    Attentive staff and excellent rehab

I'm very pleased with my loved one's care here - the staff are professional, friendly and genuinely caring, nurses and CNAs attentive, and the PT/OT rehab is excellent. The facility is clean and well-maintained with a pleasant courtyard, responsive leadership and good activities; I would recommend them for rehab and long-term care.

    Summary of reviews

Puget Sound Care elicits strongly polarized impressions. Many reviewers praised the facility’s rehabilitation services, noting effective physical and occupational therapy programs and an attentive therapy team that often expedited recovery. Multiple accounts describe compassionate and skilled nursing staff, a clean and well-maintained building with pleasant outdoor spaces, an active activities calendar, and helpful administrative personnel including front-desk staff and financial coordinators. These positive reports emphasize frequent provider engagement, supportive discharge coordination in some cases, and a welcoming environment that can feel like home for residents.

Contrasting these positives, a recurrent set of operational concerns appears across reviews. Staffing levels and workload are a consistent theme: reviewers describe delayed responses to call bells, long wait times for toileting and bathing assistance, and missed or inconsistent personal-care tasks. Related to staffing are gaps in meal service and dietary management — reviewers reported inconsistent meal availability, food quality variability, and instances where dietary or allergy needs were not reliably managed. Several comments also raise sanitation and odor concerns in specific areas, inconsistent housekeeping, and occasional pest-control concerns.

Clinical coordination and transition-of-care processes show variability. There are multiple references to delayed medical follow-up (labs, equipment, or specialist orders), slow replacement of mobility equipment, and discharge planning errors that created confusion or led to rapid readmission in a few cases. Some reviews described unacceptable communication tone and unprofessional conduct by certain staff members, and others noted privacy or front-desk process inconsistencies (for example, an unexpected sign-in/photo requirement or unattended reception periods). A small number of serious individual claims — including allegations about financial motives and concerns following resident deaths or critical incidents — were also raised; these items warrant direct verification with the facility and, if needed, regulatory follow-up.

Taken together, the pattern suggests that Puget Sound Care delivers high-quality rehabilitation and has many staff and operational strengths, but experiences uneven performance tied largely to staffing, consistency of routine care, and care-transition practices. Prospective residents and families should consider an in-person tour, ask for current staffing ratios and average call-bell response times, review meal-accommodation practices, inquire about infection-control and pest-control protocols, and request specifics about discharge planning and equipment availability. Verifying how the facility documents and resolves clinical incidents, and asking for recent inspection or quality-data summaries, can help families weigh the well-documented clinical strengths against recurring operational concerns.

    About Puget Sound Care

Puget Sound Care Center sits in Olympia, WA, and has been serving the Puget Sound area since 1989 as a skilled nursing facility owned by Caldera Care with 108 licensed beds. The place offers both long-term nursing care for people who can't live at home or in assisted living and short-term rehab services, so you'll find rooms that are private or semi-private and folks recovering from illness, injury, or surgery as well as those staying for ongoing care. Residents get 24-hour care from licensed and certified nurses, therapists, healthcare aides, and a medical director oversees all medical care, plus there's a director of nursing, a director of rehab, and a director of social services to help things run smoothly. Therapies on site include physical, occupational, speech, and respiratory therapy for those who need help regaining strength, independence, or communication after a setback, and there's even outpatient rehab if you don't need to stay overnight. There's specialized dementia and memory care with extra personalized attention and a senior Paw-sitive Care Provider is around too, which gives the place a little warmth. You'll find activity programs, fitness and recreation areas, and life enrichment programs, while meals and amenities aim to make things comfortable, though specific details aren't always listed. The building is single-story, which can be easier for getting around. The staff can also manage wound care, nutritional support, and case management, and they help plan out care and get you ready for a safe move back home if needed. Medicare and Medicaid certify their beds, so folks using those programs will find coverage here, and they contract with several insurance providers to help with care planning and costs. The facility takes a broad approach, offering personalized care plans, compassionate end-of-life care, respite care if you need a short break, and subacute services with skilled nursing, making sure everyone gets the attention suited to their situation, and there's always someone who can speak English. There's no public reviews or awards mentioned right now, but they've got a steady history of serving seniors and supporting their different care needs.
