    Caring accommodating staff clean rooms

I'm very pleased with the caring, accommodating staff and responsive management - special thanks to Debbie, Jackie, Phillip, and Lois. My room and bathroom are kept clean, laundry is handled, and staff assist with bathroom/diaper needs and showers three times a week. Meals are hospital-style but the tray was good and substitutions were accommodated. Residents and staff are positive and friendly; I feel well cared for.

    Summary of reviews

Overall impression: Reviews for Hereford Nursing & Rehab are mixed, with a clear split between families who describe attentive, accommodating care and others who describe operational gaps that affected a resident's experience. Several reviewers praised individual caregivers and management, while other accounts raised substantive concerns about care consistency, administrative practices, and responsiveness.

Care quality: Positive comments highlight helpful bathing assistance (regular showers noted), assistance with toileting/incontinence needs, and staff who respond quickly to questions. However, other reviewers described inconsistent personal-hygiene care and declines in mobility or skin integrity, which suggest gaps in ongoing clinical monitoring and transfer-safety practices. There are also concerns about delayed attention in urgent situations; prospective families should ask about skin-check protocols, repositioning schedules, and the facility's process for escalating clinical changes.

Staff and responsiveness: Many reviewers named specific staff members and characterized frontline caregivers as friendly and receptive. At the same time, other feedback indicates variable staff professionalism and responsiveness, including staff being occupied with personal devices or slow to attend to requests. This mixed pattern points to uneven performance across shifts or units rather than a uniformly poor or uniformly excellent staffing profile.

Dining and nutrition: Dining impressions range from positive—hospital-style meals, acceptable tray service, and willingness to accommodate substitutions—to negative characterizations of poor-quality food. That variability suggests meal quality and satisfaction may depend on specific meal times or individual expectations. Families may want to sample meals and ask how dietary preferences and substitutions are handled.

Activities and social environment: Review content included references to a positive resident atmosphere and a generally caring culture. Few reviews provided detailed information about structured activities or engagement programs, so activity offerings should be confirmed during a visit.

Facilities and housekeeping: Clean rooms and bathrooms and an on-site laundry service were noted as strengths. At the same time, some reviewers raised odor and incontinence-care concerns tied to inconsistent hygiene attention; this indicates that while physical facilities and laundry operations can be adequate, operational follow-through on personal care can vary.

Management and administrative issues: Management received praise in several accounts for prioritizing resident care; however, there are serious administrative concerns raised elsewhere, including billing disputes, abrupt Medicare-related discharges, and allegations of inappropriate administrative conduct. These are significant enough to warrant verification — ask for written billing policies, Medicare-transition procedures, and examples of recent abridged discharges. If applicable, request copies of the most recent state inspection and any corrective-action plans.

Notable patterns and recommendations: The overall pattern is one of polarized experiences — some families report compassionate, responsive care while others report operational inconsistencies that directly affected resident comfort and safety. Prospective residents and their families should tour the facility, meet direct-care staff, observe a shift change if possible, request meal samples, and ask specific questions about bathing schedules, incontinence protocols, skin checks, transfer safety, staffing ratios, emergency-response procedures, and billing/discharge policies. Reviewing state inspection reports and speaking with current resident families can help clarify whether the facility's strengths align with your priorities.

    About Hereford Nursing & Rehab

Hereford Nursing & Rehab sits as a for-profit, individually owned skilled nursing facility that's been managed by Llano Estacado Healthcare LLC since 2014, owned by Aaron Terrel and Audra Terrel, offering care mostly for older adults who need skilled nursing or rehabilitation after hospital stays, illness, or surgery, and what stands out is how they have 120 beds though they only keep about 47 residents in their daily census, so there tends to be some space, and they have a nursing home section with a 5-star rating from the Centers for Medicare & Medicaid Services, plus good grades in their inspections, like an A+ for their facility inspection grade and an overall B+ for quality, though you'll see 13 deficiencies listed in their records, some tied to things like infection prevention, food storage, and accommodating residents' needs, but inspectors found only one deficiency considered a risk for patient safety and no confirmed complaints in the last year, which isn't always the case at other places.

You'll find a trained staff team here, such as registered nurses, LPNs, CNAs, and a medical director, and their nurse turnover rate sits at 42.6%, with staffing levels around 3.84 nurse hours per resident each day, which gives a sense of staff presence in the building, and the facility's staffed for 12 to 16 hours of nursing care per day plus 24-hour call system and supervision. Nursing staff and aides help with everyday things like bathing, dressing, transferring, and medication help, and they also provide more specialty healthcare like pain management, wound care, physical therapy, speech therapy, nutritional counseling, recreational therapy, and memory care, including Alzheimer's care. The therapy department offers occupational and physical therapy along with aides, and residents have access to short-term rehabilitation between hospital and home, so folks can regain strength before heading home. Hereford Nursing & Rehab also supports daily living with meals, laundry, personal care, and homemaking, accepting both Medicare and Medicaid for payment, and even takes care of transportation and parking needs.

Residents can choose between room options-private, studio, or shared-with amenities such as private bathrooms, air conditioning, cable TV, furnished rooms, kitchenettes, telephones, and Wi-Fi, so families and residents have a few choices based on what's wanted or needed. There's all-day dining in a community dining room with professional chef-prepared meals covering allergies and diabetes needs, plus meal service for special diets, and the community keeps family councils and resident councils so there's a channel for people to be heard about their care. For activities, Hereford Nursing & Rehab offers outdoor gardens, walking paths, community gardens, arts and crafts, bingo, movie nights, fitness, social outings, and various resident-run or community-sponsored events, including off-site outings and programs in their arts or spa/wellness rooms, which can be important for staying busy and connected.

The staff runs 24-hour supervision with an emergency alert system and helps with all kinds of daily assistance, so families know there is oversight, and the facility employs only English-speaking bilingual caregivers. Services cover housekeeping, dry cleaning, family support, and move-in help, with on-site beauty services available as well, and for those concerned about healthcare standards, most needs get met with regular care, memory care, pain management, and non-ambulatory support, though the history of deficiencies suggests some areas do need watchful oversight. With a steady ownership record and care services for intermediate and long-term medical needs, Hereford Nursing & Rehab typically supports residents who need frequent medical care, routine help, or specialized nursing, but it's good to review their inspection records and quality grades for full context, since they do maintain mostly high marks but have room for improvement in certain procedures.
