortable residence for both short-term rehab and longer stays.

Management and operations: Management responsiveness is a notable pattern—when issues arise they are described as being addressed by leadership. Several comments point to an approachable administration and a manager who is easy to work with. At the same time, the reviews imply occasional operational variability: there are references to staff-related issues that required managerial intervention, suggesting moments of inconsistency in staff performance or conduct.

Notable patterns and considerations: The dominant pattern is positive—strong sanitation and upkeep, engaged staff, effective short-term rehabilitation, and a robust activities program. Prospective residents and families should note a couple of operational caveats to confirm during a visit: inquire about staffing consistency across shifts, processes for addressing staff-performance concerns, and how the facility tracks and communicates long-term clinical outcomes beyond short-term rehab. These targeted questions will help determine whether the facility’s generally favorable attributes align with specific medical and long-term-care needs.

About Fairfield Nursing and Rehabilitation Center

Fairfield Nursing and Rehabilitation Center, sitting in Fairfield, TX, holds 101 certified beds and tends to seniors needing medical and personal care in a setting that feels deliberate in its home-like design, and you'll see they focus on both long-term care and short-term rehab, which means folks get things like assisted living, memory care, vent care, and in-house dialysis, along with paths for rehab-to-home programs and respite care for folks who just need a place for a little while. The center offers physical, occupational, and speech therapy, while their skilled care team also handles nursing restorative programs, stroke recovery, diabetes management, cardiac illnesses, and wound care-so they're set up for people dealing with many health conditions, and yes, you'll notice they even provide support for folks after surgery and those who need hospice care. There's a strong focus on comfort too: residents get laundry, housekeeping, beauty and barber services, and nutritional support, plus there's transportation for trips and religious services for those who need them, and people can go to the podiatrist, dentist, or optometrist right in the facility along with having medication managed by staff. The staff puts an effort into daily life, with engaging activities, a structured environment for memory care, and areas dedicated for different care needs, along with routines for meals, outings, and support services that help improve quality of life. Rehabilitation is a focus, and they run programs and therapies for folks recovering from illness or injury, guided by a care team that tries to keep improving how they do things, though the nurse turnover rate is 60.6%, which tells you there's been a fair amount of staff changes. Facility management changed hands to Fairfield I Enterprises, LLC in May 2022, with day-to-day running handled by Purple Door, LLC, while ownership includes Liberty County Hospital District No 1, and the place itself is tied to Fundamental Healthcare and part of the Creative Solutions in Healthcare network. State inspections show two infection-related deficiencies recently and one related to timely reporting of suspected abuse, neglect, or theft-inspection reports are available and do point out infection control concerns under F0880 and F0609, though the staff do have infection control measures in place, just not without issues. Residents and families can use a contact form or a care line to get help or share worries, and the website's got tools that make it easier to read, like changing the text size, which helps folks with vision trouble. With a rating of 5.0 from 25 reviews, residents seem to find something good here, though as always, it's best to review the reports and get a full picture, especially if infection control or staff consistency is a concern for you.
