    Caring staff, clean and welcoming

I visited and was impressed: helpful, courteous, professional staff and caring management (Mr. McCaskill) made the place feel home-like and very clean, and my brother's health improved. The remodeling, elegant piano in the dining area, good food and staff who listened left a positive impression - I would recommend this facility.

    Summary of reviews

Overall impression: Reviews present a mixed picture. Several accounts emphasize caring, courteous staff and visible management engagement, while others raise facility-level operational concerns. Families described positive personal interactions, improvements in individual residents, and recent aesthetic upgrades; concurrently, reviewers identified recurring issues with sanitation, staffing, and administrative communication.

Care quality: Many reviewers praised nurses and direct-care staff for kindness and attentiveness, and some families reported measurable health improvements in residents. However, there are also concerns about staff conduct, timeliness of responses, and inconsistent application of care protocols. Specific care-related operational weaknesses include delayed assistance due to staffing levels and lapses in privacy and bedside communication practices.

Staff and management: Staffing appears variable. Positive comments describe courteous, professional employees and an engaged manager who is visible to families. At the same time, reviewers noted understaffing, perceived rudeness from some nursing or administrative personnel, and comments linking compensation pressures to workforce instability. Administrative communication was described as inconsistent, which can complicate care coordination and family relations.

Dining and activities: The facility offers an appealing dining environmentโ€”reviewers mentioned an elegant piano in the dining area and generally favorable comments about the food. The overall atmosphere in public areas was described as home-like by some visitors. Specific activity programming beyond the dining setting was not frequently described in the remarks provided.

Facilities and operations: The facility shows signs of recent improvements in common areas, including painting and remodeling, but reviewers also indicated that some resident rooms and building areas are dated. There are operational gaps reported in housekeeping and sanitation practices, and several comments referenced odor concerns in parts of the building. Security and reception processes appear limited in practice; reviewers mentioned a lack of consistent front-desk presence and visitor screening. In addition, the absence of bedside phones or other resident-accessible communication options was cited as a practical shortcoming.

Notable patterns and recommendations for visitors: The pattern in these reviews is one of strong interpersonal strengths among many caregivers and visible management efforts, coupled with operational weaknesses that include inconsistent communication, staffing shortfalls, housekeeping variability, and infrastructure gaps. Prospective residents and families should tour the facility during different times of day, ask about staffing ratios and turnover, review cleaning and maintenance schedules, confirm visitor and reception procedures, and verify availability of resident communication devices and privacy protocols. Speaking directly with management about recent renovations and ongoing improvement plans may clarify which concerns are being actively addressed.

    About Lytle Nursing Home

Lytle Nursing Home is a small, for-profit facility with 70 beds, offering both short-term rehabilitation and long-term care services. The facility participates in both Medicare and Medicaid, providing care options for a variety of resident needs and financial backgrounds. It is not part of a continuing care retirement community and is managed by an LLC ownership structure. Residents of Lytle Nursing Home benefit from a focused environment geared towards those who require assistance with daily activities and ongoing medical care.

The community at Lytle Nursing Home caters to individuals who need consistent support with activities such as medication administration, dressing, eating, and personal hygiene. A significant portion of residentsโ€”about 71.2%โ€”are able to maintain self-care abilities like moving, eating, and using the bathroom with little or no help. The facility maintains a strong commitment to resident health through comprehensive vaccination efforts, with a 100% influenza vaccination rate reported among residents, demonstrating attention to preventive health practices.

In terms of staffing, Lytle Nursing Home provides an average of 1 hour and 16 minutes of nurse staffing per resident each day. Weekend staffing levels are noted to remain stable compared to weekday coverage, which helps support consistent care seven days a week. This level of staff consistency is important in long-term care settings to ensure familiarity and continuity in the relationships between caregivers and residents.

Recent inspection reports highlight several areas that the facility has addressed, such as care plan development and implementation, appropriate record-keeping, and assessment coordination. Some challenges have been identified related to creating care plans promptly upon admission, providing adequate room sizes, and establishing policies for resident needs, such as smoking. In addition, there have been findings related to the delivery of catheter care and preventive measures for urinary tract infections. Over the past three years, the facility has paid a notable amount in fines, indicating a period of regulatory oversight and correction.

Despite these challenges, the facility demonstrates a commitment to making improvements and adhering to required standards for food preparation, resident safety, and overall well-being. Lytle Nursing Home participates in regular health, fire, and safety inspections, and acts to resolve any deficiencies within specified timeframes. Residents are supported by a care environment that aims to accommodate their preferences and uphold standards for privacy, medical record maintenance, and immediate care needs upon admission. The nursing home continues to provide care for older adults in the community, striving to meet the complex needs associated with both short-term rehabilitation and long-term support.
