    Summary of reviews

Reviews of Seneca Trail Healthcare Center present a mixed but consistent set of themes. Many families and visitors emphasize the facility's strengths: staff are frequently described as caring, respectful, and experienced, with examples of attentive, hands-on assistance such as feeding help. The activity program is highlighted as varied and engaging, with regular offerings (bingo, crafts, movies, outings) and outdoor amenities such as a patio and access to community events. Several reviewers praised dining quality and an overall friendly atmosphere that supports resident companionship.

At the same time, multiple operational and quality-control concerns emerge. The most commonly cited issues are inconsistent responsiveness to call bells and family phone inquiries and delays in attending to resident requests. These concerns are closely linked to accounts of inadequate supervision for residents with confusion or high-dependency needs, and to perceptions of a congested nursing-station workflow that may limit timely staff response. Privacy and unit-layout limitations—including shared spaces and doors left open—were also noted as affecting resident comfort and separation.

Facility maintenance and cleanliness observations are mixed: while some reviewers describe clean, odor-free areas, others raised sanitation and odor concerns and described uneven performance by maintenance staff. A few reviewers reported missing personal belongings, which points to gaps in personal-item management and security that prospective families may wish to address directly. Communication experiences were variable as well: several families found staff informative and helpful, while others experienced unclear, unhelpful, or unresponsive phone interactions and inconsistent communication tone.

Taken together, the pattern suggests a facility with clear strengths in caregiving culture, activities, and social environment, paired with operational weaknesses that affect consistency of care and family confidence. Prospective residents and families would benefit from an on-site visit focused on current staffing levels and response times, unit layout and privacy arrangements, procedures for handling personal items, and examples of how the facility addresses past concerns about responsiveness and cleanliness.

    About Seneca Trail Healthcare Center

Seneca Trail Healthcare Center sits at 1115 Maplewood Avenue in Lewisburg, West Virginia, and has room for up to 80 residents, usually about 77 people live there each day, and it offers skilled nursing, memory care, hospice, assisted living, and rehabilitation services for seniors. This place holds both SNF and NF dual certification, so it can take care of people with different needs, and it takes Medicare, Medicaid, and private pay, making it possible for people from many backgrounds to come here, and the management has been handled by Health Care Facility Management, LLC since April 2023, all under the legal name GB WV Maplewood Leasing Co., LLC, and it's affiliated with Communicare Health. The facility runs as a licensed and certified nursing home, and gets regular inspections for life safety, infection control, nutrition, and dietary practices, so it's subject to surveys for complaints and recertification. In the most recent standard inspection from September 10, 2024, there were 9 deficiencies found, especially related to infection control and handling of garbage, and over time, a total of 25 deficiencies have been reported in different inspection reports, so while the place keeps up with federal and state requirements, issues have been found in some areas. Nurse turnover here is about 34.7%, and nursing staff average 3.77 nurse hours per resident every day, which means staffing stays consistent with nursing home standards.

They have skilled nursing and intermediate care for those who are very frail or depend on others, as well as short-term rehabilitation for people moving between hospital and home, including post-surgical care and help with complicated medical needs. The staff includes medical experts who regularly check and update each person's care plan, meeting with residents and their families to make things personal and give thorough healthcare and social support, and they also offer physical, speech, and occupational therapy right there in the building. There's a PACE center called InnovAge Roanoke Valley running on site, and a physician-run clinic gives more medical help if it's needed. For people who have dementia or Alzheimer's, the center offers special support, including activities like crafts, bingo, pet therapy, and holiday parties to help with emotional and mental health, so residents can feel engaged and cared for. There are specialized services, hospital-level equipment, and private spaces focused on comfort, and the facility emphasizes a supportive environment for everyone who lives there. In addition, the team gives long-term care, palliative care, and hospice care for seniors with serious illnesses, working with residents and families to make each day as comfortable and meaningful as possible. Seneca Trail serves seniors from the Lewisburg area and around West Virginia, helping people who've reached a stage where they need more hands-on care and support to keep their best quality of life.
