    Attentive staff, effective care, family

I was very pleased with the care my mom received - nurses and CNAs were attentive and caring, therapy was effective and helped her recover quickly, and medications and wound care were managed reliably. The facility was spotless, welcoming and resident-focused; staff from administration to drivers and the social worker went above and beyond, meals were enjoyable, and social activities made it feel like family.

    Summary of reviews

Legacy at Jacksonville presents a mixed profile: many families describe a clean, modern building with strong rehabilitation resources and caring direct-care staff, while other families report significant operational and communication weaknesses. Positive accounts emphasize attentive nursing, effective therapy teams, supportive CNAs, helpful social-work intervention and practical discharge planning that facilitated safe returns home. The facility's public spaces and rooms are frequently described as bright and well maintained, and on-site amenities such as salon services, organized activities (bingo and social engagement), and transportation support are noted as meaningful quality-of-life contributors.

Care quality is uneven across reports. Several reviewers praised rapid rehabilitative progress, daily wound care, and reliable medication administration that led to quick recoveries and safe discharges. Conversely, other families described delays in clinical responsiveness, inconsistent pain-management coordination, and breakdowns in handoffs between shifts. There are a small number of serious clinical concerns described, including severe infection and hospitalization; these are significant and families highlighted gaps in clinical incident response and subsequent communication.

Staff behavior and team performance receive both strong praise and criticism. Many accounts highlight compassionate, attentive CNAs and nurses who engage residents and support recovery. At the same time, others describe inconsistent conduct from some therapy staff and aides, and concerns about how certain residents were handled or communicated with. High staff turnover and reports of low morale or supervisory support appear to correlate with perceptions of declining consistency in care.

Dining and daily-life experiences are variable. Some residents and families enjoyed meals and named vendors favorably, while others found food bland, inconsistent in quality, and occasionally lacking in protein or variety, particularly on weekends. Recreational offerings such as bingo, social activities, and personalized touches (flowers, music) were positively noted, but families of higher-dependency residents reported that those residents sometimes received limited socialization or were not consistently escorted to communal areas.

Facilities and accessibility are generally regarded as strengths: the building is described as clean, modern, and welcoming. However, a few reviewers identified practical accessibility and safety issues โ€” for example, door mechanisms and exit monitoring โ€” that suggest opportunities to strengthen resident-monitoring and facility access controls.

Management, communication, and billing are recurring areas of concern. Multiple families reported difficulty reaching administrative staff, inconsistent or contradictory information, and inadequate corporate follow-up. Financial coordination issues including insurance disputes and unexpected out-of-pocket expenses were also described as sources of stress for families.

Overall, Legacy at Jacksonville appears to provide high-quality rehabilitation and compassionate direct-care support in many cases, but experiences are polarized. Prospective residents and families should weigh the facilityโ€™s strong therapy and nursing capabilities and its clean environment against the documented variability in staffing, communication, and operational consistency. When considering admission, families may wish to ask specific, documented questions about current staff turnover rates, shift-handoff procedures, pain- and medication-management protocols, accessibility features, dining menus and nutrition plans, and the facilityโ€™s process for handling clinical incidents and billing disputes.

    About Legacy at Jacksonville

Legacy at Jacksonville is a senior living community with 101 certified beds and an average of 81 residents per day, offering both long-term nursing care and short-term rehabilitation, and it's affiliated with Southwest LTC, so folks get access to a broad range of care including skilled nursing, rehab services, palliative care, tracheostomy care, psychiatric care, podiatry and dental services, as well as social services by licensed staff. The community provides private and semi-private rooms that are comfortable, plus there's a spacious lounge, a dining area with restaurant-style service, walking paths, and a courtyard, and you'll also find a beauty parlor, daily social activities, wifi, and satellite television. There's a focus on personal care, with a licensed administrator, medical director, consulting pharmacist, and registered dietician leading the team, and the nurses and therapists provide speech, physical, and occupational therapy, all in dedicated spaces with plenty of therapy equipment like resistance bands and therapy balls, and the wound care nurse is VOHRA-certified, which is good for complicated wounds. Legacy at Jacksonville emphasizes daily activities and engagement, aiming to support each resident's independence and overall health, though staff turnover for nurses is high at 71.7%, with each resident getting about 3.4 hours of nurse time per day. The facility does have some challenges, with 12 total deficiencies on inspection reports - these include an infection control issue, a care planning problem about resident assessments, and a resident rights matter related to grievances, as noted in the February 5, 2025, inspection. Legacy at Jacksonville uses telehealth to help with care, and there's a virtual tour and photo gallery for people who want to look before coming, and with a number of remodeled spaces, comfortable seating, and home-like touches, residents have places to relax or join in group or individual activities. Safety is considered with an emergency call system, and there's fall prevention and diabetes management along with stroke rehab. Being next door to a dialysis clinic adds some convenience for those who need that care, and with social workers focusing on emotional and spiritual support, the community is set up to help people work toward their best possible health.
