    Excellent care, friendly staff, homelike

I live here and am very pleased - the staff are consistently friendly, attentive and knowledgeable, nursing and therapy care are excellent, and communication with families is proactive. The facility is beautiful, well-kept with a lovely courtyard, activities foster real camaraderie, and it truly feels like home, though there's still a bit of room for small improvements.

    Summary of reviews

Reviewer feedback presents a polarized picture of Focused Care at Hogan Park. Many comments highlight positive elements โ€” an active social program, a strong therapy/rehabilitation department, a pleasant courtyard and outdoor spaces, and staff who are described as caring, friendly, and knowledgeable. Several reviewers praised a welcoming, home-like atmosphere and cited proactive family communication and attentive customer-service interactions. Recent management changes and facility upgrades were also noted as positive developments.

At the same time, there are recurring operational concerns that prospective families should evaluate closely. Multiple accounts indicate inconsistent personal-care practices, including delays in addressing incontinence needs and irregular bathing schedules, which suggest gaps in routine care protocols. Related to this, reviewers raised sanitation and linen-management issues in resident rooms and shared areas, as well as incidents of residents wearing othersโ€™ clothing, pointing to weaknesses in room-assignment, laundry, or housekeeping oversight.

Infection-control and PPE compliance emerged as another pattern: reports described quarantine signage and inconsistent use of protective equipment by staff. Families also reported uneven responsiveness from staff and variability in professionalism, with some shifts or employees delivering high-quality care while others were perceived as unresponsive or upsetting to residents. Several reviewers described problematic communication after incidents and difficulties resolving billing or relocation disputes; there is a clear need for stronger incident-response procedures and more transparent family communication channels. Separately, there are allegations of theft and other financial-misconduct incidents that warrant verification through facility management or regulatory records.

Facilities-wise, the property and outdoor areas receive favorable mention, but there were concerns about environmental-safety communication during renovations โ€” specifically, notifications and safeguards around hazards such as asbestos were described as inadequate. Dining was not a prominent theme in the feedback provided; there is no consistent commentary on meal quality or service, so families should request current menus and sample meal service during a visit.

For families considering this facility, recommended due diligence includes touring multiple units and meal times, meeting nursing and direct-care staff across shifts, reviewing recent inspection and incident records, asking for written infection-control and renovation-safety plans, and clarifying billing and complaint-resolution processes. The facility appears capable of delivering very good therapy and social programming and can be a positive environment for many residents, but the operational inconsistencies noted should be addressed or verified before making a placement decision.

About Focused Care at Hogan Park

Focused Care at Hogan Park is a nursing home in Midland, Texas, that offers skilled nursing and intermediate care for people who need help with daily life and medical care, and the care team is available 24 hours a day, seven days a week, so residents always have someone nearby if they need support or have a health issue. The facility has 106 certified beds and handles short-term rehabilitation for those needing therapy before going home from the hospital, so there's help for people who need to regain their strength and independence, and folks can get specialized therapies right there, including physical therapy to help with movement and pain, occupational therapy to work towards doing everyday tasks again, and speech therapy to help with communication, especially for people who've had strokes or other setbacks. The care team also handles wound care, respiratory therapy including care for things like COPD, pain management, dialysis management, and even more specialized services like TRACH care, hospice, palliative support, pharmacy, podiatry, and dental care-so there's a range of medical services under one roof. The staff manages chronic and acute wounds with treatments like Negative Pressure Wound Therapy and helps people after surgery, and the center has a new therapy gym with modern equipment for different types of rehabilitation. The whole building uses electronic health records so doctors, nurses, and therapists can stay updated, and they can coordinate with pharmacies or labs as needed since everything is linked together. The facility stays open at all hours, and services include personal and nursing care plus dietary assistance for very frail residents who can't live alone anymore, so families can consider this as a place with round-the-clock support.

The staff at Focused Care at Hogan Park tries to make care plans personal, working with each resident's needs to help manage their health and make daily life more comfortable. The center belongs to Focused Post Acute Care Partners and is managed by FPacp Hogan Park LLC together with Midland County Hospital District, and management has been in place since 2017. The center also offers support to families who are planning long-term care for their loved ones, and it gives expert advice for caregiving situations, which some families find helpful. On the practical side of things, nurse staffing is measured at 2.15 nurse hours per resident per day, and the turnover rate for nurses is 72.1%, which means staff may change more than some other places. The facility has had documented issues from inspections, including deficiencies related to infection control, food sourcing, and visitation rights, so families may want to review inspection reports themselves for more context. Overall, Focused Care at Hogan Park has typical amenities you'd expect in a skilled nursing facility for people who need a high level of support, and its review rating is 3.7 based on 6 reviews online.
