    Summary of reviews

Autumn Park Assisted Living is portrayed overall as a small, cottage-style community that emphasizes a home-like atmosphere and individualized attention. Reviewers frequently highlight the facility’s modest size (around 32 beds), large private rooms, and modern, inviting interior as contributors to a sense of comfort and dignity. Many accounts describe a warm, family-like environment where residents receive one-on-one interaction and personalized touches such as names on doors.

Care and staffing are among the facility’s strongest themes. Numerous comments describe compassionate, attentive caregivers and a responsive leadership team; several reviewers named individual staff members and cited professional, kind interactions. That said, there is variability in experience. A subset of accounts raises concerns about inconsistent staff behavior and lapses in supervision or timeliness of clinical response. Those accounts suggest that quality of care can depend on the particular staff on duty, indicating variability in conduct and clinical responsiveness across shifts or personnel.

Dining and activities are generally viewed positively. Many reviewers praise the meals — including special holiday dinners — and note that activity programming supports social engagement and resident satisfaction. Conversely, some feedback points to uneven meal quality, indicating that menu execution and consistency can fluctuate.

Facility condition and management impressions are mixed. The community is often described as clean, modern, and well-maintained, with recent improvements and resident-focused features (for example, accessible bathrooms). At the same time, other accounts flag sanitation concerns and occasional maintenance reliability problems that affect essential services (for example, heating or water availability). Management is credited at times with pride in care and responsiveness, yet other comments describe inconsistent communication and perceived gaps in follow-through.

Notable patterns for prospective residents and families: positive experiences tend to emphasize the community’s small size, dedicated caregivers, and personalized approach; negative accounts cluster around inconsistent staffing practices, occasional failures in maintenance or utility reliability, and instances of delayed response to urgent resident needs. A practical next step for families evaluating Autumn Park is to tour the facility at varied times, ask about staff-to-resident ratios and shift handoffs, review emergency-response protocols and recent maintenance records, and sample the dining program. These steps can help clarify whether the community’s strengths align with an individual resident’s needs and whether the facility’s variability is acceptable in a particular care situation.

The community opened in 2015 and holds a state license under Facility License Number 2018-ALII-UT000820. Residents can pick from studio layouts and live in a medium-sized setting with 31 beds. The atmosphere feels like a home, where the staff treats people like family and pays close attention to each resident with a strong patient-to-staff ratio. There's a priority on making everyone comfortable, keeping the place clean and easy to get around, and serving home-cooked, fresh meals in a welcoming dining room.
