    Compassionate staff and excellent rehab

I'm very pleased - the staff were warm, compassionate, and treated my husband like family; nurses, CNAs and therapists consistently went above and beyond and the DON was visible on the units. The facility is clean, updated and feels like an upscale hotel with private rooms, excellent rehab outcomes, lovely dining and lively activities (church services, live music, memorable holiday meals). Transparent staffing data and favorable staff ratios gave me confidence; I highly recommend it for the golden years.

    Summary of reviews

Reviews exhibit a polarized pattern: many accounts praise the facility’s physical environment, therapy services, and individual caregivers, while others raise repeated operational and conduct-related concerns. The building and rooms are frequently described as recently renovated and upscale, with private bathrooms and in-room entertainment that support resident comfort. Rehabilitation and therapy services receive consistent positive comment for effectiveness and outcomes. Several reviewers also highlighted warm, compassionate interactions from specific nurses, CNAs and therapists, as well as visible leadership presence on units and availability of spiritual programming.

Care quality is uneven in practice: while therapy and individual caregivers are often lauded, there are recurring observations of delayed responses to call lights, inconsistent bathing and incontinence-care schedules, and lapses in routine bedside care and linen changes. Medication and prescription management problems — including delays, running out of medications, and perceived errors — were noted and contribute to concerns about clinical reliability. High staff turnover and understaffing are recurring themes that reviewers link to these inconsistencies and to longer wait times for assistance.

Dining and nutrition present contrasting impressions. The facility stages well-received special events and holiday meals with live music and decorative presentation, which many families praised. At the same time, routine meal service is described as inconsistent: late trays, cold food, portion and diet-order mismatches, and variable preparation quality were raised as operational weaknesses that affect daily resident experience.

Facilities and activities benefit from the renovation: common areas are described as clean and attractively maintained, and staff assist residents with dressing and participation in programs. However, there are mentions of periods with restricted visitation and limited activities, and some reviewers reported delayed maintenance and equipment repairs that affected residents’ mobility or comfort.

Management and administration show both strengths and vulnerabilities. Positive notes include transparent staffing data and visible nursing leadership; negative indicators include lapses in communication with families, coordination problems during transitions and discharges, supply shortages, billing and insurance errors, and instances of concerning staff conduct or tone. There are also serious governance-related concerns in the record, including allegations of theft and documentation irregularities that suggest a need for careful review of compliance and oversight.

For prospective residents and families: the facility offers strong physical amenities and effective therapy programs, but operational reliability varies. It is advisable to verify current staffing levels and turnover, observe routine meal service, review recent inspection or regulatory actions, confirm billing and record-keeping procedures, and ask about protocols for maintenance response, medication management, and family communication before making placement decisions.

    About Charleston Healthcare Center

Charleston Healthcare Center sits at 3819 Chesterfield Ave in Charleston, West Virginia, and operates as a skilled nursing facility that belongs to CommuniCare Health, which is one of the nation's larger post-acute care providers and a family-owned for-profit company, so you're looking at a place that's handled by people who have many other facilities across the country. The center can serve up to 184 residents and has caregiving staff on duty 24 hours a day, every day of the week, offering short-term and long-term services, medical care, social programs, and help with daily life. Charleston Healthcare Center's staff can provide wound care, hospice care, palliative care, psychiatric and psychoanalyst services, and they use hospital equipment and supplies as needed, plus residents can access physical therapy, occupational therapy, and speech therapy when needed, and there's respite care for family caregivers who need a break or when someone returns home after surgery.

English is the main language spoken among staff, the staff works to involve both residents and families in planning care, and they do their best to maintain personalized care plans for everyone living there, including for folks needing memory care for Alzheimer's and dementia, with secure areas and regular mental stimulation activities. Residents get help with personal care and have access to emotional and physical well-being support, plus they can join in plenty of activities like bingo, music, food events, arts and crafts, and holiday gatherings for entertainment and social time. Charleston Healthcare Center aims to offer a comfortable environment similar to what you'd find at home, and they do focus on letting people keep as much independence as they can.

However, it's important to point out that the federal Centers for Medicare & Medicaid Services (CMS) have given Charleston Healthcare Center a low overall rating of 1 star, showing they've struggled to meet some expected standards, and in recent years the facility's gotten violations and citations tied to staffing, cleanliness, and the way they give care, plus there have been fines related to broken rules, slow follow-up to resident complaints, and outdated paperwork. Charleston Healthcare Center offers a wide range of health and social support services but if you're looking at options, know the facts about their ratings and past issues.
