    Caring staff, spotless facility, excellent

I had a very positive experience. The staff - especially Melissa, Tina and Brittany - were helpful, responsive and genuinely caring. The new, modern building is spotless and welcoming; my mom's large one-bedroom with full kitchen and patio overlooks a lovely courtyard, and the big common areas (fireplace, big-screen TV) feel very community-like. Food and proactive medication management were solid, and the team made the transition easy. Overall excellent with only minor room for improvement; I'd highly recommend.

    Summary of reviews

Reviews of Patriot Place Assisted Living & Memory Care are strongly mixed, showing a facility with attractive physical features and a polarized record on care and operations. Multiple comments highlight the building’s strengths: larger-than-average apartments with separate bedrooms and living areas, in-unit kitchenettes in some units, patio or courtyard access, and sizable, well-appointed common areas with a fireplace and welcoming entrance. Several families also described an initial positive admissions experience and staff members who provided helpful tours and transition support.

Care quality and staffing are the most prominent areas of concern. Many families described inconsistent direct-care performance: slow responses to call buttons, missed or delayed bathing and toileting, missed meals, and uneven attention to resident needs. Reviewers characterized the workforce as frequently young and inexperienced and noted high turnover, which reviewers linked to variability in daily care. There are consistent reports suggesting weaknesses in medication-management practices and controls, along with concerns about clinical oversight. A subset of families identified a small clinical leadership group as the de facto decision-makers; others questioned whether staff had adequate training and supervision for the level of resident needs.

Dining and housekeeping show a similar split. Several reviewers praised the dining spaces and reported good meals at times, while others described inconsistent meal delivery, occasional poor-quality offerings, and instances where residents missed meals. Housekeeping and sanitation were described as uneven: some described a clean, modern facility, while others raised sanitation concerns in particular common areas and rooms, inconsistent laundry practices, and pest-control or odor issues. Families also raised incontinence-care concerns and delays in routine hygiene assistance.

Management, communication, and safety processes generated notable commentary. Multiple reviewers expressed frustration with managerial responsiveness and perceived weak accountability at the ownership level, including concerns about billing for services that families said were not consistently provided. Several families reported filing regulatory complaints and found limited visible outcomes. Safety-related issues were also raised: fall incidents, at least one reported elopement/security lapse, and delays or inadequacies in incident response led families to question transfer and emergency protocols.

There are clear examples of positive staff interactions and successful resident placements alongside repeated operational shortcomings. Prospective families should weigh the facility’s appealing physical environment and the presence of licensed clinical staff against consistent themes of understaffing, variability in care practices, housekeeping inconsistencies, and concerns about medication and safety processes. If considering this community, recommended due diligence includes: meeting the current clinical leadership, clarifying staffing ratios and training programs, reviewing medication-management and incident-response protocols, asking for examples of how the facility addressed past family complaints, and touring during mealtime and staff-shift change to observe daily operations firsthand.

About Patriot Place Assisted Living & Memory Care

Patriot Place Assisted Living & Memory Care gives seniors both assisted living and memory care services, so folks get help for their daily needs and for memory challenges, too, and what they have there is a mix of safety and comfort, because they put in things like walk-in tubs, medical alert systems, secure doors, and even ID theft protection because kids these days worry about scams and all that. Residents get furnished rooms that come with phones, and the rooms have wheelchair accessible showers, so moving around isn't too hard, and there's always someone on staff, because the care team and nurses are trained to help twenty-four hours a day, all week, which means someone's there for bathing, dressing, transfers, and medication. For folks who need diabetic care, help with treatments or who need dementia waiver services, they handle that, too, and they even do hospice care for when things get hard.

Meals are set up three times a day in the dining room, and if someone's got diabetes or dietary restrictions, they'll adjust, so people don't have to fuss, and if someone wants, they can eat any time with the all day dining. Housekeeping and laundry get done every week, so the residents have clean spaces and clothes. You find walking paths outside, a garden to look at or sit in, and there's an arts room and a barber or salon spot, so you can get a haircut or just sit and visit. The common rooms inside and outside are places people gather, and the building's got a home security system, too.

The staff helps with transportation and parking, makes arrangements for health care and pharmacy deliveries, and helps line up visits from doctors or clinics. For folks with hearing and tech needs, the facility offers special internet plans, phones, hearing aids, and support for using those, which means older folks won't be left out of touch. There are memory care programs for people with Alzheimer's or other cognitive issues, with services and routines made just for memory needs, plus devotional activities on and off site, so if faith's important that gets supported, too.

Residence here can be short or long term, and the place is licensed by the State of Wisconsin Department of Health & Human Services as both a Community Based Residential Facility and a Residential Care Apartment Complex, so it follows state regulations for care and safety. Regular case management means someone pays attention to each person's needs and changes their plan if something new comes up. What stands out is the respectful care for people's physical, mental, emotional, and spiritual well-being, because the whole point is to keep folks safe, comfortable, and treated with dignity while they live there.
