    Caring, competent rehab helped me

I recovered from a knee replacement in the New Horizons rehab wing and was impressed - the rehab team, nurses and aides were caring and competent, PT/OT and the gym equipment helped me regain independence and get home sooner than expected. The facility is very clean, bright and family-like; staff were attentive, professional and warm, and I would choose this community again.

    Summary of reviews

Across the reviews there is a clear split between strong clinical rehabilitation and a set of operational concerns. The facility’s rehabilitation services receive repeated praise: physical and occupational therapy teams, well-equipped gym space, and goal-focused programs that helped some residents regain independence. Many reviewers highlighted attentive rehab staff who facilitate timely progress and successful discharges home.

Direct caregiving is frequently described as warm and compassionate. Numerous comments note friendly aides and nurses, round-the-clock availability, proactive dining assistance, and staff who create a family-like atmosphere. The building and common areas are consistently characterized as clean, bright, and home-like, with active programming (bingo, outings, a general store, and transportation to events) that supports resident engagement and social connection.

However, several reviewers described operational gaps that could affect safety and continuity of care. These include inconsistent staffing levels and delays in responding to call lights and requests, which some families linked to missed monitoring (for example, neurological checks and wound-care follow-up). There are also concerns about transfer practices and fall prevention; a pattern of fall-related incidents and handling issues suggests review of safe-transfer protocols may be warranted. Medication administration and early medication-management issues were also mentioned as areas needing attention.

Dining and food service present mixed experiences. Some families praised caring dining staff and adequate meal variety, while others described inconsistent food quality, cold or poorly prepared items, and portions or presentation that did not meet expectations. Several reviewers singled out individual dining staff for excellent service, which suggests strengths in staff engagement even when kitchen execution is uneven.

Communication and management practices are another recurrent theme. Positive accounts cite managers and nurses who meet family needs and correct problems, but other accounts describe coordination and documentation breakdowns (for example, appointment paperwork and pre-procedure preparation), as well as concerns about staff conduct and tone in interactions with families. There are also serious individual claims, including allegations of theft and property mishandling, which families should treat as matters for direct inquiry and verification with facility leadership.

Overall, Manitowoc Health & Rehabilitation Center presents as a facility with notable strengths in rehabilitation, engaged direct-care staff, and a welcoming physical environment. Prospective residents and families would be well served to observe peak and off-peak staffing, ask about fall-prevention and transfer training, review medication and wound-care protocols, and discuss communication and documentation procedures with management before making placement decisions.
