    Loving professional care, clean facility

I placed my mom here and have been impressed by the loving, professional staff who treat residents like family and provide excellent care. The facility is clean, bright and home-like with roomy rooms, on-site therapy, engaging activities and a warm, social atmosphere - I'd recommend it.

    Summary of reviews

Sage Meadow of De Pere receives substantial praise for its staff, programming, and facility aesthetics, but reviews also reveal operational inconsistencies that families should evaluate during a tour.

Care and staff: Many reviewers describe staff as compassionate, professional, and attentive; residents and family members frequently praise individualized attention and tenderness from caregivers. The Activity Director and programming receive specific positive mention for engagement and variety. At the same time, staffing stability appears variable—turnover and inconsistent assignments are cited as factors that can affect continuity of care. Prospective families should inquire about current staffing ratios, staff tenure, and shift continuity to understand day-to-day reliability.

Dining and dietary services: The facility has an attractive dining room and generally positive presentation of meals; reviewers note a variety of menu options and occasional culinary highlights. However, there are recurring comments about uneven meal quality (notably lighter or less substantial offerings on some weekend days) and an emphasis on convenience items by some reviewers. There are also concerns about the facility’s capacity to accommodate complex or restrictive diets. Visitors should request sample menus, ask about weekend meal plans, and clarify processes for documenting and meeting individual dietary restrictions.

Activities and social life: Activity offerings are a clear strength. The community supports frequent programs, social interaction among residents, and a welcoming, home-like atmosphere. Garden access and occasional use of fresh produce contribute to a small-community feel. Observing an activity session and speaking with the Activity Director can provide a good sense of resident engagement and the daily rhythm.

Facilities and services: Physical spaces are consistently described as clean, updated, and welcoming, with updated furnishings, new paint and artwork, a single-floor layout, and wide hallways that support mobility. On-site physical therapy is available, which is an asset for rehabilitation needs. Nevertheless, some reviewers raise sanitation or odor concerns in particular areas, and housekeeping consistency—especially room cleaning—has been variable. Ask about housekeeping schedules, maintenance processes, and recent environmental surveys during a visit.

Management and communication: Experiences with management and corporate responsiveness are mixed. Positive comments reference supportive management and warm interactions among residents and staff. Conversely, there are examples of delayed responses to family concerns, missed follow-up calls, and perceived gaps in complaint escalation. Prospective families should clarify the facility’s complaint-resolution protocol, point-of-contact expectations, and how concerns are escalated to leadership.

Notable patterns and recommendations: Overall sentiment skews positive regarding staff demeanor, activities, and the community’s appearance, but recurring themes of inconsistency—particularly in staffing, housekeeping, meal service, and responsiveness—warrant targeted questions. When visiting, observe mealtime (including a weekend if possible), inspect a resident room for size and cleanliness, meet caregiving staff and the Activity Director, and ask management for recent staffing metrics and examples of how family concerns are resolved. These focused checks will help determine whether the facility’s strong points align with a prospective resident’s priorities and whether operational inconsistencies are being addressed.

About Sage Meadow of De Pere

Sage Meadow of De Pere Assisted Living and Memory Care sits in De Pere, Wisconsin, and offers a mix of care options for older adults, including assisted living, memory care, companion care, and some nursing resources, so residents have choices and specialized help as they age. The building has rooms and common spaces set up for wheelchairs and people who need help moving, and rooms come furnished or can be set up to fit the resident's needs, which gives folks a real sense of home without being overwhelming. There's always a staff member awake and around, ready to help 24 hours a day-especially important for residents who need transfers with mechanical lifts, regular diabetic care, or incontinence support, and their team has experience with insulin injections, sliding scale therapy, and other medical needs typical for older adults.

The team at Sage Meadow pays close attention to details, and the care level is steady at a medium level, meaning residents get steady support with daily living like bathing, dressing, managing medicines, and help getting around, but it isn't a heavy nursing home environment. Memory care is a focus here, with staff trained for Alzheimer's and dementia, and they have alerts on exits, secure outdoor spots, and personalized plans if someone's at risk of wandering or confusion. People who live here are aged 55 and up, and they find different ways to stay active, like card games, movie nights, trips into the community, and special events organized by a dedicated Activity Director, making sure everyone can be social whether they like quiet groups or bigger gatherings.

Meals are provided every day and can be adjusted for allergies and diets, with effort put into making them both tasty and healthy, so residents don't have to worry about meal prep and can instead eat together in the dining room or in their rooms. Extra features stand out for many folks, such as walking paths, gardens, transportation for medical appointments or errands, a beautician available on-site, and laundry services which help cut down worries about chores. There's a no-smoking policy in all indoor areas, but pets are allowed, and larger dogs are considered on a case-by-case basis.

Sage Meadow has thought about safety and technology, providing resources about hearing aids, senior cell phones (including Jitterbug), internet plans, home security, medical alert systems, and ID theft protection, making daily life smoother and safer for everyone. Residents have access to hospice services, respite care for those who need short-term support, move management for relocating, help with legal and financial questions, and even support for caregivers. There isn't any fuss with the amenities since the main goal is to deliver practical comfort-spaces to gather, easy transportation, and responsive staff, all recognized in a recent deficiency-free survey from the Wisconsin Department of Health Services. So, Sage Meadow offers steady, attentive care in a simple, pet-friendly, and safe space where residents can live as independently as possible and get the help they need with dignity, whether they're active or managing memory loss.
