    Clean welcoming facility, caring staff

I'm very satisfied - clean, welcoming, and family-like with caring, long-tenured staff (Meagan, Tonya, Jennifer and team) who communicate well and regularly go above and beyond. Nursing and therapy were high quality, activities are engaging, and transitions/transportation were smooth. Overall a very good facility I would recommend.

    Summary of reviews

Overall impression: Lakeview Rehabilitation and Healthcare Center is consistently described as a small, family‑style facility with notable strengths in staff compassion, clinical rehabilitation, and resident engagement. Families frequently described staff as caring and attentive, with many reviewers praising specific employees and leadership for hands‑on involvement. The facility’s atmosphere is characterized as welcoming, home‑like, and clean, and several accounts highlight successful transitions and improvements in mobility and clinical status during stays.

Care and clinical services: Nursing and therapy services receive strong positive remarks. Reviewers praised clinical attentiveness, effective rehabilitation outcomes, and coordination between nursing and therapy teams. Several families credited the staff with identifying clinical issues missed elsewhere and with improving mobility and stabilization. Safety measures such as alarm bracelets and attentive monitoring were mentioned as contributing to resident security.

Staff and management: A prominent pattern is high staff longevity and low turnover, which residents and families associate with continuity of care and trusting relationships. Multiple reviewers named administrators, the director of nursing, social workers, and activity staff as being accessible and actively involved. Social‑work and admissions staff received positive comments for easing transitions and helping with insurance and placement logistics. Leadership involvement appears to be visible and appreciated by families.

Dining and activities: Dining and activity offerings are described positively. Residents and families commented on enjoyable meals and an activities program that includes regular events, games, outings, and holiday celebrations; the activity director is singled out for strong engagement. The program is presented as a meaningful contributor to resident quality of life and family satisfaction.

Facilities and operations: The facility is generally described as clean, well‑maintained, and small/intimate, which contributes to a home‑like feel and easy family access. On‑site transportation and staffed drivers are noted as practical conveniences. Housekeeping and maintenance are referenced as consistent strengths.

Areas of concern and variability: While the dominant tone is positive, there are operational concerns to note. A subset of comments identifies periodic staffing shortages that can affect responsiveness to personal‑care needs; this includes mentions of inconsistent bathing or personal‑care scheduling. Families also described occasional variability in continuity of care across shifts. Administrative and insurance‑related delays were mentioned as a barrier for some admissions or program starts. These issues suggest that while clinical quality and interpersonal care are strong, operational strain during certain shifts or administrative processes can affect timeliness and consistency.

Summary assessment: For prospective residents and families, Lakeview appears to offer strong clinical rehabilitation, a compassionate and stable workforce, an active life‑enrichment program, and a clean, welcoming environment. Interested parties should confirm current staffing patterns and inquire about how the facility manages personal‑care scheduling and insurance/program authorizations to ensure a match with individual needs and expectations.

About Lakeview Rehabilitation and Healthcare Center

Lakeview Rehabilitation and Healthcare Center has 60 certified beds and is managed by Nexion Health At Winnsboro, Inc., while West Wharton County Hospital District owns it, and you'll notice that it's got a long history and a variety of services aimed at long-term care, skilled nursing, and rehabilitation for its residents, with the place offering around-the-clock nursing care and admitting people day or night, seven days a week, which comes in handy when you need to get someone settled quickly. The care team includes staff with clinical expertise, providing 3.41 nurse hours per resident per day, and their nurse turnover rate sits at 37.5%. Residents here have access to individualized care services, long-term care services, and skilled nursing, with extra support for wound care, as well as telemedicine services for remote doctor visits, and specialized care for those recovering from different health conditions, along with amenities and activities meant to make life a little brighter, like games of poker, foot spa days, and drinks, so folks get a bit of social life and comfort even if they're in recovery or living there for a longer stay. The facility allows pets to visit and encourages a home-like atmosphere, putting focus on both health and the well-being of residents, and if someone needs rehab, they've got programs designed to help with that, along with features around the facility to support recovery and comfort during the process, and they keep a dedication to comprehensive services and a comfortable setting for everyone who calls it home.

Now, there are some things to keep in mind, because Lakeview Rehabilitation and Healthcare Center has faced citations for not always telling residents, families, or doctors right away about issues that affect health or safety, and they've been cited for problems with safe and proper pain management, which led to fines, including one for $34,234, and while the staff tries to keep the place clean and safe, the latest standard inspection in November 2024 showed twelve deficiencies, with one being infection-related, and out of their total inspections, there have been twenty-four deficiencies, with two infection-related, so they've got room for improvement in how they handle infections and health concerns, and though they do offer individualized care and claim a home-like, welcoming environment, it's important to know there've been documented shortfalls in following all health and safety rules. The facility delivers on a wide range of services and tries to meet each resident's needs with specialized programs and activities for social engagement and recovery, but anyone considering Lakeview should weigh the good with the areas that need attention, especially when thinking about a safe, long-term home for a loved one.
