    Attentive, professional care and amenities

I'm very pleased with the care my grandfather received - friendly, professional staff who went above and beyond, an excellent PT/OT team that got him back to baseline, and clean, newly remodeled rooms in a welcoming, family atmosphere. Management and communication have been straightforward and helpful, meals are good, and I'd recommend a visit to see the attentive, high-quality care for yourself.

    Summary of reviews

Reviews for Curis at Lynchburg Nursing & Rehabilitation Center describe a facility with a wide range of experiences, from highly positive rehabilitation and individualized caregiving to persistent operational and communication shortcomings. The most consistent positive theme relates to therapy and rehabilitation: the facility’s PT/OT teams and therapy department receive repeated praise for getting residents back to baseline and for delivering structured, effective rehabilitation. Multiple reviewers also singled out individual caregivers and nurses for compassionate, attentive care, and several described a welcoming admissions experience, updated rooms, and an attractive courtyard.

At the same time, a substantial set of operational concerns emerges across reviews. Staffing and shift coverage are described as inconsistent, producing delays in responding to resident needs and family inquiries. Linked to staffing issues are communication breakdowns with families and occasional unresponsiveness from leadership or clinical managers. Several comments point to gaps in clinical coordination — for example, missed or delayed treatments, challenges coordinating dialysis, and inconsistent medication or therapy follow-up — which suggest weaknesses in clinical handoffs and oversight.

Dining and personal‑care patterns are uneven. Some families praise homemade meals and special offerings, while others describe failures to follow prescribed renal or diabetic diets, abrupt kitchen closures, and inconsistent meal delivery. Personal‑care concerns include irregular bathing schedules, inconsistent incontinence care, and reports of bedding or clothing mishandling. Environmental maintenance and sanitation are also mixed: some rooms are described as newly remodeled and clean, whereas other accounts raise odor concerns, pest‑control issues, and HVAC or room temperature maintenance problems.

Management and administrative processes appear to be in transition for parts of the facility. Reviews reference new ownership and leadership with stated improvement efforts, and some families saw positive change over time. However, other reviewers report problematic administrative practices including billing and discharge coordination problems, inadequate post‑discharge follow‑up, perceived favoritism, and front‑desk coverage lapses that affect safety and access. A number of accounts describe emergency transfers to higher levels of care linked to coordination or dietary issues, underscoring the operational impact of the listed gaps.

For prospective residents and families: the facility demonstrates clear strengths in rehabilitation and has individual staff who provide highly regarded, resident‑centered care. At the same time, operational inconsistencies — particularly staffing reliability, clinical coordination (including dialysis and medication timing), dietary adherence, personal‑care scheduling, environmental maintenance, and communication with families — are recurring themes that merit direct inquiry. When assessing this facility, consider touring during different shifts, asking about staffing ratios, clinical protocols for dialysis and specialty diets, incontinence and bathing schedules, pest‑control measures, HVAC maintenance, discharge planning, and examples of recent quality‑improvement initiatives and outcomes. Consulting recent state inspection reports and speaking with current families or discharge planners will help clarify whether recent management changes have addressed the documented issues.

About Curis at Lynchburg Nursing & Rehabilitation Center

Curis At Lynchburg Nursing & Rehabilitation Center sits at 2081 Langhorne Road in Lynchburg, VA, inside a hospital, and has 120 beds for residents needing skilled nursing care and rehabilitation. The center is open every day and night and uses a 24-hour call system with 12-16 hour nursing support, so residents always get help when needed, though staffing hours are lower than average, which shows in the 1 out of 5 staff rating. It offers a range of care, including rehabilitation, wound care, help with daily activities, medication management, and memory care for people with dementia or Alzheimer's in secure settings. Residents get services from registered nurses and therapy staff for speech, occupational, and physical needs, and the rehabilitation program stands out as exceptional in helping folks recover and regain strength or mobility, with both inpatient and outpatient options.

People can stay for short or long periods, and the center takes both Medicare and Medicaid. Private and semi-private rooms have private bathrooms, air conditioning, furnished spaces, cable TV, Wi-Fi, kitchenettes, and phones. Amenities aim for comfort and wellness, like a dining room with restaurant-style meals and special diets, gardens, walking paths, a game room, library, fitness room, spa/sauna, beauty salon, and activity spaces, plus scheduled activities, movie nights, art programs, and outdoor events to keep residents active and connected. There's also help with housekeeping, move-in, laundry, and family support.

Curis At Lynchburg is owned by The Portopiccolo Group and switched hands within the last two years. The administrator is Nathan Libassi. The facility operates as a for-profit LLC and doesn't have resident or family councils. It's had some problems in the past, including $53,547 in fines and eight substantiated complaints, and its overall quality and health ratings stand at 2 out of 5, with most long-term care grades being D or F but some recent inspections earning a B-minus. Some minor deficiencies have come up, but nothing severe was found in inspections. The facility is ranked low overall in Virginia-274th out of 286 nursing homes-but is ranked higher within Lynchburg and within 100 miles, even if those rankings are still toward the bottom among local facilities. Despite the low scores, staff aim to give residents a safe place to live, offering a range of therapies and lifestyle options, including independent living with maintenance-free options, round-the-clock support, and planned group activities.

Residents benefit from a sprinkler system for fire safety, a high occupancy rate, and amenities that cover most needs, like a computer center, common areas, outdoor spaces, and transportation for trips or appointments. Services focus on helping people regain independence after illness or surgery while providing support for those who need more help with daily life. The environment works hard to feel caring and family-like, with care personalized to each resident, but people looking at Curis At Lynchburg Nursing & Rehabilitation Center should know about its inspection history, low ratings, and staffing numbers when weighing options.
