    Compassionate staff and clean facility

I'm very pleased with the care my family member received - the staff were compassionate, friendly and professional, nurses and therapists knowledgeable, and admissions was smooth. The facility is clean, bright and homey with engaging activities and a genuinely caring team; I'd happily recommend it.

    Summary of reviews

Reviews of Tate Springs Health & Rehabilitation are mixed but consistent in two core themes: many families praise the staff and therapy capabilities, while operational weaknesses produce variability in day-to-day care.

Care quality and staffing: Numerous accounts highlight compassionate, attentive nurses, caring CNAs, and a therapy team that provides strong short-term and long-term rehab outcomes. At the same time, recurring concerns center on inconsistent staffing levelsโ€”especially nights and weekendsโ€”and uneven responsiveness. These staffing patterns are linked to delays in assistance, missed or delayed medications, and inconsistent delivery of basic-care tasks such as bathing and timely toileting assistance. A subset of reviews describes serious clinical incidents (including falls or transfer-related events) and delayed family notification; these accounts underscore the need to verify transfer protocols, staffing ratios, and incident-reporting practices before placement.

Therapy, activities, and dining: The rehabilitation and therapy staff receive frequent positive mention for being knowledgeable and effective, and many families report smooth transitions to memory care and efficient equipment coordination. The facility offers a range of activities and social programming that contribute to resident engagement and a family-like atmosphere. Dining impressions are mixed: some reviewers commend nutritious and tasty meals and attentive kitchen staff, while others note problems with meal temperature, timing, and occasional supplement/dietary delivery issues.

Facilities and cleanliness: Many reviewers describe a welcoming, home-like environment, clean common areas, well-kept grounds, and responsive housekeeping. However, several reviews point to aging infrastructureโ€”worn corridors, dark rooms, dated bathing fixturesโ€”and specific sanitation and maintenance inconsistencies in some units (for example, bathing areas and ceiling cleanliness). Accessibility improvements such as walk-in tubs and updated bathrooms are noted as needed by families with higher mobility needs.

Management and communication: Experiences with administration vary. Positive comments highlight helpful admissions staff, an efficient business office, and clinical leadership that has improved care in some periods. Conversely, reviewers describe high administrative turnover, slow or unresponsive follow-up on inquiries, and occasional problems with managing residentsโ€™ belongings and patient funds. Infection-control policies and visitor restrictions during outbreaks were sources of frustration for some families, particularly when communication about those policies felt inflexible.

Notable patterns and considerations for prospective families: The facility generates polarized experiencesโ€”many families feel reassured by the staffโ€™s compassion and the therapy outcomes, while others encountered service gaps tied to staffing variability and operational controls (medication timing, transfer safety, meal service consistency). Prospective residents and families should directly inquire about current staffing ratios (including night/weekend coverage), transfer and fall-prevention protocols, medication-administration procedures, policies for handling personal belongings and funds, recent management changes, and the status of planned facility upgrades (bathroom accessibility, ventilation/cleaning routines). Given the mix of strong clinical and interpersonal strengths alongside recurring operational issues, an in-person tour, staff interview, and current quality metrics or inspection results are advisable to assess whether Tate Springs aligns with a specific residentโ€™s clinical and safety needs.

About Tate Springs Health & Rehabilitation

Tate Springs Health & Rehabilitation sits over at 2200 Landover Place in Lynchburg, Virginia, and the place is pretty new, having been founded in 2023, so it's got those fresh renovations with modern amenities like a rehab gym, a beauty and barber shop, a library, and some well-kept outdoor spaces, including a patio garden and a recreation area, which folks tend to enjoy. This facility runs as a non-profit and falls under Hill Valley Healthcare, which puts a big focus on skilled nursing, short-term rehab, long-term care, and dementia care, and they take folks in for both short stays to recover or for longer-term needs when that's what life calls for.

Inside, there are 59 licensed beds, and the staff keeps a "People First" approach, meaning the care team, including registered nurses, LPNs, CNAs, and a Certified Activity Director and Admissions Coordinator, works all around the clock to help with complex medical needs like wound care, tracheostomy care, pulmonary support, orthopedic rehab, oncology, bariatric care, and post-surgical care, as well as occupational, physical, and speech therapy, and of course, Alzheimer's and dementia care if folks need extra support with memory. There's always someone to help, whether that's managing medications, providing 24-hour nursing, or just making sure the environment feels supportive and safe with comfortable living areas.

There are also hospice care options for residents and transportation services when folks have somewhere to be, and they make sure to have things like pastoral support, community programs, and a variety of resources for veterans, seniors, and those living with disabilities, all backed up by a resource library filled with articles, books, guides, and news on pretty much every aspect of aging and senior living. The facility has a strong staff training program, supports ongoing learning, and values diversity, with a wellness program to help staff stay healthy too. Jobs sometimes come with sign-on bonuses for nursing roles like LPNs and RNs. Tate Springs is part of the Virginia Center for Assisted Living and is listed under skilled nursing facilities, with efficiency measures in place, and daily rates for both semi-private and private rooms, and the administrator, Mr. Matthew Brooks, oversees operations.

With services like 24-hour nursing, comprehensive post-acute rehabilitation, long-term care, respite stays, dementia care, and all those amenities, the team sticks close by to answer questions and help folks set up a tour when they're thinking about making a move.
