    Attentive, clean, home-like personalized care

I've had a very positive experience here - attentive, kind, long-tenured staff provide personalized, family-like care. My private room was spotlessly clean, meals were home-style and enjoyable, and the friendly front desk, rehab gym and activities make it feel like home. Barbie and the team were especially helpful; I would happily recommend this facility for my mom.

    Summary of reviews

The reviews present a mixed picture of Heritage at Longview Healthcare Center. Several families and residents emphasize strong points: many describe clean rooms and common areas, a welcoming front desk, and a staff culture that can be warm and teamโ€‘oriented. Clinical staff โ€” including nurses and CNAs โ€” are frequently described as caring and attentive, and long staff tenure is cited as a stability factor. The facilityโ€™s rehabilitation resources receive repeated praise for their size, equipment, and physical space, and the dining experience is often framed positively (homeโ€‘style meals, enjoyable food). Activities and programming are also noted as inclusive and engaging for residents.

Alongside those positives, recurring operational concerns appear across reviews. Communication and customerโ€‘service responsiveness are inconsistent; families report phone handling issues and difficulty getting clear, timely updates. Staffing levels are a significant theme: reviewers reference understaffing or inconsistent nurse coverage that can translate into delays in personalโ€‘care assistance and slower responses to resident needs. Meal service is another area of concern โ€” while food quality is praised by some, other comments point to limited menu variety, repetitive rotations, and a lack of customization or alternatives for resident preferences and dietary needs.

Clinical quality patterns are mixed. Although many reviewers commend attentive nursing, some raise serious concerns about wound care and pressureโ€‘injury prevention, and there are isolated allegations of severe clinicalโ€‘care failures. Rehabilitation receives praise for facilities and equipment, but there are multiple reports of variability in therapy intensity and a desire for more oneโ€‘onโ€‘one attention or atโ€‘home followโ€‘through. Facility condition is generally described as clean, but a few reviewers note aging or unevenly maintained areas.

For prospective residents and families, the overall picture suggests a facility with substantial strengths in environment, therapy space, and individual staff members, balanced against operational weaknesses that can affect dayโ€‘toโ€‘day care: communication, staffing consistency, meal customization, and certain aspects of clinical care. When evaluating the facility, families may want to ask about current staffing ratios, woundโ€‘care protocols and incident followโ€‘up, meal menus and accommodation processes, therapy plans and expected session intensity, and the facilityโ€™s approach to family communication and complaint resolution. These targeted questions can help clarify whether the facilityโ€™s prevailing strengths align with a prospective residentโ€™s priorities and clinical needs.

About Heritage at Longview Healthcare Center

Heritage at Longview Healthcare Center is a skilled nursing facility with over 130 certified beds, offering both short-term rehabilitation and long-term care services for people needing help with health and daily living. The center provides nursing home care, memory care, assisted living, and support for veterans, alongside therapy services like physical, occupational, and speech therapy. Its care team handles specialized needs with programs for cardiac care, diabetes, wound care, stroke recovery, respiratory support including vent care, dialysis in-house, as well as tracheotomy care. Pain management, medication services, and pharmacy services are available, and a licensed pharmacist is on staff. Heritage at Longview manages hospice, respite care, and even provides transportation to dialysis and for general outings, plus religious services, beauty shop, and outings for social engagement. The team creates personalized care plans and dietary experts prepare meals to support nutrition. Renovations have updated the building and created comfortable, home-like common spaces and outdoor areas for relaxing or visiting. Laundry, housekeeping, podiatry, dental, and vision services support residents' everyday needs. The average daily census is about 57 residents, and the facility is set up for 140 beds, supporting both privacy and care as needed.

In recent regulatory inspections, the facility's records document 24 total deficiencies, including areas like pressure ulcer care, pharmacy services, and timely resident health notifications. Some issues related to pressure ulcer care were marked as immediate jeopardy, which means they represented a serious risk to residents' safety, though the scope was limited to a few people. Infection control had two separate deficiencies. Heritage at Longview reports a nurse turnover rate of 40.9%, lower than the state's average, while the nurse staffing level is 2.63 hours per resident each day, which is under the Texas average. The facility's inspection reports use care quality and regulatory terms, and complaints from February 2025 are in the record. Heritage at Longview is managed by Southwest LTC since 2017 and is listed as part of the Creative Solutions in Healthcare family network, which focuses on quality care and customer service, with a dedicated customer care line for families and residents. Public sources do not show details about the owners, which suggests private or undisclosed ownership.

The staff places importance on resident rights, making sure residents and families get updates about injuries or serious changes in health, and an online portal helps families stay informed. The center supports an environment where residents are encouraged to relax, participate in activities, and connect with others. Heritage at Longview's team continues to focus on patient-centered care, attention to safety, and improving residents' quality of life through a mix of programs and support services.
